Does Medicaid Cover Wegovy in NJ? Exclusions and Prior Auth

NJ FamilyCare, the state’s Medicaid program, does not cover Wegovy when it is prescribed for weight loss in adults. It does cover Wegovy, with prior authorization, when the drug is prescribed to reduce cardiovascular risk in adults who have established heart disease, and it covers Wegovy for members under 21 who meet clinical criteria. So the answer to “does Medicaid cover Wegovy in NJ” depends almost entirely on why the drug is being prescribed and how old the patient is.

The Three Coverage Situations

Anti-obesity medications are a non-covered pharmacy benefit for NJ FamilyCare members 21 and older. Wegovy is not on the state’s Medicaid preferred drug list, and the managed care plans that administer the benefit, including Horizon NJ Health and UnitedHealthcare Community Plan, explicitly exclude weight loss drugs.1Horizon NJ Health. Adults 18 and older can get Wegovy covered when it is prescribed for cardiovascular disease risk reduction. The FDA approved Wegovy for this use in March 2024, and because it is a medically accepted indication beyond weight loss, NJ FamilyCare is required to cover it when clinical criteria are met.3NJ Department of Human Services. Members younger than 21 can be approved for Wegovy, including for weight management, under the federal Early and Periodic Screening, Diagnostic, and Treatment benefit, which requires Medicaid to cover medically necessary treatments for children.3NJ Department of Human Services. What Prior Authorization Requires

Even in the covered scenarios, Wegovy needs prior authorization through the member’s managed care plan before the pharmacy will fill it.

For an adult seeking coverage for cardiovascular use, the prescribing physician has to document the specific cardiovascular condition, confirm the patient is obese or overweight, and show that the prescription was written by or in consultation with a cardiologist or vascular specialist. The patient must also be on appropriate cardiovascular medications and receiving individualized lifestyle counseling. The prescriber has to confirm Wegovy is not being used alongside another GLP-1 drug such as Ozempic or Mounjaro.4Horizon NJ Health. Anti-Obesity Medications Fax Form

For a member under 21, the physician documents the child’s height and weight, any weight-related health conditions, and participation in a comprehensive weight loss plan that includes nutritional counseling and physical activity. The child has to continue that plan while on the medication, and the no-combination rule with other GLP-1 drugs applies here too.5Horizon NJ Health. Anti-Obesity Medications for Young Members

In both groups, Wegovy is contraindicated for patients with a personal or family history of medullary thyroid carcinoma, Multiple Endocrine Neoplasia syndrome type 2, or a history of serious allergic reactions to semaglutide.4Horizon NJ Health. Anti-Obesity Medications Fax Form

Medicaid enrollees are allowed to use it, but you have to agree not to seek reimbursement from your insurance, and the money you spend will not count toward any deductible or out-of-pocket maximum.8NovoCare. Why the Exclusion Exists and Whether It Might Change

Federal law lets states leave weight loss drugs out of Medicaid. The Medicaid Drug Rebate Program generally requires states to cover nearly all FDA-approved outpatient drugs from participating manufacturers, but the statute carves out an exception for drugs used for weight loss.10KFF. Optional Not Mandatory – NAMDs Recommendations on Anti-Obesity Medication Coverage New Jersey has used that exception.

The state is studying whether to change course. The fiscal year 2026 budget directed the Department of Human Services, with the Department of Health, to examine the viability and likely cost of providing weight loss drug coverage through NJ FamilyCare, the Pharmaceutical Assistance for the Aged and Disabled program, and the AIDS Drug Distribution Program.3NJ Department of Human Services.
