Medicaid does cover the Dexcom G7 in most states, but whether you personally qualify depends on your state’s rules, your diagnosis, and whether your plan treats continuous glucose monitors as a pharmacy benefit or as durable medical equipment. As of mid-2025, 45 states and the District of Columbia cover continuous glucose monitors through Medicaid, and the Dexcom G7 is specifically listed as a covered or preferred device in many of those programs.1CHCS. Who Qualifies
There is no single national Medicaid rule for CGM coverage. Each state writes its own, and managed care plans within a state can add requirements on top. Still, most state programs share the same core criteria, which closely track Medicare’s.2ADCES. Standard Coverage Policy for CGMs
- A confirmed diagnosis of type 1, type 2, or gestational diabetes. A few states have historically limited coverage to type 1, though that has been loosening.
- Insulin treatment, often multiple daily injections or pump use. Some states also cover patients who are not on insulin but have documented problematic hypoglycemia.
- A face-to-face or telehealth visit with the prescribing provider, typically within six months of the request and periodically thereafter.
- Confirmation that the patient or caregiver has been trained on the device.
Ohio runs one of the broadest programs, covering CGMs for all people with diabetes regardless of what medications they take.3JMCP. Continuous Glucose Monitor Access for Medicaid Beneficiaries Texas revised its criteria effective November 2025 to extend coverage to non-insulin users who have frequent problematic hypoglycemia, unexplained glucose fluctuations, or episodes of ketoacidosis.4TMHP. Pharmacy Benefit or Durable Medical Equipment
One of the biggest practical differences between states is how the benefit is routed. When CGMs are a pharmacy benefit, you can usually pick up sensors at a retail pharmacy with a prescription, the way you’d fill a medication. When they run through DME, you go through a medical supply company, with more paperwork and often longer waits.
As of July 2025, 33 state fee-for-service Medicaid programs classify CGMs as a pharmacy benefit, and more states are expected to follow.2ADCES. Prior Authorization
Prior authorization is the norm. Your provider has to send in documentation and get approval before Medicaid will pay. For an initial authorization, that typically means a prescription dated within the last 12 months, documentation of the diabetes diagnosis, records of a recent face-to-face evaluation, and confirmation of device training. In North Carolina, the initial authorization lasts six months, after which the provider has to show that the patient is actually using the device and that glycemic control has improved or held steady.7Carolina Complete Health. CGM Devices Authorization Guidelines
A handful of states have dropped prior authorization for preferred devices. Indiana puts CGMs on its Preferred Diabetes Supply List with no PA required. Ohio covers CGMs on its preferred diabetic supply list through the pharmacy benefit without prior authorization, though providers still need to document a qualifying appointment within six months.5ADCES. Is the Dexcom G7 a Preferred Device
Where the G7 sits on a state’s preferred drug list or preferred diabetic supply list affects both paperwork and copays. Preferred devices generally move fastest. Non-preferred devices may trigger step therapy, meaning you have to try a preferred product first, document that it did not work for you, and only then get access to the non-preferred option.
Several states list the Dexcom G7 as preferred. Ohio’s 2026 Preferred Diabetic Supply List names the Dexcom G7 sensor, receiver, and 15-day sensor alongside FreeStyle Libre and Medtronic products.10Ohio Department of Medicaid. Preferred Diabetic Supply List Minnesota’s uniform preferred drug list, effective January 2025, includes both the Dexcom G6 and G7 as preferred, with no CGMs listed as non-preferred.11Minnesota DHS. Pregnancy and Children
At least 15 states include gestational diabetes as a qualifying diagnosis for CGM coverage. How long coverage lasts after delivery varies. California’s Medi-Cal covers CGMs for the pregnancy and up to 12 months postpartum.5ADCES. Medicaid Coverage Overview CGMs Dexcom Illinois also extends coverage 12 months postpartum.12Illinois HFS. Children have a stronger claim than adults in most states. Under the federal Early and Periodic Screening, Diagnostic, and Treatment benefit, states have to cover any medically necessary service that corrects or ameliorates a health condition for Medicaid-enrolled people under 21, even if that service isn’t in the standard adult benefit package.14Medicaid.gov. What You’ll Pay
Medicaid cost-sharing is capped by federal rules. For beneficiaries with household income at or below 150 percent of the federal poverty level, copayments are limited to nominal amounts, meaning no more than $4 for preferred drugs and $8 for non-preferred drugs under current rules. Total premiums and cost-sharing for a Medicaid household can’t exceed 5 percent of family income.17MACPAC. Cost Sharing and Premiums Pregnant women and most children pay nothing.18Medicaid.gov. How to Confirm Coverage and What to Do If You’re Denied
Call the state Medicaid agency or the managed care plan listed on your Medicaid card. Managed care organizations in many states have to follow the state’s preferred drug list, but they often run their own authorization processes with their own timelines.5ADCES.
