Nevada DMV Form ADM-205 is the payment authorization form that lets you pay by credit or debit card when you mail or fax a request to the Department of Motor Vehicles instead of paying in person. You fill in your card number, billing details, and the exact fee amount, sign it in ink, and send it along with your application. When You Need the ADM-205
Use the form any time you’re sending a paper request to the DMV and want to pay by card rather than check or money order. Common transactions include:

How to Fill Out the Form
Download the current version from dmv.nv.gov before you start, and have your card and your application’s fee schedule in front of you.
Card and Payment Amount
Check the box for Visa, MasterCard, or Discover, then write the full card number and expiration date. The form does not ask for the three-digit CVV code on the back of the card, so do not add it anywhere on the page.1Nevada Department of Motor Vehicles. ADM-205 Payment Authorization Form
Enter the exact dollar amount you’re authorizing. Match it to your application’s fee schedule. A certified driver history, for example, is $11, not $7 — the $4 certification fee is on top of the $7 search fee.2Nevada Department of Motor Vehicles. Driver History Reports

Cardholder Information
Print your full legal name as it appears on the card, then the billing address, city, state, and zip code on file with your bank. Small mismatches cause declines.
The form also has a field for an identifying account number. Fill in whichever one applies to your transaction: license plate number, driver’s license number, business license number, or records account number. Signature and Date
Sign and date the form in blue or black ink. Your signature authorizes the DMV to charge your card for the stated amount on or after the date shown. The DMV does not accept electronic signatures on this form; it needs a wet-ink original by mail or fax. Where to Send It
Bundle the signed ADM-205 with the application it supports and send everything together. Most transactions route through the Carson City office, and the specific application form lists the correct suite or mail stop.

Mistakes That Get Your Packet Returned
- Billing address that doesn’t match your card issuer’s records. If you updated your address with the DMV but not your bank, the charge will decline.
- Missing signature or missing date. Both are required, no exceptions.
- Wrong payment amount. Check the exact fee for your transaction, including any certification or out-of-state surcharges.
- An expiration date that falls before the DMV finishes processing. Mail-in work is not instant, so use a card with several weeks of runway.
- Missing supporting documents. The ADM-205 only handles payment. You still need the underlying application, any required identification, and any affidavits the transaction calls for.7Nevada Department of Motor Vehicles. Public Records Access
Paying for Someone Else’s Records
If you’re using the ADM-205 to pay for another person’s driver history or vehicle record, payment alone will not get the record released. Under NRS 481.063, the DMV requires a written release signed by the person whose records you’re requesting, dated within 90 days of the request.
