SoonerCare does not cover weight loss shots. Oklahoma’s Medicaid program excludes drugs prescribed primarily for obesity, which rules out Wegovy, Zepbound, and similar GLP-1 injectables when the only reason for the prescription is weight management.1Oklahoma Health Care Authority. When SoonerCare Does Cover the Same Shots
The exclusion is tied to the reason for the prescription, not the drug itself. If you have a different FDA-approved condition that one of these medications treats, SoonerCare can cover it with prior authorization.
Type 2 Diabetes
Several GLP-1s are on the SoonerCare formulary for type 2 diabetes. Victoza (liraglutide), Byetta, and Bydureon (exenatide) sit at Tier 2. Ozempic (semaglutide), Trulicity (dulaglutide), and others are at Tier 3. Mounjaro (tirzepatide) is available under a Special Prior Authorization tier.6Oklahoma Health Care Authority. Cardiovascular Risk Reduction
Wegovy is covered to reduce the risk of major adverse cardiovascular events in members with established cardiovascular disease. Qualifying history includes a prior heart attack, stroke, or symptomatic peripheral arterial disease. The member must also be on standard heart-related treatments like blood pressure or cholesterol medication and following a diet and exercise plan. Initial approval runs through the dose-escalation period, and maintenance approval lasts up to one year. The current Wegovy prior authorization form also sets criteria for noncirrhotic metabolic dysfunction-associated steatohepatitis (MASH) with moderate to advanced liver fibrosis, confirmed by specific diagnostic tests and prescribed by or in consultation with a gastroenterologist or hepatologist.8Oklahoma Health Care Authority. Obstructive Sleep Apnea
Zepbound is covered for moderate to severe obstructive sleep apnea in adults with obesity. The member must be 18 or older, have a BMI of at least 30, and have an apnea-hypopnea index of 15 or greater confirmed by a sleep study. The medication cannot be approved for obesity alone without the sleep apnea diagnosis.9Oklahoma Health Care Authority. Metabolic Disorders Prior Authorization10Oklahoma Health Care Authority. Aetna Better Health of Oklahoma’s formulary lists Zepbound and Wegovy under an “Anti-Obesity” category as Tier 1 drugs with prior approval, quantity limits, and a minimum age of 18.11Aetna Better Health of Oklahoma. Obesity Treatments SoonerCare Will Pay For
Even though the shots are off the table for weight loss, two other obesity-related services are covered.
Nutritional Counseling
SoonerCare covers up to six hours per year of counseling with a licensed registered dietitian, in person or by telehealth. A doctor, physician assistant, nurse practitioner, or nurse midwife has to prescribe it. For adults, counseling for obesity alone is not covered unless there is documentation that the obesity contributes to another illness. For children, counseling for obesity can be covered under the Early and Periodic Screening, Diagnostic and Treatment (EPSDT) benefit.12Oklahoma Health Care Authority. Bariatric Surgery
Bariatric surgery is covered with prior authorization for members between 15 and 65. Eligibility depends on BMI combined with any obesity-related conditions:
- BMI of 40 or higher: eligible without any additional comorbidity.
- BMI of 35 to 39.9: eligible with at least one serious comorbidity such as diabetes, hypertension, or obstructive sleep apnea.
- BMI of 30 to 34.9: eligible with at least two serious comorbidities, or with dysmetabolic syndrome or difficult-to-control diabetes.
Approval runs in two phases. The first is a multidisciplinary workup covering a psychological assessment, medical and surgical evaluations, at least two follow-up visits with a registered dietitian, a supervised exercise program, and verified tobacco cessation for at least six months. The second is submission of all that documentation for final approval before surgery. Surgeons and facilities must be certified through the American College of Surgeons’ bariatric surgery accreditation program.14Oklahoma Health Care Authority. Bariatric Surgery Guidelines
Paying Out of Pocket
If you don’t qualify for a covered indication, buying the shots directly is the main alternative. Prices have come down from the roughly $1,300 per month that was once standard. As of 2026:
- Novo Nordisk sells most doses of Wegovy and Ozempic for about $350 per month through its own website or retail partners.
- Eli Lilly sells Zepbound through LillyDirect for $299 to $449 per month as single-dose vials.
- TrumpRx.gov, a federal program launched in late 2025, offers various GLP-1s for $149 to $299 per month.15U.S. News and World Report. How to Pay for GLP-1 Without Insurance
Both Novo Nordisk and Eli Lilly run patient assistance programs for low-income patients. Novo Nordisk’s program sets the income cap at 200% of the federal poverty level; Lilly Cares sets it at 300%. Both typically require a Medicaid denial letter showing other coverage has been exhausted.15U.S. News and World Report. How to Pay for GLP-1 Without Insurance
Compounded Semaglutide Is No Longer a Workaround
Compounded semaglutide used to be a cheaper route, and it isn’t anymore. The FDA resolved the semaglutide shortage in February 2025 and ended its enforcement discretion for compounding pharmacies by spring 2025. A pharmacy can now compound semaglutide only when there is a specific clinical reason the commercial product doesn’t work for a particular patient, such as an allergy to an inactive ingredient. Cost alone doesn’t qualify, and neither does the diagnosis by itself.16FDA. FDA Clarifies Policies for Compounders as National GLP-1 Supply Begins to Stabilize17Compounding Finder.
