Medicare covers nursing home care in Myrtle Beach only in a narrow situation: short-term skilled rehabilitation after a qualifying hospital stay. Under Part A, it will pay for up to 100 days in a Medicare-certified skilled nursing facility when you need daily professional nursing or therapy. It will not pay for a long-term stay whose purpose is help with everyday tasks like bathing, dressing, or eating. That line between skilled care and custodial care decides almost every coverage question families run into here.
What Medicare Part A Pays For
Part A covers skilled nursing facility care when the services require a licensed professional. Think physical therapy after a hip replacement, wound care after surgery, IV medications, occupational therapy, or speech therapy. To qualify, you need skilled nursing care seven days a week or skilled therapy at least five days a week, and the care has to relate to the condition that put you in the hospital.1Medicare Interactive. SNF Basics
For 2026, the cost structure per benefit period is:
- Days 1 through 20: $0 coinsurance after the Part A deductible of $1,736.
- Days 21 through 100: $217 per day in coinsurance.
- After day 100: Medicare pays nothing, and you are responsible for all costs.
These amounts reset by benefit period, not by calendar year. A benefit period begins the day you’re admitted as an inpatient and ends only after 60 straight days with no inpatient hospital or skilled nursing care.2Medicare.gov. Skilled Nursing Facility Care
The Three-Day Hospital Stay
Before Medicare pays a dollar toward a skilled nursing stay, you must have been formally admitted as a hospital inpatient for at least three consecutive days. The day of discharge doesn’t count. You then have 30 days from leaving the hospital to enter a Medicare-certified facility, and the nursing care must treat the same condition.2Medicare.gov. Skilled Nursing Facility Care
Two exceptions exist. Some Accountable Care Organizations have a waiver from CMS that lets patients skip the hospital stay, and many Medicare Advantage plans waive it as well. Check your plan or your doctor’s ACO status before you count on this.3CMS. SNF Waiver Guidance
The Observation Status Trap
Time spent in a hospital bed under “observation services” does not count toward the three days, even if the care looks identical to inpatient care. This is where families get blindsided. Hospitals must give you a Medicare Outpatient Observation Notice within 36 hours of hitting 24 hours on observation.4Medicare Rights Center. What Medicare Will Not Pay For
Medicare does not cover long-term nursing home care. If the primary need is help with activities of daily living — bathing, dressing, eating, using the bathroom, getting in and out of bed — that is custodial care, and Medicare treats it as outside the program no matter how long the person has been enrolled.6Medicare.gov. Nursing Homes Payment Medigap supplemental policies don’t fill this gap either.7Medicare.gov. Long-Term Care
The practical test is whether you’re recovering or being maintained. Daily physical therapy from a licensed therapist after a stroke is skilled. Help with bathing and dressing for someone with advancing dementia who isn’t expected to recover is not.8Medicaid Planning Assistance. Who Pays for Nursing Homes Medicare will still cover doctor visits, prescription drugs, and medical supplies for a person living in a long-term nursing home, but not the room, board, or personal care.
If You Have Medicare Advantage
Medicare Advantage plans must cover at least what Original Medicare covers, but they can impose network restrictions and often require advance notification before a skilled nursing admission. Skip that notification and you may owe more or all of the bill. Out-of-pocket amounts can differ from Original Medicare’s structure, and as noted above, some plans waive the three-day hospital requirement. Call your plan before any planned nursing facility stay.9Medicare.gov. Medicare Skilled Nursing Facility Care10NCOA. Does Medicare Cover Nursing Homes
How Medigap Reduces the Daily Coinsurance
A Medigap policy can cover that $217-per-day coinsurance during days 21 through 100. Plans C, D, F, G, M, and N pay 100% of the skilled nursing coinsurance. Plan K pays 50%, Plan L pays 75%, and Plans A and B pay nothing toward it.11Medicare.gov. Compare Medigap Plan Benefits
Plans C and F are no longer available to people who became Medicare-eligible on or after January 1, 2020. For newer enrollees, Plan G gives the broadest coverage of this coinsurance. Medigap still won’t extend coverage past day 100 and still won’t cover custodial care.

