Maryland doesn’t issue an IRS-style tax transcript. To get your Maryland state tax information online, sign in to Maryland Tax Connect, the Comptroller’s portal, where you can view your filing and tax history for prior years.1Comptroller of Maryland. NEW Maryland Tax Connect Portal Information – Taxpayer Services If you need an official, certified copy of a return you filed, that isn’t available online at all: you have to request it using Form 129, either in person at a Comptroller branch office or by mail.2Comptroller of Maryland. 2025 Maryland Form 129 Request for Copy of Tax Return
Which one you need depends on who’s asking. For your own records or to fill in numbers on another form, the online view is usually enough. For a mortgage lender, court, or government agency that wants a stamped official record, you need the Form 129 copy.

Register with an email you can check right at the moment of login, because the passcode is time-sensitive.
Nothing you view or download from the portal is a certified document. If a lender accepts self-reported figures or a plain screenshot, the online view will do. If they need the Comptroller’s seal, keep reading.
Getting a Certified Copy With Form 129
Form 129 (Request for Copy of Tax Return) is the only way to obtain an official Maryland copy that carries the weight of a state record. The Comptroller provides the copies at no charge.2Comptroller of Maryland. 2025 Maryland Form 129 Request for Copy of Tax Return
The form asks for the taxpayer’s name and address as they appeared on the original return, your current mailing address, your Social Security Number (and your spouse’s, if joint), and the specific tax year or years you need. If you only want copies of W-2s, check the designated box on line 1a and list the employer’s name, address, and EIN on the back of the form.2Comptroller of Maryland. 2025 Maryland Form 129 Request for Copy of Tax Return
Submitting Form 129 in Person
Bring your completed Form 129 and a valid photo ID to any Comptroller branch office. Do not sign the form in advance. A Comptroller representative will have you sign it in their presence.2Comptroller of Maryland. 2025 Maryland Form 129 Request for Copy of Tax Return Branch offices are in Annapolis, Baltimore (multiple locations), Bel Air, Cumberland, Frederick, Greenbelt, Hagerstown, Salisbury, Waldorf, Wheaton, and Windsor Mill.4Comptroller of Maryland. Submitting Form 129 by Mail
If you mail the form, your signature generally has to be notarized first. Copies come back by U.S. Postal Service only. The Comptroller will not email, scan, or fax them.2Comptroller of Maryland. 2025 Maryland Form 129 Request for Copy of Tax Return The instructions don’t give a firm turnaround, so build in several weeks of lead time if you’re working toward a loan closing or application deadline.
If Someone Else Is Requesting on Your Behalf
The Comptroller won’t release tax information to a third party without a power of attorney. Your representative must file Form 548 with the Revenue Administration Division, and the representative has to be an individual, not a firm. Form 548 has to name specific tax types, forms, and years; blanket language like “all years” is invalid. Unless the representative is an attorney, CPA, or enrolled agent, a copy of your government-issued photo ID has to accompany the form.6Comptroller of Maryland. 2024 Maryland Form 548 Power of Attorney Instructions
One useful shortcut: if a valid Form 548 is already on file, the representative doesn’t have to submit a separate Form 129 to request your return.2Comptroller of Maryland. 2025 Maryland Form 129 Request for Copy of Tax Return
For a deceased taxpayer, the personal representative of the estate can sign Form 129, but a copy of the letters of administration from the probate court must accompany the request.2Comptroller of Maryland. 2025 Maryland Form 129 Request for Copy of Tax Return
Confirm What the Lender or Agency Actually Wants
Requesters sometimes ask for a “state tax transcript” as a habit, without knowing Maryland doesn’t produce one under that name. Before you start, ask whether a certified copy of your Maryland Form 502 will satisfy their requirement. It usually does, since the certified copy shows the same core data a transcript would: adjusted gross income, filing status, and the tax liability or refund. Getting that answer up front can spare you from waiting weeks on a document nobody actually needed.
