Georgia Form T-138: IRP Schedule A for New Accounts and Added Vehicles

Georgia Form T-138 is the IRP Vehicle Schedule you submit to register a commercial vehicle under the International Registration Plan, and since July 2023 it functions as a supporting document uploaded through Georgia’s EZ IRP online system rather than a standalone paper filing. You use it to open a new IRP account, add vehicles to an existing fleet, or update vehicle information. The form itself captures vehicle-level details; the harder part of getting it accepted is having the right supporting documents ready and knowing which portal handles your situation.

What T-138 Asks For

The schedule collects vehicle-by-vehicle information for every truck you want registered under the account. For each unit, you enter:

  • Vehicle Identification Number (VIN)
  • Make and model year
  • Fuel type
  • Unladen weight
  • Desired gross vehicle weight for each jurisdiction where the vehicle will operate

The VIN you enter must match the VIN on the title exactly. Weight entries drive both your fee calculation and the maximum operating weight printed on your cab card, so the numbers you list are the numbers you’ll be held to at inspection.

Do You Actually Need to File It

T-138 belongs to IRP registration, which applies when a vehicle operates in two or more jurisdictions (states or Canadian provinces) and meets at least one of these thresholds:

  • Gross vehicle weight over 26,000 pounds, alone or in combination with a trailer
  • Three or more axles, regardless of weight

If your operation is entirely within Georgia, or the vehicle falls below both thresholds, IRP registration isn’t the right track and T-138 isn’t the right form.1International Registration Plan, Inc. International Registration Plan

Documents to Gather Before You Start

Georgia’s system requires you to upload supporting documents during the application itself, so save each one as a PDF on your computer before logging in. Multi-page documents have to be combined into a single file before uploading.

  • A copy of the applicant’s valid driver’s license
  • Georgia Vehicle Title Receipt or IRP Registration Only Title Record for each vehicle
  • Proof of liability insurance meeting Georgia’s IRP requirements
  • IRP Equipment Lease Agreement if you’re leasing equipment; IRP Employment Lease Agreement as well if the applicant isn’t the carrier responsible for safety
  • Power of Attorney (Form RD-1061) if someone other than the applicant is filing, or if a Registered Bonded Service Provider is handling the registration
  • IRS HVUT 2290 Checklist reflecting every VIN that will be registered above 54,999 pounds
  • Actual distance traveled by jurisdiction from the previous July 1 through June 30, or estimated mileage if you don’t have a full reporting period yet

Prerequisites That Have to Be in Place First

Three things need to exist before your T-138 will clear review. Missing any one of them stalls the whole application.

Georgia Base Documentation

You must prove either an Established Place of Business or Georgia Residency. Georgia accepts five documents from the Established Place of Business list or three from the Georgia Residency list. You cannot mix documents between the two lists, every document has to show the same address, and PO boxes and virtual offices do not qualify. Your fleet also has to produce Georgia miles, and distance records must be kept or accessible in the state.3Georgia Department of Revenue. IRP Open a New Account

Active USDOT Number

Interstate carriers need an active USDOT number from the Federal Motor Carrier Safety Administration before applying. Get one through FMCSA’s online registration if you don’t already have it.

Federal Heavy Vehicle Use Tax

Any vehicle with a taxable gross weight of 55,000 pounds or more needs proof of HVUT payment. You file IRS Form 2290 and receive a stamped Schedule 1 as your receipt. States will not register a taxable vehicle without it.4Internal Revenue Service. Filing T-138 for a New IRP Account

New account applications go through the EZ IRP portal. Emailed or mailed paper submissions are no longer accepted for new accounts.5Georgia Department of Revenue. Georgia Trucking Portal

Log into EZ IRP and request a new IRP account. Upload the completed T-138 vehicle schedule along with Schedules A and G and the supporting documents listed above. Allow five business days for a response through the portal, which will be either approval or a list of items to correct. If it isn’t approved on the first pass, fix whatever the reviewer flagged and resubmit through the same portal.3Georgia Department of Revenue. IRP Open a New Account

Once approved, you pay for a Temporary Apportioned Permit that lets you operate legally while permanent credentials are processed. Full IRP login credentials come once all requirements are satisfied. Payment is by e-check or credit card.6Georgia Department of Revenue. IRP Replace Cab Cards

Filing T-138 to Add a Vehicle to an Existing Account

If you already have a Georgia IRP account, adding a truck goes through the main IRP application rather than EZ IRP. It’s a two-step process with a built-in review period.

Log into the Georgia IRP system, open the Applications tab, select IRP, and choose Add Vehicle from the Supplement tab. On the Supplement Search screen, click Proceed without filling in the empty fields, then pick the fleet registration year the vehicle belongs to. If the new truck fits an existing weight group, keep going. If it doesn’t, add a new weight group first through the Weight tab.2Georgia Department of Revenue. IRP Add Vehicles

On the Vehicle Details screen, enter the VIN and fill every field marked with a red asterisk. Leave the Safety Change field set to No. After verifying the vehicle information, move to the Submission screen and upload each required document by picking the document type from the dropdown and browsing to your saved PDF. One timing point: vehicles added during an open renewal period can only be added to the current registration period and are not guaranteed continuous registration into the next year.

What Your Invoice Will Reflect

After the T-138 and supporting documents clear review, Georgia issues an invoice. It combines three things: Georgia’s base registration fee for the weight class, the alternative ad valorem tax set by statute for that weight class, and apportioned fees owed to every other jurisdiction where the fleet operates. Georgia’s share of the base fee is calculated by multiplying the annual rate by the percentage of total fleet miles driven in the state, which is why the mileage figures you report matter as much as the vehicle details.7Justia. For-hire (common or contract) carriers pay higher base rates than not-for-hire operations at every weight class, so make sure the carrier type on your application matches your actual operation.

After Payment: Plate and Cab Card

Once the invoice is paid, the Department of Revenue issues an apportioned license plate and cab card. The cab card lists every jurisdiction where the vehicle is authorized to operate and the maximum weight allowed in each. Keep it inside the vehicle at all times. Operating without it can bring fines or an out-of-service order at a roadside inspection.
