Whether Medicaid covers Wegovy for weight loss depends entirely on the state you live in. As of January 2026, only 13 state Medicaid programs pay for GLP-1 drugs like Wegovy when they’re prescribed specifically to treat obesity, and that number has been falling.1KFF. Medicaid Coverage of and Spending on GLP-1s The other 37 states and the District of Columbia don’t. Even where coverage exists, you’ll almost always need prior authorization and have to meet specific clinical criteria.
Why Your State Gets to Decide
Federal Medicaid law generally requires states to cover nearly all FDA-approved outpatient drugs, but Section 1927(d)(2) of the Social Security Act carves out a short list of exceptions. Drugs used for “anorexia, weight loss, or weight gain” are on that list.2JAMA Network. Medicaid Coverage of Antiobesity Medications Each state Medicaid program chooses whether to pay for Wegovy, Zepbound, Saxenda, and similar drugs when the prescription is written for weight loss alone.
The same drug prescribed for a different reason is treated differently. When Wegovy is prescribed to reduce cardiovascular risk in adults with established heart disease, or when Zepbound is prescribed for moderate to severe obstructive sleep apnea, Medicaid programs have to cover it, because those uses fall outside the weight-loss exclusion.1KFF. Where Coverage Stands Right Now
In October 2025, 16 state Medicaid fee-for-service programs covered GLP-1s for obesity. By January 2026, that number was 13. Four states dropped coverage during that window, and one dropped it and then put it back.1KFF. Medicaid Coverage of and Spending on GLP-1s
States That Ended Coverage on January 1, 2026
Four states cut Medicaid coverage of GLP-1s for weight loss effective January 1, 2026: California, New Hampshire, Pennsylvania, and South Carolina.1KFF. Medicaid Coverage of and Spending on GLP-1s Each cited budget pressure.
California’s Medi-Cal program ended coverage after state officials projected that keeping it would cost nearly $800 million a year within four years. The change was written into the state’s budget law, SB 101. Coverage continues for GLP-1s prescribed for type 2 diabetes, cardiovascular disease, and other non-weight-loss indications, and Medi-Cal members under 21 can still receive the drugs for weight loss.3KFF Health News. Pennsylvania ended adult Medicaid coverage for weight-loss GLP-1s on the same date. The Shapiro administration said GLP-1 prescriptions for Medicaid recipients had grown from $223 million in 2022 to $650 million in 2024, with costs projected to double again in 2025. The state expects to save around $380 million through the end of the next fiscal year.5Spotlight PA. South Carolina added weight-loss GLP-1 coverage in November 2024, then reversed and eliminated it effective January 1, 2026, citing unsustainable budget pressures.7Milliman. Evolving Landscape of Obesity Coverage in Medicaid Coverage for type 2 diabetes continues.
North Carolina Restored Coverage
North Carolina ended coverage for GLP-1s used solely for obesity on October 1, 2025, amid a $319 million funding shortfall. At the time, about 43,500 of the state’s 91,000 Medicaid recipients on GLP-1s were taking them specifically for weight management.8The News & Observer. North Carolina Medicaid Cuts Effective October 2025 The state reinstated coverage on December 12, 2025, and returned Wegovy to the Preferred Drug List with prior authorization.9NC Medicaid. States Tightening or Restricting Access
Michigan has not eliminated coverage, but as of January 2026 only patients with a BMI of 40 or higher who have tried and failed other weight-loss medications like phentermine and Qsymia qualify. The state projects $240 million in savings from the tighter criteria.10University of Michigan Medical Research. Michigan Medicaid’s New Limits on GLP-1 Weight Management Medications
Rhode Island is moving in the same direction. Governor Dan McKee proposed ending GLP-1 coverage for weight loss in the FY2027 budget, and state lawmakers approved a budget restricting coverage to individuals with type 2 diabetes. It was sent to the governor’s desk in June 2026.11News From the States. The Case for Maintaining Medicaid Coverage of GLP-1s to Treat Obesity in Rhode Island
A few states don’t cover newer GLP-1 anti-obesity drugs at all. New Mexico’s Medicaid fee-for-service program lists Wegovy, Saxenda, and Zepbound as “not covered,” though it does cover older weight-loss drugs like phentermine and orlistat with prior authorization.12New Mexico Human Services Department. What You Have to Meet to Get Approved
In states that do cover Wegovy for weight loss, approval is never automatic. Every state that covers it uses some form of utilization control, usually prior authorization.13Healthcare Dive. Medicaid State Coverage of GLP-1s for Obesity The specifics vary a lot, but requirements generally include:
- A minimum BMI, often 30 or higher, or 27 and above with qualifying comorbidities. Michigan currently requires a BMI of 40.
