Does SoonerCare Cover Dentures for Adults? Approval, Costs, and Dentists

Yes, SoonerCare does cover dentures for adults. Full dentures, partial dentures, and immediate dentures are all included in the Adult Limited Dental Benefit for members 21 and older, in both traditional Medicaid and the Healthy Adult Program expansion.1Oklahoma Health Care Authority. SoonerCare Dental Coverage comes with prior authorization, clinical criteria you have to meet, and firm limits on how often a denture can be replaced.

What’s Covered

The benefit covers complete dentures for the upper arch, lower arch, or both. Immediate dentures, which go in right after extractions, are covered too. On the partial side, SoonerCare pays for resin-base, cast-metal-framework, flexible-base, and unilateral partial dentures.2Liberty Dental Plan. OK SoonerCare Adult Schedule of Benefits

The surrounding work you need to actually use a denture is covered as well: medically necessary extractions before the denture is placed, alveoloplasty, adjustments, repairs to broken bases or frameworks, replacement of individual teeth or clasps on an existing denture, relines, rebases, and tissue conditioning.2Liberty Dental Plan. A few things are not covered, and they matter if you’re weighing your options. Dental implants are not a benefit for adults under SoonerCare; the only implant-related code in the fee schedule is for removing an existing implant, not placing a new one.2Liberty Dental Plan. OK SoonerCare Adult Schedule of Benefits Root canals and crowns are also not covered for adults; they’re only available to members under 21.1Oklahoma Health Care Authority. SoonerCare Dental If a tooth could potentially be saved with a crown or root canal, extracting it and getting a denture through SoonerCare may be the only path the program will pay for.

How Often You Can Get a New Denture

Replacement is tightly limited. If you are 25 or older, SoonerCare will cover a new complete or partial denture once per arch every seven years. If you are under 25, replacement is allowed once per arch every five years.3Oklahoma Health Care Authority. Adult Limited Dental Benefit Member Guide Immediate dentures are stricter still: once per arch in a lifetime.2Liberty Dental Plan. OK SoonerCare Adult Schedule of Benefits

For maintenance after you receive the denture, relines are covered once per arch every 36 months, and no reline will be approved within the first six months after a new denture is delivered. Repairs to a broken base or framework are limited to once per arch per calendar year.2Liberty Dental Plan. When a Partial Denture Will Be Approved

Not every pattern of missing teeth qualifies. SoonerCare covers a partial denture to replace missing permanent front teeth, or to replace two or more missing back teeth when all of the missing posterior teeth are on the same arch.3Oklahoma Health Care Authority. Adult Limited Dental Benefit Member Guide The provider has to identify the specific teeth being replaced, and your mouth has to have been free of active oral disease for at least 12 months before the request will be considered.5Oklahoma Health Care Authority. One rule that catches people: once a provider has billed for a full denture or a partial, switching to the other type may not be allowed. Raise any concerns about the treatment plan before impressions are taken. You have the right to a second opinion.3Oklahoma Health Care Authority. Adult Limited Dental Benefit Member Guide

What You’ll Pay

Non-exempt adult members pay a $4 copay per dental visit. Emergency dental visits carry no copay, and providers cannot refuse to treat you if you can’t pay the copay at the time of the visit.6Oklahoma Health Care Authority. OHCA Providing Additional Dental Benefits for Adult SoonerCare Members

Several groups owe no dental copay at all: pregnant women, members in an institution, hospice recipients, American Indians and Alaska Natives who have received services through an Indian health care provider, members on Medicaid coverage for breast and cervical cancer, and anyone under 21.6Oklahoma Health Care Authority. Getting the Denture Approved

Every adult denture requires prior authorization. Your dentist submits the request, but it helps to know what goes into it. The provider has to send a comprehensive treatment plan along with a panoramic or full-mouth series of X-rays, bitewings, periapical films of edentulous areas, six-point periodontal charting, and documentation of your oral hygiene history. All images are submitted electronically and must clearly identify you and the requested service.8Oklahoma Health Care Authority. Dental Prior Authorization and Amendments

Processing has historically taken four to six weeks, though some requests move faster depending on workload.9Oklahoma Health Care Authority. SoonerSelect Dental Plans

Since February 2024, most adult SoonerCare members have been moved from fee-for-service dental coverage into the SoonerSelect managed care program. Under SoonerSelect you have to be enrolled in one of two dental plans: DentaQuest or Liberty Dental Plan. Both are required to cover everything the old fee-for-service benefit covered, including dentures. Members who didn’t actively choose a plan were auto-assigned, and plans can be switched during open enrollment windows.11Oklahoma Health Care Authority. SoonerSelect Dental Member Guide

American Indian and Alaska Native members are not required to enroll in SoonerSelect and can stay in their existing SoonerCare program. AI/AN members are also exempt from the prior authorization requirement for dentures and other dental services.2Liberty Dental Plan. OK SoonerCare Adult Schedule of Benefits

The SoonerSelect dental plans also offer some extra benefits above the standard coverage, including zero copays for all appointments, teledentistry, sedation when medically necessary, extra cleanings for members with certain chronic conditions, and nutritional counseling for diabetes or obesity.12Oklahoma Health Care Authority. Finding a Dentist Who Will Do the Work

You can search for participating dentists through the OHCA Provider Directory online and filter by dental specialty and location.13Oklahoma Health Care Authority. A directory listing doesn’t guarantee you an appointment. Offices may have age restrictions, may not be taking new SoonerCare patients, or may have long waits.1Oklahoma Health Care Authority. SoonerCare Dental Under SoonerSelect, routine appointments should be available within 30 days and specialist referral appointments within 60 days. If you can’t get an appointment within those timeframes, contact your dental plan’s member services for help.14DentaQuest. SoonerSelect Member Handbook

Access is harder in rural parts of the state. About 45 percent of Oklahomans live in rural areas, 1.3 million live in counties designated as dental health professional shortage areas, and fewer than half of Oklahoma dentists accept Medicaid.15Frontiers in Oral Health.
