How Does Medi-Cal Share of Cost Work in California?

The Medi-Cal Share of Cost is a monthly dollar amount of medical expenses you have to incur before Medi-Cal starts paying for your care that month. It works like a deductible that resets on the first of every month. You don’t send this money to the state; you pay providers (or agree to pay them) until your bills reach your Share of Cost figure, and then Medi-Cal covers eligible expenses for the rest of the month. A 2025 reform substantially raised the income threshold used in the formula, which lowered or eliminated the Share of Cost for many people, and asset limits returned in 2026 after a two-year pause.

Who Gets Assigned a Share of Cost

A Share of Cost is assigned to people who qualify for Medi-Cal through the Medically Needy pathway, meaning your income is too high for a free eligibility category but you still qualify because federal Medicaid rules let states cover people who contribute “excess” income toward their medical costs.1DHCS – CA.gov. Medi-Cal Overview If you qualify through an income-based (MAGI) category or one of the zero-Share-of-Cost programs described later in this article, you don’t have one at all.

The Share of Cost applies month by month. Meet it in March and Medi-Cal pays your remaining March bills; on April 1, the counter goes back to zero and you start over.

How Your Share of Cost Is Calculated

Your county eligibility worker uses a formula set in state law: monthly countable income, minus the Maintenance Need Standard for your household size, minus qualifying deductions such as health insurance premiums. What’s left is your monthly Share of Cost.2California Legislative Information. California Welfare and Institutions Code 14005.7

The Maintenance Need Standard, or MNS, represents the minimum income the state assumes you need for non-medical living costs like housing and food. For decades it sat at $600 per month for one person and $934 for a couple. Under Share of Cost reform, the MNS was raised to 138% of the Federal Poverty Level for your family size. For 2026, the Federal Poverty Level for one person in the 48 contiguous states is $15,960 per year, so 138% is roughly $1,835 per month.3U.S. Department of Health and Human Services, Office of the Assistant Secretary for Planning and Evaluation. 2026 Poverty Guidelines: 48 Contiguous States That is more than triple the old figure, and the practical result is that many people who used to face a steep Share of Cost now have a much smaller one or none.

A Worked Example

Say you’re a single adult receiving $2,400 per month in Social Security and paying a $185 monthly Medicare Part B premium. Your county would calculate:

  • Gross monthly income: $2,400
  • Minus Medicare Part B premium: −$185
  • Minus MNS (138% FPL for one person): −$1,835
  • Monthly Share of Cost: $380

Under the old $600 MNS, that same person would have been assigned a Share of Cost of $1,615. The reform cut the obligation by more than 75%.

2026 Maintenance Need Standard by Family Size

Because the MNS is now tied to the poverty guidelines, it adjusts each year. Based on the 2026 Federal Poverty Level, the approximate monthly amounts are:

  • 1 person: $1,835
  • 2 people: $2,489
  • 3 people: $3,142
  • 4 people: $3,795

Deductions That Lower Your Share of Cost

The county doesn’t work from your gross income. Several deductions come off before the formula runs, and using every one you qualify for is the most direct way to reduce what you owe each month.

  • Health insurance premiums. Monthly payments for Medicare Part B, Medicare Part D, Medigap supplements, dental plans, and vision plans are all subtracted from your income before the calculation.   • Earned income disregards. If you work, part of your earnings is excluded from countable income under standard Medi-Cal budgeting rules.
  • Dependent allowances. If you support dependents, the county subtracts an allowance for each one before running the formula.

If your figure looks high, ask your county eligibility worker to walk you through the deductions applied to your case. Enrolling in a low-cost Part D or dental plan specifically to claim the premium deduction is a common strategy and can shrink a Share of Cost by a meaningful amount.

How to Meet Your Share of Cost Each Month

The process of accumulating enough medical expenses to reach the threshold is called the spend-down. Until you meet it, Medi-Cal pays nothing that month. Once you do, coverage activates for the entire calendar month.

What Counts Toward the Spend-Down

All medically necessary services count, whether or not Medi-Cal would ordinarily cover them: doctor visits, prescriptions, medical supplies, lab work, dental care, and over-the-counter items recommended by a provider.4CA.gov. Share of Cost (SOC) – Part 1 You don’t have to pay the bills before they count. Bills you’ve been charged for but haven’t yet paid still qualify, provided you’re legally responsible for them.

Using Old Unpaid Medical Bills

This is where anyone with a high Share of Cost should pay close attention. Under a longstanding court order, you can apply old unpaid medical bills from prior months toward the current month. Unpaid bills of a family member in your household may also count.5LA County DPSS. Share of Cost Flyer PA 6034 Each bill can be credited only once, so you can’t reuse the same one repeatedly, but a stack of old medical debt can knock out several months of obligations.

There’s a timing rule. Old bills submitted to your county eligibility worker must have been issued within 90 days of the date you hand them over.6Department of Health Care Services (DHCS). Once the county verifies the expenses reach your threshold, coverage becomes effective for the rest of the month.

