Who Can Supervise the Practice Hours
The adult signing off on those 40 hours has to meet specific criteria. Under Georgia law, the supervising driver must be at least 21 years old, hold a valid Class C license, be physically capable of taking control of the vehicle, and sit in the seat directly beside the teen.4Justia. Filling Out the Form
DDS provides the affidavit form on its website, and you can also pick one up at a customer service center. The form asks for the teen’s full legal name as it appears on their Georgia instructional permit, the permit number, date of birth, and residential address, plus the supervising parent’s or guardian’s name and contact information.

Check every field against the teen’s permit before signing. A misspelled name or a wrong permit number is enough to get the form rejected, which means going home and starting over.
Getting It Signed Under Oath

What to Bring to the Road Test
The affidavit is one item in a larger packet. Confirm you have all of the following before the appointment:

The examiner reviews everything before the test begins. Missing or mismatched paperwork means no road test that day.
Penalties for a False Affidavit
Signing the affidavit is a sworn legal act. The parent or guardian is swearing the teen actually completed the hours. Knowingly providing false information on a Georgia state government document is punishable by a fine of up to $1,000, imprisonment for one to five years, or both.8Justia.
