Does SoonerCare Cover Zepbound? Criteria, Approvals, and Copays

SoonerCare does cover Zepbound, but only for one purpose: treating moderate to severe obstructive sleep apnea in adults who also have obesity. Prescriptions written solely for weight loss are not covered. Even for the sleep apnea indication, coverage requires prior authorization with detailed clinical documentation from the prescribing provider.

What SoonerCare Will and Won’t Cover

Zepbound (tirzepatide) has two FDA-approved uses: weight management in adults with obesity or overweight plus a weight-related condition, and treatment of moderate to severe obstructive sleep apnea in adults with obesity.1FDA. The sleep apnea indication is separate, and SoonerCare covers Zepbound for that use through a dedicated prior authorization process. If your prescription is for weight loss without qualifying sleep apnea, expect a denial.

What Your Prescriber Must Document

Diagnosis

You must have moderate to severe obstructive sleep apnea along with obesity. The prescriber has to report your Apnea-Hypopnea Index score and note whether it came from polysomnography or a technically adequate home sleep apnea test. The form also requires confirmation that you do not have central or mixed sleep apnea, and it asks for your BMI, whether you have Type 1 or Type 2 diabetes, and your current HbA1c.3Oklahoma Health Care Authority. Diet and Exercise

Zepbound must be paired with a reduced-calorie diet and increased physical activity. The initial request has to include clinical documentation, such as office notes, showing you are participating in a diet and exercise program.3Oklahoma Health Care Authority. No Combinations With Other GLP-1s

Zepbound can’t be used with another tirzepatide product (like Mounjaro) or with any other GLP-1 receptor agonist. If your prescriber wants Zepbound vials instead of the pen, the form requires a patient-specific clinical reason why the pen won’t work.3Oklahoma Health Care Authority. How Approvals Are Timed

SoonerCare approves Zepbound in two stages. During the titration period, when the dose is stepped up, approvals are granted four weeks at a time for each dose level. If you can’t tolerate a step because of side effects, an extra four weeks at the same dose may be approved with supporting documentation.3Oklahoma Health Care Authority. Once you reach and tolerate a maintenance dose (10 mg or 15 mg for the sleep apnea indication), coverage is approved for one year at a time. Renewal requires your prescriber to confirm you are sticking with treatment, tolerating the dose, and continuing diet and physical activity, and to show clinical improvement, such as less daytime sleepiness, fewer episodes of snoring or breathing pauses, or a measurable drop in AHI events.3Oklahoma Health Care Authority. How the Request Gets Submitted

Your prescriber sends the completed Pharm-316 by fax to 1-800-224-4014 or contacts the Pharmacy Management Consultants Product Based Prior Authorization Unit at 1-800-522-0114 (Option 4). The form instructs providers not to send chart notes unless specifically requested. In an emergency, SoonerCare may reimburse a 72-hour supply while the prior authorization is pending.4Medicaid.gov. Coverage Under SoonerSelect Managed Care Plans

Most SoonerCare members receive benefits through a SoonerSelect managed care plan rather than fee-for-service Medicaid. Zepbound is generally available across the plans with prior authorization, though details differ.

Oklahoma Complete Health lists Zepbound as a Tier 1 (preferred) drug on its February 2026 preferred drug list, with prior authorization and coverage limited to members 18 and older. If Coverage Is Denied

You have the right to appeal, and how you do it depends on your plan.

In a SoonerSelect plan, appeal to the managed care organization first. Standard appeals must be resolved within 30 calendar days; expedited appeals, for urgent medical need, within 72 hours.8Oklahoma Health Care Authority. Appeals

If the plan denies the appeal, or if you’re in fee-for-service SoonerCare, you can request a state fair hearing before an OHCA administrative law judge. File form LD-1 (Member Complaint/Grievance Form) within 30 calendar days of the written denial. Hearings run by phone unless you request in-person using form LD-4 at least 10 days ahead. If You Have Type 2 Diabetes Instead

SoonerCare won’t cover Zepbound for Type 2 diabetes, but tirzepatide is available for that condition under its diabetes brand name, Mounjaro. Mounjaro sits in SoonerCare’s Tier 3 GIP/GLP-1 agonist category, meaning you must have tried and failed at least three months on a Tier 2 medication in the same class first. Copays and Prescription Limits

Adult SoonerCare members pay a $4 copay per prescription. Members under 21 have no copays for medically necessary prescriptions.10Oklahoma Health Care Authority.