Appealing a Coverage Cutoff
If Medicare or the facility tells you your covered stay is ending before you think it should, you have the right to a fast appeal. The facility must give you a Notice of Medicare Non-Coverage at least two days before covered services end. Call the Beneficiary and Family Centered Care Quality Improvement Organization listed on that notice by noon the day after you receive it, and a reviewer typically decides within 72 hours.13Medicare.gov. Fast Appeals
One point worth knowing before you appeal: under the Jimmo v. Sebelius settlement, coverage does not depend on your potential to improve. Care that maintains your condition or slows decline still qualifies as skilled if it requires professional oversight.14Center for Medicare Advocacy. Self-Help Packet for Expedited SNF Appeals
Paying for Care Once Medicare Stops
When day 100 ends, or when the need is custodial from the start, the bill falls to the resident and family. Nationally in 2026, the median shared nursing home room runs $9,277 per month and a private room $10,646.10NCOA. South Carolina Medicaid
Medicaid is the primary payer for long-term nursing home care, and South Carolina’s Healthy Connections program covers eligible residents indefinitely. For 2026, the key thresholds are:
- Income limit: $2,982 per month for a single applicant.
- Asset limit: $2,000 for a single applicant; $4,000 for a married couple when both apply.
- Community Spouse Resource Allowance: the non-applicant spouse may keep up to $66,480 in assets.
- Home equity exemption: a primary home is exempt if equity is at or below $752,000, provided the applicant lives there or files intent to return.
South Carolina is an income-cap state. If your income exceeds $2,982, you cannot spend down to qualify; you have to set up a Qualified Income Trust (a Miller Trust) and deposit the excess each month.16SC DHHS. Program Eligibility and Income Limits The state looks back 60 months at asset transfers; gifts or below-market sales during that window can trigger a penalty period of ineligibility.17Medicaid Planning Assistance. Medicaid Eligibility South Carolina Once approved, a nursing home resident contributes almost all monthly income toward care and keeps $60 for personal needs.

Staying at Home: The Community Choices Waiver
South Carolina’s Community Choices waiver serves people who qualify for nursing facility care but want to remain at home. It covers frail elderly adults 65 and older and people with physical disabilities ages 18 through 64, providing personal care, home-delivered meals, nursing visits, home modifications, and medical equipment. Referrals go through the SCDHHS online portal or the intake line at 888-971-1637.19SC DHHS. Waivers
Medicare has its own home health benefit at no cost to the patient if you are homebound and need skilled nursing or therapy ordered by a doctor and delivered by a Medicare-certified agency. It does not cover 24-hour care, meals, or personal care when that is the only need.20Medicare.gov. Home Health Services
Veterans’ Benefits
Veterans with service under general or honorable conditions may qualify for VA long-term care at VA Community Living Centers, VA-contracted community nursing homes, and state veterans homes. None of South Carolina’s six state veterans homes are in Myrtle Beach; the closest are in Florence and Columbia.21SC Department of Veterans’ Affairs. State Veterans Homes The VA’s Aid and Attendance benefit also pays a tax-free monthly amount toward assisted living or nursing home costs: in 2026, up to $2,874 for a married wartime veteran, $2,424 for a single veteran, and $1,558 for a surviving spouse.22VA. Private Long-Term Care Insurance
Long-term care insurance pays for the custodial services Medicare won’t, including nursing home, assisted living, and in-home personal care. Benefits usually trigger when the policyholder cannot perform at least two of six activities of daily living or has cognitive impairment, with an elimination period of 30 to 90 days before payments begin, and daily or monthly caps up to a lifetime maximum.6Medicare.gov. South Carolina participates in the Long-Term Care Insurance Partnership Program. For every dollar a qualified partnership policy pays out, a dollar of personal assets is protected from Medicaid spend-down and from estate recovery if the policyholder later needs Medicaid.23SC Department of Insurance.
