PEEHIP does not cover weight loss medication. Alabama’s Public Education Employees’ Health Insurance Plan lists “Anti-Obesity Preparations, Weight Loss Medications” as a general exclusion on its pharmacy formulary, and the Supplemental Medical Plan separately confirms that “medications for weight loss” are not a covered benefit.
What the Exclusion Covers
The pharmacy formulary is administered by Express Scripts, and the exclusion appears in both the January 2025 and January 2026 editions, so the policy has held steady across plan years. It applies to the GLP-1 receptor agonists driving most of the current demand: semaglutide (Wegovy for weight loss, Ozempic for diabetes) and tirzepatide (Zepbound for weight loss, Mounjaro for diabetes). None appear as covered drugs in the formulary sections provided to members.
There is no prior authorization path around the exclusion. The formulary states that prior authorization is “generally not available for drugs that are specifically excluded by benefit design.” The PEEHIP Supplemental Medical Plan (Group 61000) reinforces the same restriction and adopts all exclusions from the primary PEEHIP Group #14000 plan.
Diagnosis does not change the answer at PEEHIP. Some insurers draw a line between the diabetes indications (Ozempic, Mounjaro) and the weight management indications (Wegovy, Saxenda, Zepbound), but the PEEHIP formulary exclusion is a blanket one for the anti-obesity category, independent of indication.
What You Pay Without Coverage
Without insurance coverage, GLP-1 weight loss medications can cost $1,300 or more per month, according to guidance issued by the Alabama Department of Insurance in August 2024. That guidance noted that employers have the authority to exclude GLP-1 drugs from employer-sponsored plans, and PEEHIP has exercised that authority.
Members who want to confirm coverage for a specific drug, or ask whether a diabetes diagnosis changes anything in their individual case, can call Express Scripts at (800) 363-9389.
Medicare-Eligible Retirees
Medicare-eligible PEEHIP retirees face an additional layer of restriction. Medicare has historically been prohibited from covering weight loss drugs when used exclusively for weight loss. Beginning January 1, 2026, PEEHIP’s Medicare Advantage plan is administered by Humana, replacing UnitedHealthcare. The Humana plan materials cover “obesity screening and therapy” as a preventive service at no cost and include personal health coaching for weight management, nutrition, and exercise, but the plan documents do not confirm coverage for weight loss drugs in the pharmacy benefit.
One temporary federal program is worth knowing about. The Medicare GLP-1 Bridge Program runs from July 1, 2026 through December 31, 2027 and provides coverage for specific weight loss medications, including Wegovy and Zepbound, for eligible members of Medicare Advantage plans with prescription coverage, at a $50 copayment per 30-day supply. Whether the program reaches PEEHIP’s Humana Medicare Advantage members depends on that plan’s participation. Humana or PEEHIP can confirm.
What PEEHIP Does Cover for Weight
Two routes do exist inside PEEHIP for members working on weight: bariatric surgery in limited circumstances, and free wellness programs.
Bariatric Surgery
The Summary of Benefits and Coverage states that bariatric surgery is covered for “morbid obesity in limited circumstances.” Under the Blue Cross Blue Shield of Alabama PPO plan (Group #14000), the surgery must be performed at an Alabama Blue Distinction Center. In-network, it is covered at 100% of the allowed amount after applicable copays; out-of-network, at 80% of the allowed amount subject to the calendar year deductible. Inpatient admissions require precertification at 1-800-354-7412, and benefits are not available without it.
Free Wellness Programs
Members on the Hospital Medical Plan administered by Blue Cross Blue Shield of Alabama have access to several no-cost programs that address weight:
- Wondr Health, a digital weight management program teaching science-based skills for sustainable weight loss, open to subscribers and covered spouses at no cost through wondrhealth.com/PEEHIP. Enrollment runs on scheduled windows with limited space.
- Chews Wisely Alabama, a nutrition-focused program centered on healthier food choices.
- BCBS Disease Management Coaching for chronic conditions including diabetes, available at 888-841-5741.
- Annual wellness screenings through the Alabama Department of Public Health, participating pharmacies, or primary care providers, which collect height, weight, and BMI. Completing one by August 31 each year earns the wellness premium waiver.
The PEEHIP Wellness Program names obesity, hypertension, pre-diabetes, diabetes, and heart disease as the risks it aims to reduce. Coaching participation is voluntary, and the annual screening is the piece tied to the premium waiver.
For Medicare-eligible retirees, the previous UnitedHealthcare plan (through 2025) offered Real Appeal, an online weight management and healthy lifestyle program at no cost to members with a BMI of 19 or higher. The Humana plan taking over in 2026 includes personal health coaching that covers weight management, nutrition, and exercise.
