The Georgia DDS-MR-274 vision report form is a free document you download from the Department of Driver Services website, have a licensed optometrist or ophthalmologist complete after an eye exam, and return to DDS online, in person, or by mail. You need it when you fail the vision screening at a Customer Service Center, or when you are 64 or older and want to renew your license online instead of taking the screening in person.1Georgia Department of Driver Services. When You Need the Form
Three situations put this form in your hands.
The first is failing the vision screening at a DDS Customer Service Center during a license application or renewal. The examiner will tell you a Vision Report Form completed by a licensed optometrist or ophthalmologist is required before your visit can move forward.2Georgia Department of Driver Services. The second is online renewal at age 64 or older. Georgia law requires drivers 64 and over to pass a vision screening at each renewal.3Georgia Code. The third is a DDS Medical Review request, where the department has been asked by a doctor, relative, judge, law enforcement officer, or other identified party to evaluate your fitness to drive and has directed you to submit a vision report.4Georgia Department of Driver Services. Drivers 64 and Over There is no fee for the form itself. Your eye exam is a separate cost between you and your provider; DDS does not set or reimburse it.
How to Fill It Out
The form has three parts. You complete the first; your eye doctor completes the other two.
Patient Information
Fill this section in before or at the appointment. It asks for your full name, date of birth, street address, city, state, ZIP code, and Georgia driver’s license number, plus your signature and the date.5Georgia Department of Driver Services. Georgia Department of Driver Services Vision Report
Report on Visual Examination
Your eye doctor records the clinical results during the exam:
- Best corrected visual acuity for each eye and both eyes together, taken without corrective lenses, with corrective lenses, and (if applicable) with a bioptic telescope.
- Horizontal field of vision measured in degrees for the right side, the left side, and total.
- Whether you have functional vision in only one eye. If so, the doctor records the nasal and temporal field measurements in degrees.
If your vision is corrected with anything other than standard glasses or contact lenses, the doctor checks a designated box and attaches a report explaining the correction.5Georgia Department of Driver Services. Georgia Department of Driver Services Vision Report
Physician’s Statement and Acknowledgement
The doctor answers three questions: whether you have double vision and whether it is correctable, whether there is evidence of eye disease or injury, and whether in their professional opinion you have sufficient vision to safely operate a motor vehicle. If yes, they indicate whether any driving restrictions apply and check the relevant codes.
The acknowledgement block at the bottom requires the practice name, the doctor’s full name, specialty, license number and state, office address, phone number, signature, and exam date.5Georgia Department of Driver Services. Check them before you leave the office.
How to Submit the Completed Form
You have three routes.
In person. Bring the completed original to any DDS Customer Service Center. This is the standard path when you failed the in-office screening and are returning with the report. The examiner reviews it on the spot and continues processing your license if it checks out.
Online. If you are 64 or older and renewing, log into your DDS Online Services account, open “Online Licensing Services,” and choose “Submit Vision Documents.” Upload a scan or clear photo of the form. It must be dated within the last two years and include the acuity readings, horizontal perception degrees, or monocular field measurements.1Georgia Department of Driver Services. Drivers 64 and Over DDS emails you an approval, a denial, or an “incomplete” notice. Approved means you can finish renewing online. Denied means you need to visit a Customer Service Center. Incomplete means fix what’s missing and resubmit.

Keep a photocopy if you mail it. This route is used mainly when a medical review case is already open and your doctor is sending documentation directly to that unit.
What the Form Is Measured Against
For a standard (noncommercial) Georgia license, you need visual acuity of at least 20/60, corrected or uncorrected, in at least one eye.7Justia Law. Georgia Code 40-5-27 – Examination of Applicants Your horizontal field of vision must be at least 140 degrees with both eyes open. If only one eye has usable vision, the field requirement is 70 degrees temporal and 50 degrees nasal.8Legal Information Institute. Georgia Code 375-3-5-.08 – Vision Those are the numbers the physician’s statement is judging.
Restrictions That May Appear on Your License
If the doctor says you can drive safely but with limitations, the codes checked on the form carry over to your license:5Georgia Department of Driver Services. Georgia Department of Driver Services Vision Report
- B — Corrective lenses required while driving.
- G — Daylight hours only.
- F — Right exterior mirror required.
- I — Left exterior mirror required.
- R — No highway or interstate driving.
- 1 — Bioptic lenses required.
If you had corrective surgery such as LASIK and no longer need the lenses restriction, you can visit a Customer Service Center, pass the standard vision screening, and have it removed.6Georgia Department of Driver Services. Medical and Vision Information
When the DDS-MR-274 Is Not Enough by Itself
Two situations require more than this form.
If your acuity falls below 20/60 but is better than 20/200 with standard correction, you may still qualify to drive through Georgia’s bioptic telescopic lens program. That path involves prescribed telescopes, certified training, an on-the-road evaluation with a DDS examiner, a driver’s education course completed while wearing the bioptics, and written and behind-the-wheel tests at a DDS-operated site.7Justia Law. Georgia Code 40-5-27 – Examination of Applicants Bioptic drivers also renew every four years instead of eight and see their eye doctor at least every two years.
Commercial drivers use a different federal form. If a CDL holder fails the vision screening, the Vision Evaluation Report (MCSA-5871) is what gets completed to pursue an FMCSA exemption, not the DDS-MR-274.6Georgia Department of Driver Services. Medical and Vision Information CDL vision standards are stricter: 20/40 in each eye and both together, 70 degrees of field in each eye, and the ability to distinguish red, green, and amber.
