Does Meridian cover Zepbound? Sometimes, and the rules are strict. In Michigan, Meridian’s Medicaid plan still covers Zepbound, but as of January 1, 2026, a prescription written for weight loss alone must clear four tough criteria. In Illinois, Meridian’s Medicaid plan excludes weight loss as a benefit entirely, though Zepbound can be authorized for obstructive sleep apnea under a separate clinical policy.1Meridian Health Plan of Illinois. Michigan Meridian: Weight-Loss Coverage After January 1, 2026
Zepbound is still on the Michigan Meridian formulary, listed as Tier 2 (non-preferred) with prior authorization required.2Meridian Health Plan of Michigan. To get Zepbound covered for weight loss, a Michigan Medicaid patient must now meet all four of these conditions:
- A body mass index of 40 or higher (morbid obesity).
- Documented failure of other clinically appropriate weight-loss approaches.
- A trial and failure of preferred anti-obesity drugs on the state’s Preferred Drug List, such as phentermine or Qsymia (phentermine with topiramate).
- Prescription of the GLP-1 specifically as a way to avoid bariatric surgery.
The patient must also be at least 18. Quantity limits are 2.4 mL per 28 days for the Zepbound KwikPen and 2 mL per 28 days for the SOAJ formulation.2Meridian Health Plan of Michigan. Michigan Meridian Formulary 2026
If you had an active prior authorization for Zepbound before January 1, 2026, your coverage continued for the remaining time on that six-month approval. Once it expired, any renewal was reviewed against the new criteria. Meridian and the state began mailing notices to affected enrollees in early December 2025.5Meridian Health Plan of Michigan. Michigan Coverage That Wasn’t Cut
The January 2026 restrictions apply only when Zepbound is prescribed solely for weight loss. Other medical indications have their own approval pathways.
Obstructive Sleep Apnea
The FDA approved Zepbound in late 2024 for moderate-to-severe obstructive sleep apnea in adults with obesity.6PR Newswire. FDA Approves Zepbound for Moderate-to-Severe Obstructive Sleep Apnea in Adults With Obesity Michigan Medicaid continues to authorize Zepbound for OSA under separate clinical criteria. Prior authorization is still required, but the approval path does not depend on the weight-loss criteria above.7Michigan Medicaid. Michigan Medicaid Clinical and PDL Criteria
Type 2 Diabetes
Tirzepatide is also sold as Mounjaro for type 2 diabetes. GLP-1 coverage for diabetes was not changed by the 2026 budget.8Priority Health. Medicaid GLP-1 Coverage Changes Effective January 1, 2026 If your tirzepatide prescription is for diabetes, the correct product is Mounjaro, not Zepbound.
Cardiovascular Risk
Wegovy (semaglutide) has an FDA-approved indication for reducing cardiovascular risk. Michigan Medicaid continues to cover Wegovy for patients who are overweight or obese and have established cardiovascular disease or severe liver disease.9University of Michigan Medical Research. Expert Q&A: Michigan Medicaid’s New Limits on GLP-1 Weight Management Medications Zepbound itself does not carry a cardiovascular-risk indication.
Illinois Meridian: No Weight-Loss Coverage, But an OSA Pathway
In Illinois, Meridian’s Medicaid plan treats weight loss as a benefit exclusion. The plan’s GLP-1 clinical policy states that “weight loss is a benefit exclusion and is not a covered benefit,” and directs that requests for GLP-1 medications outside of type 2 diabetes be denied on that basis.1Meridian Health Plan of Illinois. Glucagon-Like Peptide-1 Receptor Agonists Clinical Policy Zepbound is not on the Illinois Medicaid Preferred Drug List as of January 2026.10Illinois Department of Healthcare and Family Services. Illinois Medicaid Preferred Drug List
There is one opening. Meridian’s Illinois plan maintains a separate clinical policy for Zepbound prescribed for obstructive sleep apnea. To qualify, a patient needs:
- A BMI of 30 or higher.
- A confirmed OSA diagnosis through a sleep study.
- Documented participation in a physician-directed weight-loss program.
- Adherence to positive airway pressure (PAP) therapy, or documentation that PAP is not an option.
Even under this pathway, the policy repeats that Zepbound will not be authorized for weight management.11Meridian Health Plan of Illinois. Tirzepatide (Zepbound) for OSA Clinical Policy For a tirzepatide prescription written for diabetes, the covered product is Mounjaro, not Zepbound.
If You’re Denied
With criteria this narrow, denials are common. You and your prescriber can appeal.
- Start with the denial letter. It states the specific reason and the filing deadline. Many plans allow up to 180 days; some are as short as 60 to 65 days.
- Build the appeal around that reason. A Letter of Medical Necessity from your prescriber should include current BMI, weight history, comorbidities with ICD-10 codes, prior weight-loss attempts, and clinical rationale for Zepbound specifically.
- Submit the internal appeal. Standard decisions are generally due within 30 days, urgent ones within 72 hours.
- If the internal appeal fails, request an independent external review. Available guidance indicates external reviewers overturn about 40% of the denials that reach them.
If your BMI is below the morbid obesity threshold but you have moderate-to-severe obstructive sleep apnea, the indication coded on the prior authorization matters. A request submitted under the OSA policy follows different rules than one submitted for weight loss.11Meridian Health Plan of Illinois. Tirzepatide (Zepbound) for OSA Clinical Policy
What Could Change: The BALANCE Model
CMS has launched a voluntary program called the BALANCE Model (Better Approaches to Lifestyle and Nutrition for Comprehensive Health) that lets state Medicaid agencies opt into federally negotiated GLP-1 pricing. States can begin joining as of May 2026, and the application deadline is July 31, 2026.12KFF. What to Know About the BALANCE Model for GLP-1s in Medicare and Medicaid The model covers the Zepbound KwikPen, Wegovy, Ozempic, Mounjaro, Rybelsus, and orforglipron if the FDA approves it. Participating states must use the model’s coverage criteria and cannot make them stricter, though they can be more generous. Prior authorization still applies.13Centers for Medicare & Medicaid Services. BALANCE Model Whether Michigan or Illinois joins is not yet decided, but for Medicaid enrollees in both states it is the clearest route to broader Zepbound access.
