Louisiana Medicaid does cover Mounjaro, but only when it is prescribed for Type 2 diabetes. The program will not pay for Mounjaro as a weight-loss drug. Even for diabetes, every prescription requires prior authorization, a documented diagnosis, and a qualifying hemoglobin A1C result.1Louisiana Medicaid. GLP-1 and A1C Clinical Authorization Criteria
Mounjaro (tirzepatide) is FDA-approved as an add-on to diet and exercise to improve blood sugar control in adults and children age 10 and older with Type 2 diabetes. The Mounjaro brand itself is not approved for obesity; the weight-management version of tirzepatide is sold separately as Zepbound.2FDA. What You Need To Qualify for Mounjaro Coverage
Since November 15, 2024, every GLP-1 receptor agonist in Louisiana Medicaid requires clinical authorization. The rule applies the same way in fee-for-service Medicaid and in all four managed care plans: Aetna Better Health, AmeriHealth Caritas, Healthy Blue, and Louisiana Healthcare Connections.1Louisiana Medicaid. GLP-1 and A1C Clinical Authorization Criteria
Your prescriber has to document three things:
- A diagnosis of Type 2 diabetes mellitus.
- A hemoglobin A1C of 6.5% or higher drawn within the past six months. If your current A1C is below 6.5%, a prior A1C at or above that threshold on record is enough.
- That you meet the minimum age requirement verified through the pharmacy system.
You do not have to try metformin first. Louisiana does not require that step before a GLP-1 drug.1Louisiana Medicaid. GLP-1 and A1C Clinical Authorization Criteria
An approval is good for 12 months. To renew, your prescriber confirms you are established on Mounjaro and showing a positive response to therapy.1Louisiana Medicaid. GLP-1 and A1C Clinical Authorization Criteria
If Mounjaro Is Treated as Non-Preferred
Tirzepatide does not appear by name on the Louisiana Medicaid Preferred Drug List documents.4Louisiana Department of Health. Louisiana Medicaid Preferred Drug List If Mounjaro is handled as non-preferred, your prescriber must also document one of the following before coverage will go through:
- Treatment failure with at least one preferred GLP-1 product.
- Intolerable side effects from at least one preferred GLP-1 product.
- Documented contraindications to all preferred GLP-1 products appropriate for your condition.
- No preferred product is appropriate for your condition.
How the Prior Authorization Gets Submitted
Your prescriber files the request, not you. Louisiana Medicaid accepts prior authorizations through four channels:
- Online through the electronic prior authorization system at lamedicaid.com or CoverMyMeds.com.
- By phone at 1-866-730-4357 for the state PA unit, or 1-800-424-1664 for the Magellan pharmacy help desk serving managed care.
- By fax at 1-866-797-2329.
- By mail to ULM College of Pharmacy, Rx PA Program, 1800 Bienville Drive, Monroe, LA 71201.
Once complete documentation arrives, a decision has to be made within 24 hours. If the request is denied, your prescriber can request a phone consultation with a physician reviewer, and a final determination follows within 48 hours of that call. The PA unit operates Monday through Saturday, 8 a.m. to 6 p.m. Central Time.5Louisiana Medicaid. Pharmacy Prior Authorization Instructions
Mounjaro Is Not Covered for Weight Loss
If you do not have Type 2 diabetes, Louisiana Medicaid will not approve Mounjaro, no matter your BMI or related conditions. The authorization criteria are tied to a diabetes diagnosis and A1C evidence; there is no obesity pathway for the Mounjaro brand.
Two narrow exceptions exist for other tirzepatide and semaglutide products, and neither is a general weight-loss benefit:
- Zepbound (the weight-management version of tirzepatide) is covered only for adults 18 or older with moderate to severe obstructive sleep apnea, a BMI of 30 or higher, an apnea-hypopnea index of 15 or higher on a recent sleep study, and ongoing use of positive airway pressure therapy unless contraindicated. Patients with Type 1 or Type 2 diabetes are excluded, and a signed treatment agreement is required.6Louisiana Department of Health. Zepbound Clinical Authorization Criteria7Louisiana Department of Health. Zepbound Treatment Agreement for Louisiana Medicaid Recipients
- Wegovy (semaglutide) is covered for people 45 and older with established cardiovascular disease and a BMI of 27 or higher, or for patients with metabolic dysfunction-associated steatohepatitis (MASH) at stage F2 or F3 fibrosis prescribed by or with a gastroenterologist or hepatologist. Patients with diabetes are excluded from both pathways.8Louisiana Department of Health. Wegovy Clinical Authorization Criteria
A 2027 Expansion Is Coming
Governor Jeff Landry signed Senate Bill 433 into law as Act 898 on June 9, 2026. Starting January 1, 2027, Louisiana Medicaid will be required to cover FDA-approved weight-loss medications for adults 18 and older who meet one of two criteria:9TrackBill. Louisiana Senate Bill 433
- BMI of 30 or higher with at least one related comorbidity, such as Type 2 diabetes, prediabetes, hypertension, or cardiovascular disease.
- BMI of 35 or higher, regardless of comorbidities.
The statute refers to FDA-approved weight-loss medications generically, so it covers Zepbound, Wegovy, and any other qualifying agents. Prior authorization is limited to verifying eligibility, and step therapy is prohibited, meaning the state cannot force you to try a cheaper drug first. Initial approvals run six months, and continuation requires documented clinical improvement.10Louisiana State Legislature. SB 433 Enrolled Text
One caveat worth knowing: the law includes a $3 million General Fund appropriation and provides that the program only proceeds if the legislature funds it.11Louisiana State Legislature. If Your Prior Authorization Is Denied
You have appeal rights. If you are in a Medicaid managed care plan, start by appealing through your plan’s member services. If the plan upholds the denial, it has to tell you how to request a State Fair Hearing.12Louisiana Department of Health. How to Appeal Medicaid
Your prescriber can also submit a reconsideration with additional clinical records within 30 days of the initial denial. If that reconsideration fails, you can file for a State Fair Hearing online, by mail, by fax, or by phone. Filing within 10 days of the denial notice keeps existing services in place during the review, and a final decision usually comes within 30 days.
