Does Medi-Cal Cover Zepbound? Sleep Apnea, Under 21, and Costs

Medi-Cal does not cover Zepbound for weight loss. As of January 1, 2026, the California Department of Health Care Services removed Zepbound from the Medi-Cal Rx Contract Drugs List, ending coverage for weight loss and every other weight-related use in adults 21 and older.¹ Two exceptions remain: Medi-Cal Rx will still review prior authorization requests for Zepbound prescribed for obstructive sleep apnea, and members under 21 can request coverage for weight-loss use through a prior authorization.²

What Medi-Cal Cut and What It Kept

Three GLP-1 drugs were dropped from the formulary entirely: Wegovy, Zepbound, and Saxenda. Pharmacy claims for any of them now return Reject Code 70, meaning not a covered service. Seven others — Ozempic, Rybelsus, Mounjaro, Victoza, Byetta, Bydureon, and Trulicity — stayed on the list but are now locked to a type 2 diabetes diagnosis code. Without that code on the claim, the pharmacy is denied.³

The rule applies the same way to Medi-Cal fee-for-service and to every Medi-Cal managed care plan. A managed care plan cannot decide on its own to keep covering Zepbound for weight loss.⁴

Coverage for Obstructive Sleep Apnea

The FDA approved Zepbound in December 2024 for moderate to severe obstructive sleep apnea in adults with obesity, the first drug approved for that condition.⁵ Because OSA is a separate indication, Medi-Cal Rx will still consider prior authorization requests when Zepbound is prescribed for it, reviewed one case at a time for medical necessity.⁶

A successful PA needs the OSA diagnosis code, clinically relevant lab and test results, and a record of other medications the patient has tried, with dates and outcomes. Requests submitted as “continuation of therapy” from a prior weight-loss prescription will be denied, and so will off-label weight-loss use unless the patient has type 2 diabetes.⁶

Coverage for Members Under 21

Federal law requires Medicaid programs to cover medically necessary treatments for children and adolescents under the Early and Periodic Screening, Diagnostic, and Treatment benefit. Because of that, Medi-Cal members younger than 21 can still request coverage for GLP-1 drugs, including Zepbound, for weight-loss indications. The prescriber submits a prior authorization establishing medical necessity, with clinical documentation such as lab results and a history of other treatments tried.⁷

A Note on Mounjaro

Zepbound and Mounjaro contain the same active ingredient, tirzepatide, but Medi-Cal treats them as separate products tied to separate indications. Mounjaro is still covered for type 2 diabetes, and unlike the Zepbound OSA pathway, no prior authorization is required — the diabetes diagnosis code on the prescription is enough.⁷

If You Were Already Taking Zepbound

There was no transition period. Every previously approved prior authorization for Zepbound expired on December 31, 2025. Members taking a GLP-1 for weight loss were supposed to receive a notification letter by November 1, 2025.²

If you receive a Notice of Action denying coverage, you have 90 days to request a State Hearing through the California Department of Social Services. If you were already taking the drug on or before January 1, 2026, and you file the hearing request within 10 days of getting the denial notice, you can keep receiving the medication while the hearing is pending. That “aid paid pending” coverage lasts until the original prior authorization period ends, a hearing decision is issued, or you withdraw the request, whichever happens first.⁸

Paying for Zepbound Without Medi-Cal

Options are limited. Eli Lilly’s savings card and copay programs exclude anyone enrolled in a state or federal healthcare program, which includes Medi-Cal.⁹ Zepbound is also not currently offered through the Lilly Cares Foundation patient assistance program.¹⁰

Eli Lilly does sell Zepbound directly to self-pay patients through LillyDirect. A one-month supply of the 2.5 mg starter dose starts at $299, and doses between 7.5 mg and 15 mg run $449 as long as the patient refills within 45 days. Miss that refill window and the price climbs, reaching up to $699 per month at the higher doses.¹¹ The savings card terms bar anyone in a government healthcare program, but the self-pay pricing is described separately in Lilly’s materials.

Compounded tirzepatide, which used to be a cheaper alternative, is largely gone. The FDA declared the tirzepatide shortage resolved in December 2024, ending the enforcement discretion that let pharmacies compound copies, and the agency reported 545 adverse events linked to compounded tirzepatide products as of mid-2025.¹² In April 2026 the FDA proposed removing GLP-1 receptor agonists from the list of substances outsourcing facilities can compound in bulk, which would tighten availability further.¹³

As of mid-2026, California officials have said they have no plans to reconsider the coverage cuts, and no bill has been introduced in the Legislature to restore Medi-Cal coverage of GLP-1 drugs for weight loss.¹⁴⁴
