Does Medicare Cover IHSS? Medi-Cal, PACE, and the Gap

No, Medicare does not cover In-Home Supportive Services. IHSS is a California program paid for through Medi-Cal, the state’s Medicaid program, and it provides non-medical help at home — cooking, cleaning, bathing, personal care — for aged, blind, or disabled residents. Medicare pays only for medically necessary clinical care, so the day-to-day custodial help IHSS provides sits entirely outside what Medicare will fund.

Why Medicare Will Not Pay for IHSS

Federal regulations draw a hard line between skilled medical care and custodial care. Under 42 CFR § 411.15(g), any care that does not meet the requirements for coverage as skilled nursing facility care is classified as custodial and is excluded from Medicare Part A and Part B.1eCFR. 42 CFR 411.15 – Particular Services Excluded From Coverage IHSS tasks — laundry, meal preparation, housekeeping, help with bathing — fall squarely into that excluded category.

Medicare is built around acute medical needs: hospital stays, doctor visits, skilled nursing after surgery, and similar treatments.2Medicare. Get Started With Medicare It does not pay for long-term help with daily routines, no matter how essential that help is. That is why IHSS runs through Medi-Cal, and why every IHSS applicant must have a Medi-Cal eligibility determination before services can be authorized.3California Department of Social Services. What Medicare Home Health Actually Covers

Medicare does cover a narrow slice of home-based care, and it is easy to confuse this with IHSS. The two are not the same. To qualify for Medicare home health, you must be homebound — leaving home requires a considerable and taxing effort because of illness or injury — and you must need part-time or intermittent skilled nursing care or therapy ordered by a doctor.4Medicare. Home Health Services Coverage Covered services include wound care, injections, physical therapy, and patient education.

The hours are limited too. Medicare home health is generally capped at up to 8 hours per day of combined skilled nursing and home health aide services, with a maximum of 28 hours per week, or up to 35 hours per week for a short time if medically necessary.4Medicare. Home Health Services Coverage A home health aide can help with bathing or dressing, but only as part of a broader skilled care plan. When the skilled nursing need ends, the aide service ends with it.

IHSS works differently. It provides ongoing custodial help for as long as you remain eligible, with no requirement for a skilled care component and no expectation that you will recover from a medical episode.

How IHSS Is Paid For

IHSS is funded by Medi-Cal under a federal framework that lets states pay for home and community-based services as an alternative to nursing home placement. Federal law authorizes the Secretary of Health and Human Services to grant waivers allowing state Medicaid plans to cover these services for individuals who would otherwise need institutional care.5Office of the Law Revision Counsel. 42 USC 1396n – Compliance With State Plan and Payment Provisions California uses that authority to run IHSS statewide.

Under state law, the program’s purpose is to provide supportive services to aged, blind, or disabled individuals who cannot safely remain in their homes without assistance.6California Legislative Information. Who Qualifies for IHSS

Income and Assets

Because IHSS rides on Medi-Cal, you must meet Medi-Cal’s financial eligibility rules. For aged, blind, or disabled individuals whose eligibility is not based on modified adjusted gross income, income limits apply — roughly $1,801 per month for an individual as of early 2026. If you receive Supplemental Security Income, California enrolls you in Medi-Cal automatically, with no separate application.7HealthCare.gov. Supplemental Security Income (SSI) Disability and Medicaid Coverage

On assets, California eliminated all Medi-Cal asset tests effective January 1, 2024. The Department of Health Care Services has proposed reinstating asset limits aligned with federal SSI resource levels no sooner than January 1, 2026, as part of a budget deficit reduction effort.8Department of Health Care Services. Check with your county IHSS office or the DHCS website for the current rules.

Other Requirements

Beyond income, you must meet all of the following:

  • Be a current California resident.
  • Live in your own home or a residence of your choosing (hospitals, licensed community care facilities, and long-term care facilities do not count).
  • Have an active Medi-Cal eligibility determination.
  • Have a licensed health care professional certify that you need in-home services to avoid out-of-home care.

Each of these comes directly from the IHSS program requirements.3California Department of Social Services. In-Home Supportive Services (IHSS) Program A county social worker then visits your home to assess which specific tasks you need help with and how many monthly hours the program will authorize.

If You Already Have Medicare

Many IHSS recipients are “dual eligible,” meaning they qualify for both Medicare and Medi-Cal. Having Medicare does not block you from getting IHSS, and getting IHSS does not change your Medicare coverage. Medi-Cal still pays for your IHSS hours regardless of what Medicare does.

If your income is low enough, a Medicare Savings Program can have Medi-Cal pay your Medicare premiums and cost-sharing. The most common is the Qualified Medicare Beneficiary program. For 2026, QMB eligibility requires monthly income at or below $1,350 for an individual or $1,824 for a married couple, with resources no more than $9,950 (individual) or $14,910 (couple).9Medicare. Medicare Savings Programs If you qualify, Medi-Cal pays your Medicare Part B premium (which is $202.90 per month in 2026), any Part A premium, deductibles, and coinsurance.10Centers for Medicare and Medicaid Services. 2026 Medicare Parts A and B Premiums and Deductibles

Dual-eligible individuals can also enroll in a Dual Eligible Special Needs Plan, a type of Medicare Advantage plan built to coordinate Medicare and Medi-Cal benefits. Some D-SNPs offer supplemental benefits like non-emergency transportation, over-the-counter medications, and dental or vision coverage. Your IHSS hours stay separate from whatever Medicare plan you pick; enrolling in a D-SNP does not reduce or replace your IHSS authorization.

PACE: A Different Route That Does Involve Medicare

If what you really want is a single program that pulls medical care and in-home help together, look at the Program of All-Inclusive Care for the Elderly. PACE replaces both your Medicare and your Medi-Cal coverage with one plan, delivering medical care and supportive services through a coordinated care team. It covers home care similar to IHSS, along with primary care, prescription drugs, adult day programs, and hospital care, all bundled together.11Centers for Medicare and Medicaid Services. PACE Chapter 4 – Enrollment and Disenrollment

To qualify for PACE, you must:

  • Be 55 years of age or older
  • Need a nursing-facility level of care as determined by the state
  • Be able to live safely in the community at the time of enrollment
  • Live in a PACE organization’s service area

PACE is currently available in a limited number of California counties, including Los Angeles, Sacramento, Alameda, San Diego, and others, with ongoing expansions. The trade-off: you cannot receive IHSS and PACE at the same time, and under PACE a care team manages your services rather than you hiring your own provider. If choosing your own caregiver matters to you, IHSS is likely the better fit. If you want medical and personal care handled through one program, PACE may work.

The bottom line on the original question: Medicare will not pay for IHSS, and it was never designed to. The program you want is Medi-Cal’s, and in California that application starts with your county IHSS office.