- Documentation of prior weight-loss efforts, including caloric restriction, exercise, or trials of other weight-loss medications.
- Qualifying comorbid conditions such as hypertension, diabetes, cardiovascular disease, or sleep apnea.
- Weight-loss benchmarks for renewal, often requiring at least 5% weight loss from pre-treatment body weight to continue the drug after the initial authorization period.
Kentucky’s criteria show how narrow a pathway can be. To get Wegovy approved, a patient has to be at least 45 years old with a BMI of 27 or greater, have documented cardiovascular disease, and already be on optimized treatment with medications like statins and blood pressure drugs. Patients with diabetes are excluded from this pathway, because they’d get a diabetes-indicated GLP-1 instead.14Kentucky Portal. Children and Teens Under 21 Are Protected
Federal law treats people under 21 differently. Under Medicaid’s Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) benefit, states have to cover any medically necessary treatment for enrollees under 21, even treatments they exclude for adults.15MACPAC. EPSDT in Medicaid A state can’t blanket-deny Wegovy for a child or teenager just because it doesn’t cover the drug for adult weight loss. If a plan denies coverage for someone under 21, it has to make an individualized medical necessity determination and give a specific reason for the denial.6Pennsylvania Health Law Project. When Wegovy Is Covered for Something Other Than Weight Loss
If your state doesn’t cover Wegovy for weight loss, you may still be able to get it covered under a different diagnosis. All state Medicaid programs are required to cover GLP-1 medications when they’re prescribed for FDA-approved indications outside the weight-loss exclusion:
- Type 2 diabetes, which keeps drugs like Ozempic, Mounjaro, and Trulicity on every state Medicaid formulary.
- Cardiovascular risk reduction in adults with established cardiovascular disease and obesity or overweight. Wegovy is FDA-approved for this use.
- Moderate to severe obstructive sleep apnea in adults with obesity, which is an FDA-approved indication for Zepbound.
- Noncirrhotic metabolic dysfunction-associated steatohepatitis (MASH) with moderate to advanced liver fibrosis, which Wegovy is also approved to treat.
Pennsylvania’s bulletin ending weight-loss coverage specifically confirmed that GLP-1s remain available with prior authorization for each of those conditions.16Pennsylvania Department of Human Services. Medical Assistance Bulletin on GLP-1 Coverage If you have obesity and one of these diagnoses, talk with your provider about whether the prescription can be written for the non-weight-loss indication.
If Coverage Is Denied
You can appeal. In every state, Medicaid recipients have the right to request a fair hearing to challenge a denial. California gives members 90 days from the notice of action to request a state hearing, and members who were already on the drug when coverage ended may be able to keep receiving it while the hearing is pending.4Medi-Cal Rx. GLP-1 Coverage Changes Pennsylvania’s policy also includes a physician override: if a physician reviewer finds a GLP-1 medically necessary for a patient who doesn’t meet the standard criteria, the prior authorization can still be approved.16Pennsylvania Department of Human Services. Medical Assistance Bulletin on GLP-1 Coverage For an enrollee under 21, any denial based on a blanket coverage rule rather than an individualized medical necessity review is legally vulnerable under EPSDT.15MACPAC. EPSDT in Medicaid Request the denial reason in writing, ask your prescriber to document medical necessity, and file the appeal within your state’s deadline.
