Hospital Presumptive Eligibility in California: Coverage and Duration

Hospital Presumptive Eligibility in California gives you temporary Medi-Cal coverage the same day you walk into a participating hospital, with no documents required and no waiting period. Trained hospital staff take your self-reported information, run it through the state’s provider portal, and hand you an Immediate Need Eligibility Document that works as a temporary Medi-Cal card. The coverage bridges the time between showing up for care and finishing the full Medi-Cal application, and it typically runs through the end of the month after the month you’re approved.1Medicaid.gov. Who Qualifies

Three basic conditions apply: you live in California, you’re not already enrolled in Medi-Cal, and your household income sits at or below 138 percent of the federal poverty level.2Department of Health Care Services. Hospital Presumptive Eligibility Aid Code Table For 2026, the annual income ceilings are roughly:3HHS ASPE. 2026 Poverty Guidelines – 48 Contiguous States

  • 1 person: about $22,025
  • 2 people: about $29,863
  • 3 people: about $37,702
  • 4 people: about $45,540

Add roughly $7,875 for each additional person. The hospital takes your word on income, so pay stubs aren’t required at the door.

The program covers children under 19, parents and caretaker relatives, adults ages 19 through 64 who aren’t on Medicare, former foster youth ages 18 through 26 who were in foster care on their 18th birthday in any state, pregnant individuals, and adults 65 and older. Former foster youth have all income disregarded, so the ceiling above doesn’t apply to them.

Non-pregnant individuals can receive HPE only once in any 12-month period. Pregnant individuals can qualify once per pregnancy, no matter how recently they last received HPE.

Finding a Participating Hospital

Not every California hospital does this. A hospital has to notify the Department of Health Care Services that it elects to make presumptive eligibility determinations, and its staff has to complete training before submitting applications.4eCFR. 42 CFR 435.1110 – Presumptive Eligibility Determined by Hospitals DHCS publishes a downloadable list of qualified HPE providers with names, addresses, and phone numbers.5Department of Health Care Services. Qualified HPE Providers List If you’re headed somewhere for urgent care and aren’t sure, call the facility and ask whether they handle HPE applications.

What Happens at the Hospital

A trained staff member helps you complete DHCS form 7022 and submits it through the Medi-Cal Provider Portal. Everything runs on self-attestation. You report your identity, address, household size, and estimated income, and the hospital enters it as given. No pay stubs, no utility bills, no ID verification at this stage.6Department of Health Care Services. Hospital Presumptive Eligibility Program

The portal returns a real-time decision. If it accepts you, the hospital prints two copies of the response. One is your Immediate Need Eligibility Document, which you sign and keep as your temporary Medi-Cal card. The other stays in the hospital’s records.7Department of Health Care Services. What HPE Covers

HPE coverage is fee-for-service, not managed care.6Department of Health Care Services. Hospital Presumptive Eligibility Program For most eligible groups that means full-scope Medi-Cal benefits: doctor visits, lab work, prescriptions, and hospital services.

Pregnant individuals are the exception. HPE coverage for pregnancy is limited to ambulatory prenatal services.7Department of Health Care Services. Hospital Presumptive Eligibility User Guide Inpatient services such as labor and delivery are not covered under the temporary status. Delivery coverage only kicks in once you’re approved for ongoing Medi-Cal, so filing the full application quickly matters more if you’re pregnant.

Because HPE runs fee-for-service, some outside providers may not know how to bill it. Before getting treatment somewhere other than the hospital that enrolled you, confirm the provider accepts Medi-Cal fee-for-service and knows you’re on temporary presumptive eligibility.

How Long the Coverage Lasts

Coverage starts the day the hospital determines you’re eligible and ends on the last day of the following month. Approved July 10, for example, and you’re covered through August 31.1Medicaid.gov. California State Plan Amendment 13-0027-MM7

That end date is a hard deadline unless you submit a full Medi-Cal application before it hits. If you file in time, temporary coverage continues until the county makes a final decision, whether that’s approval or denial.1Medicaid.gov. California State Plan Amendment 13-0027-MM7 Miss the deadline and HPE ends with no extension available.

You have roughly six to eight weeks, and the hospital hands you the application on approval day. Don’t wait until the last week.

Filing the Full Medi-Cal Application

The form you need is the Single Streamlined Application. You can submit it three ways:

  • Online at BenefitsCal (benefitscal.com), California’s statewide benefits portal
  • By mail or in person at your county social services office
  • By phone through your county’s Medi-Cal office

The online route is fastest and creates a dated electronic record, which helps if there’s any later dispute about whether you filed on time. The same application covers Medi-Cal, Covered California marketplace plans, and other coverage programs, so one submission handles all of them.

If the county approves you, coverage transitions from HPE to regular Medi-Cal with no gap. If the county denies you, HPE ends on the date of that denial. Services delivered while HPE was active stay covered; you won’t be billed retroactively for care received during the temporary period. You may also qualify for up to three months of retroactive Medi-Cal from the date of your application if you had qualifying medical expenses during that window.1Medicaid.gov. California State Plan Amendment 13-0027-MM7

If You’re Denied Medi-Cal

A denial isn’t the end of the road. With income above Medi-Cal limits but below 400 percent of the federal poverty level, you likely qualify for subsidized coverage through Covered California. Premium tax credits reduce your monthly costs as long as you aren’t enrolled in qualifying government coverage like Medicaid.8Internal Revenue Service. Eligibility for the Premium Tax Credit

Skipping coverage after HPE ends carries real financial risk. California has its own individual health coverage mandate, separate from the federal one. For 2025, the penalty was $950 per uninsured adult and $475 per child, or 2.5 percent of household income above the tax filing threshold, whichever is higher.9Franchise Tax Board. Personal Health Care Mandate A family of four could owe $2,850 or more. The amount adjusts annually, so 2026 figures may be slightly higher once the Franchise Tax Board updates them.

A few exemptions exist. You won’t owe the penalty if your income is below the tax filing threshold, if the cheapest available coverage would cost more than about 7.28 percent of your household income, or if your coverage gap lasted three consecutive months or less.9Franchise Tax Board. Personal Health Care Mandate People on limited-scope Medi-Cal are also exempt. Let HPE lapse without applying for anything else, though, and expect the penalty on your state tax return.
