Does OHP Cover GLP-1? Approved Uses, CCO Rules, and Denials

The Oregon Health Plan covers GLP-1 medications, but only when they treat a specific covered condition and only with prior authorization. OHP pays for GLP-1 and dual GLP-1/GIP receptor agonists for type 2 diabetes, for reducing the risk of a second cardiovascular event, for metabolic dysfunction-associated steatohepatitis (MASH), and for moderate-to-severe obstructive sleep apnea. It does not cover these drugs for adult weight loss on their own. Members under 21 have a separate pathway through the federal EPSDT program.

What OHP Covers and What It Doesn’t

Under federal Medicaid rules, states can exclude drugs prescribed solely for weight loss, and Oregon has done that. Trillium Community Health Plan’s pharmacy guidance states plainly that “pharmacologic treatment of obesity is an excluded benefit of the Oregon Health Plan.”1Trillium Community Health Plan. Pharmacy Information and Preferred Drug List Changes, Third Quarter 2024 Oregon’s Drug Use Review materials confirm the same for most members.2Oregon Prescription Drug List. Weight Loss DERP Summary

In practical terms: Wegovy, Zepbound, Saxenda, and similar drugs cannot be approved through OHP just because a patient wants to lose weight. The medication has to be prescribed for one of the covered medical conditions below, and the prior authorization criteria are built to verify that.

GLP-1s for Type 2 Diabetes

OHP covers GLP-1 receptor agonists for type 2 diabetes using step therapy that pushes cheaper options first. Three GLP-1s are preferred on the state’s fee-for-service drug list: dulaglutide (Trulicity), exenatide (Byetta), and liraglutide (Victoza). Semaglutide (Ozempic and Rybelsus) and tirzepatide (Mounjaro) are non-preferred and face extra prior authorization hurdles.3Oregon Prescription Drug List. Oregon Fee-for-Service Enforceable Physical Health Preferred Drug List

To get any GLP-1 approved for diabetes, a patient generally needs an HbA1c above 7%, a documented failure of metformin (or a reason it can’t be taken), and a documented history of inadequate results on liraglutide. On top of that, the patient must have failed three-month trials of two other diabetes drugs from a specific list that includes sulfonylureas, pioglitazone, alogliptin, and SGLT2 inhibitors. Mounjaro follows these same criteria rather than a separate pathway.4CareOregon. Glucagon-Like Peptide 1 and GIP Agonists PA Criteria

Patients with established atherosclerotic cardiovascular disease or chronic kidney disease can sometimes skip parts of the step therapy, but they still need to show they’ve tried or can’t tolerate an SGLT2 inhibitor. A GLP-1 cannot be combined with another GLP-1 or GIP agonist under OHP rules.4CareOregon. Glucagon-Like Peptide 1 and GIP Agonists PA Criteria

GLP-1s for Covered Non-Diabetes Conditions

As the FDA has expanded approved uses for semaglutide and tirzepatide, OHP has opened coverage pathways for three non-diabetes indications. Each one has its own criteria, and the medication must be prescribed for that specific condition, not for weight loss generally.

Preventing a Second Cardiovascular Event

Wegovy (semaglutide) is covered for adults who already have cardiovascular disease and need to lower their risk of future heart attacks and strokes, tracking the FDA’s 2023 approval. To qualify, a patient must be at least 45 years old with a BMI of 27 or higher and have a documented history of heart attack, stroke, or symptomatic peripheral arterial disease. The patient also has to have been screened for diabetes within the past year and confirmed not to have it; diabetic patients are routed to the diabetes GLP-1 pathway instead.5CareOregon. GLP-1 and GIP Agonist Drugs for Non-Diabetes Indications PA Criteria

Before approval, the patient must have participated in a provider-directed weight loss plan involving diet, exercise, or nutritional counseling for at least three months within the previous six months. They should also be taking standard cardiovascular prevention medications like high-intensity statins or antiplatelet drugs unless those are medically inappropriate.6Oregon Prescription Drug List. Semaglutide CV Risk PA Update Initial approval lasts six months. Renewal requires documented maintenance of at least a 5% BMI reduction and continued participation in a weight loss program.5CareOregon. Metabolic Dysfunction-Associated Steatohepatitis (MASH)

