Michigan Medicaid does cover Wegovy, but as of January 1, 2026, coverage for weight loss is sharply limited. To get Wegovy prescribed solely for obesity, you must have a body mass index of 40 or higher, have tried and failed other weight-loss medications, and have a prescriber attesting the drug is being used to avoid bariatric surgery. If Wegovy is prescribed for a non-obesity reason the FDA has approved it for, such as reducing cardiovascular risk or treating severe liver disease, the stricter rules do not apply and coverage is easier to obtain.
Who Qualifies for Wegovy as a Weight-Loss Drug
Under the criteria MDHHS set out in Numbered Letter L 25-73, a beneficiary seeking Wegovy for obesity must meet all three of the following:1Michigan.gov. Numbered Letter L 25-73 – Update of Pharmacy Drug Coverage for Treatment of Obesity
- A BMI of 40 or higher (morbid obesity).2University of Michigan Medicine. Expert Q&A: Michigan Medicaid’s New Limits on GLP-1 Weight Management Medications
- Documented trial and failure of all other clinically appropriate weight-loss interventions, including the preferred anti-obesity medications on the state formulary.1Michigan.gov. Numbered Letter L 25-73 – Update of Pharmacy Drug Coverage for Treatment of Obesity
- A prescriber attestation that the medication is being used to avoid higher-cost bariatric surgery.2University of Michigan Medicine. Expert Q&A: Michigan Medicaid’s New Limits on GLP-1 Weight Management Medications
The step-therapy piece is where most requests will stall. Prior authorization criteria updated June 1, 2026 require prescribers to document trial and failure of medications across five categories: benzphetamine, diethylpropion, orlistat products, phendimetrazine, and phentermine products. The combination drug Qsymia (phentermine/topiramate) is also on the preferred list that must be tried first.3Upper Peninsula Health Plan. Medicaid Benefit Non-Part D Prior Authorization Criteria Those preferred medications are lower cost, but each still requires its own prior authorization.2University of Michigan Medicine. The same restrictions apply to Zepbound (tirzepatide), Saxenda (liraglutide), and generic liraglutide when prescribed for obesity.4Bridge Michigan. Michigan Limits Access to Weight Loss Drugs for Medicaid Patients
Keeping Coverage: Renewal Rules
Authorizations run six months at a time. To renew, adult patients must show at least a 5% loss of body weight since the prior authorization was issued. Patients aged 12 to 17 must show that their BMI has been maintained or improved based on CDC growth charts.3Upper Peninsula Health Plan. Medicaid Benefit Non-Part D Prior Authorization Criteria
When the Stricter Rules Do Not Apply
The new criteria apply only when Wegovy is prescribed solely to treat obesity. If Wegovy is prescribed for another FDA-approved use, coverage is governed by separate, less restrictive criteria.1Michigan.gov. Numbered Letter L 25-73 – Update of Pharmacy Drug Coverage for Treatment of Obesity In practice, that means:
- Wegovy remains covered for beneficiaries who are overweight or obese and have established cardiovascular disease, with the drug used to reduce cardiovascular risk.2University of Michigan Medicine. Expert Q&A: Michigan Medicaid’s New Limits on GLP-1 Weight Management Medications
- Wegovy remains covered for severe liver disease (metabolic dysfunction-associated steatohepatitis).5Prime Therapeutics. Michigan Medicaid Clinical and PDL PA Criteria
- GLP-1 medications used to treat type 2 diabetes, including Ozempic, Mounjaro, Rybelsus, and Victoza, are not affected by the 2026 changes.4Bridge Michigan. Michigan Limits Access to Weight Loss Drugs for Medicaid Patients
- Zepbound is separately covered for moderate to severe obstructive sleep apnea.6Priority Health. Medicaid GLP-1 Coverage Changes Effective Jan. 1, 2026
If your prescriber is treating one of those conditions, the prior authorization request should be submitted under that clinical pathway rather than the obesity pathway.
What You Will Pay at the Pharmacy
Wegovy and Saxenda were moved to “non-preferred” status on the state’s Single Preferred Drug List. For beneficiaries who are not enrolled in a Medicaid Health Plan and who are not exempt from copayments, the copay rose from $1 to $3 per prescription.1Michigan.gov. Which Medicaid Plans Are Affected
All of them. The policy applies uniformly whether you receive benefits through traditional Medicaid, the Healthy Michigan Plan, fee-for-service, or a managed care plan.6Priority Health. Medicaid GLP-1 Coverage Changes Effective Jan. 1, 20267Meridian Health Plan. GLP-1 Benefit Reduction Bulletin There is no separate rule or carve-out for any particular Medicaid population.
If Your Prior Authorization Is Denied
You have the right to appeal. The path depends on how you receive your Medicaid benefits.
If you are enrolled in a Medicaid Health Plan, start with an internal plan appeal. Deadlines and procedures vary by plan. At Molina Healthcare, for example, you must file within 60 calendar days of the denial, and a healthcare professional who was not part of the original decision reviews the case, with a written answer within 30 calendar days. If the appeal involves a reduction or termination of a treatment you were already receiving, you can request that benefits continue during the appeal, but you must file within 10 calendar days of the denial notice.8Molina Healthcare. If the plan upholds the denial, you can request a State Fair Hearing through MDHHS. The hearing request is submitted on the department’s standard form at a local DHHS office or by fax to the state.9Washtenaw Health Project. Why the Rules Tightened
The 2026 restrictions came out of the state budget. Public Act 22 of 2025, the omnibus budget bill for fiscal year 2026, cut $240 million from GLP-1 pharmaceutical appropriations and directed MDHHS to narrow eligibility for GLP-1 drugs prescribed solely for weight loss. The department formalized the new criteria in Numbered Letter L 25-73, dated December 8, 2025, with an effective date of January 1, 2026.1Michigan.gov.
