The Florida DCF work calendar, Form CF-ES 3007, is a one-page monthly grid you use to report your daily hours and gross earnings when the Department of Children and Families needs to verify income for SNAP, TANF, or Medicaid. You fill in each workday with the date, gross pay, and hours worked, total the weeks and the month, sign it, and return it to DCF within the deadline in your notice. Caseworkers ask for it when you don’t have standard pay stubs, your hours swing week to week, or you work for yourself.

What to Gather Before You Start
Have your records in front of you before you write anything on the form. For every day you worked in the period the caseworker asked about, you need three things:
- The exact calendar date you performed work.
- The gross pay you earned that day, before taxes or any other deductions.
- The total hours you worked.
Tips, commissions, and bonuses go into the gross pay figure for the day you received them. Bank deposits, handwritten logs, payment-app receipts, and invoices all help you rebuild accurate daily totals. Check your entries against your bank statements before you commit numbers to the form. A gap between what you report and what your deposits show is one of the fastest ways to draw a follow-up verification request.
You also need your employer’s full legal name, physical address, and phone number. If you work for yourself, use your business name and contact information.
Watch the deadline. The form ties to Florida Administrative Code Rule 65A-1.205, which puts the burden on you to provide the information and documentation needed to establish eligibility. If you don’t return the completed calendar within 10 calendar days of a written request, or within 30 days of your application date, whichever is later, your application can be denied.3Legal Information Institute. How to Fill In the Daily Grid
The form is a standard monthly calendar with columns for Sunday through Saturday. In each box for a day you worked, write the date, the gross amount you earned, and the hours you put in.4Florida DCF. CF-ES 3007 Work Calendar Days you didn’t work should be left blank or marked with a zero so the caseworker can tell no-work days from ones you simply forgot.
Once the daily entries are done, add up hours and gross pay for each week. Then add the weekly totals to get your monthly gross income. Recheck the arithmetic. A small addition error can push your reported income above or below reality, which either shrinks your benefit or sends the case back for review. These figures are gross, not take-home.
If You Work More Than One Job
Use a separate CF-ES 3007 for each employer. Each calendar should carry only the hours, pay, and employer details for that one job. Your caseworker adds the totals across all the calendars to figure your household’s earned income.
If You Are Self-Employed
The grid works the same way, but you enter your daily gross receipts, meaning the total you brought in before subtracting any business costs. DCF handles allowable business expenses separately to arrive at your net self-employment income.
Federal SNAP rules require self-employment income to be averaged over the period it is meant to cover. A business that has run for a full year is typically annualized and divided by twelve. A newer business is averaged over the months it has operated and projected forward. Deductible costs include labor, materials, equipment payments, interest on business loans, insurance, and taxes on income-producing property. Signing the Form
Sign and date the form once the totals are in. Your signature certifies the information is accurate. DCF may ask your employer to co-sign as third-party confirmation of the hours and pay. If your employer will not sign or cannot be reached, tell your caseworker. A signed letter on company letterhead or copies of payment records can sometimes stand in.
How to Submit It to DCF

- Fax it or hand-deliver it to your local DCF customer service center. The storefront locator on myflfamilies.com lists addresses and fax numbers.
- Drop it off with a community partner organization that accepts documents on DCF’s behalf.
When you upload through MyACCESS, select “Proof of Income” as the document type so the file gets routed to the right queue. Photograph or scan the page against a flat, dark surface with good lighting. Blurry or unreadable submissions come back for a redo.
What Happens Next
A caseworker compares your calendar against the rest of your case file and cross-checks it with data pulled from other agencies and systems.3Legal Information Institute. If something doesn’t match, for instance if your reported earnings sit well below wage data DCF already has, a written request for additional verification will follow. Report the Real Numbers
Honest arithmetic mistakes generally mean extra paperwork. Intentional misreporting is a separate problem. Federal SNAP rules disqualify you from benefits for 12 months on a first intentional program violation, 24 months on a second, and permanently on a third.8eCFR. 7 CFR 273.16 – Disqualification for Intentional Program Violation Florida also charges public assistance fraud as a criminal offense, ranging from a first-degree misdemeanor for smaller amounts up to felony charges as the dollar value climbs.9Online Sunshine.