Wegovy is covered for adults with non-cirrhotic MASH who have moderate to advanced liver fibrosis, staged as F2 or F3. Fibrosis has to be confirmed by liver biopsy or by non-invasive testing such as vibration-controlled transient elastography, magnetic resonance elastography, or an Enhanced Liver Fibrosis score, each with its own threshold value.5CareOregon. GLP-1 and GIP Agonist Drugs for Non-Diabetes Indications PA Criteria

Patients with significant alcohol use are excluded, and Wegovy cannot be used at the same time as resmetirom (Rezdiffra), another drug approved for MASH. The patient must be engaged in dietary changes and physical activity. Initial approval lasts six months; a 12-month renewal requires clinical documentation of stable or improved fibrosis.5CareOregon. Obstructive Sleep Apnea

Tirzepatide (Zepbound) is covered for adults without diabetes who have moderate to severe obstructive sleep apnea and a BMI of at least 30. The diagnosis must be confirmed by a polysomnography study showing an apnea-hypopnea index of 15 or more events per hour. The patient must have maximized positive airway pressure therapy (CPAP or BiPAP) or shown an inability to tolerate it, and must have participated in a provider-directed weight loss plan for at least three months within the past six months.7Oregon Prescription Drug List. Renewal at 12 months requires evidence of at least a 5% BMI reduction and continued use of both PAP therapy and a weight loss treatment plan.5CareOregon. GLP-1 and GIP Agonist Drugs for Non-Diabetes Indications PA Criteria

Coverage for Members Under 21

The adult weight-loss exclusion has one carve-out. OHP members under 21 may qualify through the Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) program, which requires Medicaid to cover medically necessary services for children and young adults even when those services sit outside the standard benefit list.

For weight management drugs, criteria generally require the member to be between 12 and 20 years old with a BMI at or above the 95th percentile for age and sex (or a BMI of 30 or higher), and actively enrolled in a provider-directed weight loss program. The medication must be FDA-approved for the member’s age.8Trillium Community Health Plan. Weight Management Drugs Clinical Policy

CareOregon’s pediatric chronic weight management policy adds more: the patient must have severe obesity (BMI of 35 or higher, or at least 120% of the 95th percentile), must have tried phentermine/topiramate (Qsymia) first, and must have completed a documented lifestyle intervention program lasting three to 12 months. Initial approval is six months; renewal requires at least a 5% reduction in baseline BMI.5CareOregon. GLP-1 and GIP Agonist Drugs for Non-Diabetes Indications PA Criteria

EPSDT requests are reviewed case by case. The prescriber has to submit documentation showing why the medication is medically necessary, why it is the least costly effective option, and what alternatives have been tried.9Oregon Health Authority. EPSDT Provider Guide

Rules Vary by Coordinated Care Organization

Most OHP members get care through a coordinated care organization, and while CCOs follow state-level rules, their prior authorization details are not identical. CareOregon and Trillium both cover Wegovy for cardiovascular risk reduction and MASH, but their documentation requirements and step-therapy sequences differ. Trillium requires six months of enrollment in a physician-directed weight loss program before Wegovy approval for cardiovascular prevention, while CareOregon’s policy calls for a three-month treatment plan within the prior six months.10Trillium Community Health Plan. Check with the specific CCO for the criteria that apply. The CCO’s name appears on the OHP member identification card.

If a GLP-1 Prescription Is Denied

When a prior authorization request is denied, OHP members receive a Notice of Adverse Benefit Determination explaining the decision and the member’s rights. Members in a CCO can file an appeal with the CCO within 60 days of the date on the notice. The CCO has 16 days to decide the appeal, with an optional 14-day extension. For urgent medical needs, an expedited appeal must be resolved within 72 hours.11Oregon Health Authority. Appeals and Hearings

If the appeal fails, the member can request an administrative hearing through the Oregon Health Authority within 120 days of the appeal resolution. Hearings can be requested online or by submitting form OHP 3302. A member who is already receiving a service that is being cut off can request that it continue during the appeal or hearing, but only if the request is made within 10 days of the effective date on the notice.11Oregon Health Authority.