Retroactive Coverage for Bills Before You Applied

If you had unpaid medical or dental bills during the three months before the month you applied for Medi-Cal, you can request retroactive coverage for those months. You must have been eligible during the month the services were provided, and you need to contact your county office within one year of when those services were rendered.6Department of Health Care Services (DHCS). What Happens at the Provider’s Office

A provider cannot bill Medi-Cal for your care until your Share of Cost is certified as met in the state’s electronic eligibility system. If the system shows it hasn’t been met, the claim will be denied.7CA.gov. Share of Cost (SOC) – Medi-Cal Providers That creates an access problem. Some providers are reluctant to see Share of Cost patients early in the month because there’s no guarantee of reimbursement.

Providers have two options when your threshold isn’t yet met. They can collect the amount from you at the time of service, or they can let you “obligate” payment, meaning you agree to pay later or through an installment plan. If the provider lets you obligate and you never pay, Medi-Cal won’t reimburse them for that portion.7CA.gov. Programs That Eliminate the Share of Cost

Several Medi-Cal categories provide full-scope coverage with no Share of Cost. If you qualify for any of these, you skip the Medically Needy pathway and the monthly spend-down altogether. Ask your county eligibility worker to evaluate you for each one before accepting a Share of Cost determination.

Aged, Blind, and Disabled Federal Poverty Level Program

The ABD FPL program is the most common zero-Share-of-Cost route for seniors and people with disabilities. You qualify if you’re 65 or older, blind, or disabled, and your countable income after deductions falls at or below 138% of the Federal Poverty Level.8Santa Clara County Social Services Agency. Aged, Blind and Disabled Federal Poverty Level (FPL) Program Qualifying under this program locks in zero-cost coverage rather than requiring monthly spend-down paperwork, and it uses specific deductions that may differ from the Medically Needy calculation.

250% Working Disabled Program

This program is for people with disabilities who work. You must meet the Social Security Administration’s definition of disability, be earning income, and have countable family income below 250% of the Federal Poverty Level.10DHCS – CA.gov. Pickle Amendment

If you’re aged, blind, or disabled and once received Social Security (RSDI) and SSI/SSP at the same time, but lost SSI because of Social Security cost-of-living increases, you may qualify for zero-Share-of-Cost Medi-Cal under the Pickle Amendment. All of the following must be true: you currently receive Social Security benefits; you received both Social Security and SSI simultaneously in at least one month since April 1977; you no longer receive SSI; and your countable income and resources fall within Pickle limits.12California Department of Health Care Services. Medi-Cal Eligibility Division Information Letter No. I 24-05 – Pickle Amendment The county essentially freezes your Social Security amount at what it was when you last received SSI and ignores all later cost-of-living increases for eligibility purposes.

Disabled Adult Child Protections

Adults who became disabled before age 22 and lost SSI because they began receiving Social Security benefits on a parent’s record have a separate path to zero-Share-of-Cost coverage. The county disregards the entire RSDI amount received as child’s benefits and any later cost-of-living increases.13Santa Clara County Social Service Agency. To qualify, you must be at least 18, currently receiving disability-based RSDI, and have lost SSI specifically because of entitlement to those child disability benefits on or after July 1, 1987.

Asset Limits Returned in 2026

From January 1, 2024 through December 31, 2025, California had no asset limits for Medi-Cal. That window closed. As of January 1, 2026, asset limits are back for applicants and members whose eligibility uses non-MAGI financial methods, which includes the Medically Needy pathway that carries a Share of Cost.14Department of Health Care Services. The new limits are far more generous than the old SSI-based $2,000 limit. The 2026 limit is $130,000 for one person, with $65,000 added for each additional household member up to 10 people.15DHCS – CA.gov. Your primary home is exempt regardless of value, as long as you live there or intend to return, and it also stays exempt if your spouse, registered domestic partner, or a dependent relative lives in it.15DHCS – CA.gov. Asset Limit Frequently Asked Questions A second home or investment property counts. So do bank accounts, stocks, and similar financial resources above the limit.

Also starting January 1, 2026, a 30-month look-back period applies to anyone applying for Medi-Cal nursing home coverage. The state will examine assets you gave away or transferred during the 30 months before entering the facility. Transfers made before January 1, 2026 are not subject to review. Nursing Home Residents Follow Different Rules

If you live in a nursing home on Medi-Cal, you don’t have the standard Share of Cost. Instead, you turn over nearly all of your monthly income to the facility. The county starts with your gross monthly income, subtracts any health insurance premiums you pay out of pocket, and subtracts a $35 personal needs allowance. The remainder goes to the nursing home, and Medi-Cal covers the balance of the facility’s charges. When one spouse enters a nursing home and the other stays at home, spousal impoverishment protections reserve a minimum monthly maintenance needs allowance for the community spouse ($4,067 in 2026) before the resident’s contribution is calculated.

Reporting Changes and Appealing Your Share of Cost

Your Share of Cost is recalculated whenever your income or household size changes. You have to report changes to your county eligibility office within 10 days rather than waiting until annual renewal. If you don’t report a drop in income, you could overpay for months. If you don’t report an increase, the county may eventually catch it and pursue an overpayment.

If you think your Share of Cost was calculated wrong, you have the right to a state fair hearing. The standard filing window is 90 days from when the county mails your notice of action, though a temporary extension to 120 days has been in effect for actions tied to Medi-Cal redetermination, including Share of Cost increases.16California Department of Social Services.
