TennCare does not cover gym memberships. Tennessee’s Medicaid program lists no fitness center benefit in any of its benefit packages, and none of its managed care plans advertise gym access for standard Medicaid enrollees. If you have seen a gym perk tied to insurance in Tennessee, it almost certainly belongs to a Medicare Advantage plan or a state employee health plan, not TennCare.
Why Gym Memberships Are Not a TennCare Benefit
TennCare’s benefit packages, updated in April 2026, list the service categories the program covers: physician visits, hospital care, pharmacy, behavioral health, dental, vision, therapy, home health, and emergency transportation among them.1Tennessee.gov. TennCare Benefit Packages Each covered service must meet the program’s definition of medically necessary. Gym memberships appear in none of the packages (A through M) that correspond to the various TennCare eligibility categories.
There is a narrow discretionary channel worth knowing about. TennCare’s rules define “cost-effective alternative services” as items that are not covered benefits but that a managed care organization may choose to provide on its own.2Tennessee Secretary of State. TennCare Rules, Chapter 1200-13-13 A TennCare policy manual pre-approves “weight reduction programs for the treatment of obesity” as one such alternative that contractors may offer without extra approval.3Tennessee.gov. TennCare Policy Manual, BEN 08-001 That gives plans the option to fund a weight-loss program in an individual case. It does not create an entitlement, and no current TennCare managed care organization advertises a gym membership under this authority.
What TennCare Managed Care Plans Offer Instead
TennCare contracts with four managed care organizations: BlueCare (through BlueCross BlueShield of Tennessee), UnitedHealthcare Community Plan, Wellpoint (formerly Amerigroup), and TennCare Select.4Tennessee.gov. TennCare Managed Care Organizations None include a gym membership for standard Medicaid enrollees. Each does offer some wellness-related extras.
UnitedHealthcare Community Plan covers obesity screening, support for healthy diet and exercise choices, and health coaching. It also offers free health education classes on topics like nutrition and diabetes, with free transportation to and from those classes.5UnitedHealthcare. UnitedHealthcare Community Plan – TennCare
Wellpoint runs a Healthy Rewards program that gives gift cards to members who complete activities like wellness visits and screenings. Its listed Medicaid extras focus on dental care, children’s vision, transportation, and a 24-hour nurse helpline. No gym benefit appears.6Wellpoint. Wellpoint TennCare Extra Benefits7Wellpoint. Standard BlueCare TennCare members receive medical, behavioral, dental, and well-care benefits. The plan does not list fitness center access for Medicaid-only enrollees.8BlueCross BlueShield of Tennessee. BlueCare Tennessee Plans and Programs
The pattern is consistent. Wellness support arrives as coaching, education, screenings, and preventive checkups, not gym access.
The One Exception: BlueCare Plus for Dual-Eligible Members
BlueCare Plus does include fitness center access. It serves people who are dually eligible for both Medicaid and Medicare, and members get access to fitness centers locally and nationwide, group exercise classes, social activities, and home exercise resources.9BlueCross BlueShield of Tennessee. BlueCare Plus Helpful Tools and Resources The fitness benefit comes from the Medicare Advantage side of the plan, not the Medicaid side. If you only have TennCare and not Medicare, you do not qualify.
Fitness Programs People Confuse with TennCare
Several Tennessee fitness-insurance benefits look like they might apply to TennCare but do not.

YMCA insurance-related memberships are tied to commercial insurance. The YMCA of Middle Tennessee offers a 50% discount on the joining fee for BlueCross BlueShield of Tennessee customers but does not list a TennCare or Medicaid program.13YMCA of Middle Tennessee. Insurance-Related Memberships
Weight Management and Wellness TennCare Does Cover
If your underlying goal is weight loss or better fitness, TennCare covers several things that may matter more than a gym pass.
Since August 2025, TennCare covers FDA-approved weight-management medications, including GLP-1 receptor agonists like Wegovy and Zepbound.14American Diabetes Association. American Diabetes Association Celebrates TennCare Decision To get them, you must be participating in complementary nutritional and lifestyle changes, including dietary modification, increased physical activity, and a structured behavioral intervention or comprehensive weight management program.15OptumRx Content Hub. How to Confirm What Your Plan Covers
Benefits can vary by eligibility category and by managed care plan, so confirm your coverage directly. Call the member services number on the back of your TennCare ID card, or reach TennCare Connect at 855-259-0701.16Tennessee.gov. TennCare Covered Services Ask specifically whether your plan offers any cost-effective alternative services tied to fitness or weight management. Those discretionary offerings do not always appear on a plan’s public website, and if any wiggle room exists for gym-adjacent support, that is where it lives.
