Medi-Cal out-of-pocket reimbursement is available through a process called a Conlan claim: if you paid for a covered service during a period when you were eligible for Medi-Cal, you can file to get that money back. The state reviews your eligibility for the date of service, confirms the service was a Medi-Cal benefit, and either recovers the money from the provider or pays you directly. A complete claim takes up to 120 days to process once DHCS has everything it needs, and real-world turnaround often runs longer because the provider gets 30 days to respond before the state cuts you a check.
Which Bills Actually Qualify
Not every out-of-pocket medical expense is reimbursable. The Department of Health Care Services recognizes three eligibility windows, and your bill has to fall inside one of them.
- Retroactive period. The three months immediately before the month you applied. This only applies if you specifically requested retroactive coverage and your county eligibility worker or Medi-Cal approved it. If you never asked for retro coverage, expenses from before your application date are not reimbursable, even if your income would have qualified you.
- Evaluation period. From the date you submitted your application until the date your Medi-Cal card was issued. For services on or after February 2, 2006, the provider must have been enrolled as a Medi-Cal provider on the date of service.
- Post-approval period. After your card was issued. This covers a provider mistakenly charging you despite active coverage, plus excess copayment and excess share-of-cost charges. Again, the provider must have been a Medi-Cal provider on that date.
You also have to have been medically and financially eligible in the specific month the bill was incurred. The state checks income and assets for that month, not just overall.1Department of Health Care Services. Medi-Cal Out-of-Pocket Expense Reimbursement (Conlan)
The retroactive window trips people up. Approval does not automatically reach back three months; you have to request it. If you skipped that step at application, contact your county eligibility worker about adding retroactive coverage before filing a Conlan claim for pre-application bills.
What Services Are Covered
Reimbursement is limited to services inside Medi-Cal’s benefit package that meet medical necessity. That includes emergency room visits, physician services, outpatient surgery, prescription drugs, hospital stays, mental health treatment, substance use treatment, lab work, and dental care under Denti-Cal. Medical and dental expenses go through the same claim procedure.1Department of Health Care Services. Medi-Cal Out-of-Pocket Expense Reimbursement (Conlan)
Services outside the covered benefits list will not be reimbursed, no matter how much you paid. Elective cosmetic procedures and dental work not included in Denti-Cal are examples. For any service that would normally require prior authorization, you’ll need documentation from your provider showing the care was medically necessary.2Department of Health Care Services. How Much You Get Back
The amount depends on whether DHCS can recover the money from the provider. Three outcomes are possible:
- Voluntary provider refund. The provider agrees to refund the full amount, usually sending it directly to you.
- Involuntary recoupment. The provider refuses, so Medi-Cal recovers the full amount through recoupment and pays you the full amount you spent.
- Medi-Cal rate reimbursement. If DHCS cannot recover from the provider at all, because the provider has closed, was never enrolled, or does not bill enough to allow recoupment, the state pays you directly but only up to the Medi-Cal allowed rate for that service. That rate is usually lower than what you paid, so you may not get the full amount back.
Reimbursement never exceeds what you actually paid.1Department of Health Care Services. Medi-Cal Out-of-Pocket Expense Reimbursement (Conlan)
What to Send and Where
A complete Conlan claim packet includes:
- Medi-Cal Claim Form for Beneficiary Reimbursement (DHCS 4521). You can fill it out online at the DHCS website and print it, or print a blank copy and complete it by hand.
- Payee Data Record (STD 204). State law requires this before any refund can be issued, and claims routinely stall because people leave it out.3Department of Health Care Services. Payee Data Record (STD 204)
- A copy of your Medi-Cal Beneficiary Identification Card (BIC).
- Proof of payment: a canceled check (front and back), a provider receipt, electronic payment evidence, or a copy of a money order. If you paid cash and have no bank record, a written declaration may be accepted to supplement your other documents.
- An itemized billing statement showing the date of service, the specific services or service codes, and the amount you paid.
- Prior authorization documentation, only if the service would have required Medi-Cal authorization. Your provider supplies this.

The Filing Deadline
Your packet has to be received within one year of the date of service or within 90 days of the date your Medi-Cal card was issued, whichever is later.5DHCS – CA.gov. Conlan Frequently Asked Questions If your application took a long time to process and your card arrived eleven months after a service, you still have 90 days from the card date to file. Don’t hold a claim waiting for perfect documentation when a deadline is closing in; file what you have and respond to any follow-up requests from DHCS as they come.
What Happens After You File
Once DHCS has a complete and valid claim, processing takes up to 120 days. During that window the state checks your eligibility for the date of service, confirms the service is a covered benefit, and contacts the provider. If the claim is approved, the provider gets a letter and has 30 days to issue a voluntary refund. If the provider doesn’t respond, DHCS sends the reimbursement check to you directly, usually within seven to eight weeks after the 30-day window closes.5DHCS – CA.gov. Conlan Frequently Asked Questions
Watch your mail. DHCS may send letters asking for additional information or clarification, and a slow response is the easiest way to stall your own claim.
If Your Claim Is Denied
A denial comes with a written notice explaining the reason, the regulation or criteria used, and any clinical rationale. You can request free copies of everything DHCS relied on to make the decision.6DHCS. NOABD – Payment Denial Notice
You have 90 days from the date of the notice to request a state fair hearing through the California Department of Social Services.7CDSS.ca.gov. State Hearing Requests You can represent yourself or bring a lawyer, a relative, a friend, or any other person you choose.8Cornell Law School – Legal Information Institute. Fair Hearing Related to Denial, Termination or Reduction in Medical Services
Common denial reasons are missing documentation, services that aren’t on the covered benefits list, and eligibility gaps on the date of service. If the problem was incomplete paperwork rather than a fundamental eligibility issue, resubmitting a corrected claim is often faster than a hearing.
Two Situations the Conlan Process Doesn’t Fully Cover
If you have both Medicare and Medi-Cal, providers cannot bill you for Medicare deductibles and coinsurance; those get billed to Medi-Cal. If a provider collected those amounts from you, they must refund the money once they receive Medi-Cal’s payment confirmation. Qualified Medicare Beneficiaries cannot be billed any residual amount at all.9Medi-Cal. Medicare/Medi-Cal Crossover Claims Overview If a provider refuses to refund Medicare cost-sharing amounts they wrongly collected, you can raise the issue through the Conlan process or by complaining to your Medi-Cal managed care plan.
If your monthly share of cost was calculated too high and later reduced retroactively, that refund runs through your county, not through Conlan. Your county prepares a Share of Cost Medi-Cal Provider Letter (form MC 1054), which you give to the provider; the provider then refunds you up to what you actually paid once they receive Medi-Cal’s payment for the difference.10DHCS Medi-Cal Eligibility Procedures Manual. 12C – Processing Cases When a Share of Cost Has Been Reduced Retroactively If your share of cost dropped to zero after the adjustment, the MC 1054 letter is the only route to get your money back from the provider.
