Does OHP Cover Gym Memberships? Eligibility and CCO Options

The Oregon Health Plan does not cover gym memberships as a standard benefit, but if you are enrolled in a coordinated care organization (CCO), you may be able to get one at no cost through a category of extras called flexible services. Access depends on which CCO you belong to, whether your healthcare provider approves the request, and whether the CCO has funding available.

Standard OHP benefits cover things like doctor visits, prescriptions, mental health care, and dental care. Gym memberships sit outside that package. The Oregon Health Authority’s approved list of flexible services spending, updated in December 2025, includes “gym or community center memberships,” pool passes, and transportation to the gym as allowed uses of CCO funds. Being on the approved list does not require any CCO to offer the benefit; it only permits them to.

Who Qualifies

Three conditions generally have to line up:

  • You are enrolled in a CCO. Members with Open Card coverage, meaning no CCO assignment, cannot access flexible services at all. If you also have Medicare, you still need CCO enrollment to qualify.
  • A healthcare provider approves the gym membership as part of your treatment plan. A few CCOs waive this for a basic membership but still require it for specialized programs.
  • Your CCO has funding available and your request fits its criteria.

How To Request One

  • Check your OHP card. It will show whether you are enrolled in a CCO and list that CCO’s name and phone number.
  • Talk to your provider about whether a gym membership supports your treatment plan, and ask them to document it.
  • Call your CCO using the number on your card and ask specifically about flexible services for a gym membership. Some CCOs use a request form; others handle it through customer service.
  • Submit whatever documentation the CCO asks for. That might be a signed form, confirmation from your provider, or, with a few CCOs, an in-person visit to a participating facility with your CCO ID.

If you are not enrolled in a CCO and want to discuss your options, OHP Client Services can be reached at 1-800-273-0557.

What Each CCO Offers

AllCare CCO

AllCare runs one of the simplest programs. You can walk into a participating fitness facility with your AllCare CCO ID and a photo ID and enroll in a monthly gym membership at no cost, with no referral needed for the basic membership. Participating sites include the Ashland Family YMCA, Rogue Valley Family YMCA, Grants Pass YMCA, Gold Beach Fitness Center, and Priority Fitness in Cave Junction. Specialized programs, such as 12-week weight loss sessions, chronic pain training with a personal trainer, and youth fitness programs, do require a provider prescription and have criteria like minimum BMI thresholds. In March 2026, AllCare and the Rogue Valley Family YMCA announced a partnership designed to initially serve up to 500 AllCare members in the Medford area.

PacificSource Community Solutions

PacificSource gives OHP members up to $1,500 per calendar year in flexible services, and short-term gym passes qualify. The money does not roll over. You or your provider submits a Flexible Services Request Form by email or fax, confirming the service is part of a provider-approved treatment plan, and PacificSource reviews each request individually. Members in the same household can pool funds for shared expenses.

Health Share of Oregon

Health Share lists gym and community center memberships, weight management classes, and chronic disease prevention classes among its extras, available through flexible services with provider approval.

Jackson Care Connect

Jackson Care Connect covers exercise equipment and gym memberships through flexible services, with a $1,500 annual limit per member. The Rogue Valley Family YMCA also offers Jackson Care Connect members a financial assistance scholarship that reduces YMCA dues by roughly a third.

Trillium Community Health Plan

Trillium’s flexible services page mentions access to a warm pool for arthritis and movement therapy, but not broader gym memberships. As of June 2026, Trillium has paused new applications for flexible services.

Umpqua Health Alliance

Umpqua Health Alliance sponsors a YMCA Community Benefits Initiative through the YMCA of Douglas County for members with low incomes. Keeping eligibility requires at least eight visits per month, the program is currently waitlist-only due to limited funding, and a partial contribution toward dues may be required. Umpqua also handles gym membership requests through flexible services, with initial approvals granted in three-month increments.

InterCommunity Health Network (IHN-CCO)

IHN-CCO does not explicitly list gym memberships among its standard offerings, but it does consider flexible services case by case. Work with your provider and contact IHN-CCO customer service to submit a request.

Why Coverage Varies So Much

Oregon can fund non-medical services like gym memberships through Medicaid under its Section 1115 demonstration waiver with the Centers for Medicare and Medicaid Services. The current waiver runs from October 2022 through September 2027. It gives CCOs room to spend on services outside traditional medical care when those services can improve member health, but the spending stays at each CCO’s discretion within state and federal guardrails. That discretion is why one CCO hands out YMCA memberships at the front desk while another has paused new requests entirely.
