To apply for Medicaid in Arizona, submit an application for AHCCCS (the Arizona Health Care Cost Containment System) online through the Health-e-Arizona Plus portal, by mail, by fax, or in person at a Department of Economic Security (DES) office.1Arizona Department of Economic Security. How to Apply for Medical Assistance Most adults qualify with household income at or below 138 percent of the Federal Poverty Level, roughly $22,025 a year for an individual or $45,540 for a family of four in 2026.2AHCCCS. AHCCCS Eligibility Requirements A decision usually arrives within 45 days, and approved coverage reaches back to the first day of the month you filed.
Check Whether You Qualify Before You File
AHCCCS eligibility turns mostly on household income measured against the Federal Poverty Level (FPL). Arizona sets its adult limit at 133 percent of FPL, but a 5 percent income disregard means you effectively qualify up to 138 percent.2AHCCCS. AHCCCS Eligibility Requirements For 2026, the federal guideline is $15,960 a year for one person and $33,000 for a family of four.3HHS ASPE. 2026 Poverty Guidelines – 48 Contiguous States
The thresholds differ by category:
- Adults 19 to 64: 133 percent FPL (effectively 138 percent). A single adult can earn up to about $1,769 per month before the disregard.
- Children under age 1: 147 percent FPL.
- Children ages 1 to 5: 141 percent FPL.
- Children ages 6 to 18: 133 percent FPL.
- Pregnant women: 156 percent FPL, the highest limit in the program.
Dollar limits rise with household size, so a family of three qualifies at a higher monthly figure than a single person at the same FPL percentage.2AHCCCS. AHCCCS Eligibility Requirements
You also must be an Arizona resident and either a U.S. citizen or a qualified immigrant. People who do not meet the citizenship requirement can still receive emergency medical services through AHCCCS if they otherwise qualify.2AHCCCS. AHCCCS Eligibility Requirements
If your income is above these cutoffs, children in your household may still qualify for KidsCare, Arizona’s CHIP program. KidsCare charges a modest monthly premium capped at $70 per household and covers families earning up to about $6,188 a month for a family of four.4AHCCCS. KidsCare – Arizona’s Children’s Health Insurance Program The same application serves both programs.
Long-term care has its own track. The Arizona Long Term Care System (ALTCS) covers people who need a nursing-home level of care and uses stricter rules, including a $2,000 individual resource limit and a medical pre-admission screening.2AHCCCS. AHCCCS Eligibility Requirements ALTCS has a separate phone and fax process, described below.
Documents to Gather First
Missing paperwork is the most common reason applications stall. Pulling everything together before you start lets you file a complete package the first time. You will need:
- Social Security numbers and dates of birth for every household member seeking coverage.
- Proof of Arizona residency, such as a recent utility bill or signed lease.
- Proof of citizenship or immigration status: birth certificate, U.S. passport, or immigration documents.
- Proof of income: pay stubs from the past 30 days, recent tax returns, or business records if you are self-employed.5AHCCCS. Verifying Self-Employment Income
- Information on any other health insurance available to you, including employer-sponsored coverage.
Applying for ALTCS requires more: bank and investment statements documenting all assets, medical records showing the need for long-term care, and contact information for your providers so the eligibility specialist can verify your condition.

If You’re in the Hospital Right Now
Ask the hospital about Hospital Presumptive Eligibility. Qualified hospitals can temporarily enroll you in AHCCCS on the spot so your care is covered while a full application is processed. The temporary coverage ends when the regular application is decided, so you still need to submit one.9AHCCCS. Hospital Presumptive Eligibility
What Happens After You File
An eligibility specialist reviews your documents and may contact you to clarify income or residency. The decision comes by mail or through your Health-e-Arizona Plus account within one of three windows:1Arizona Department of Economic Security. How to Apply for Medical Assistance
- Hospitalized applicants: within 7 days.
- Pregnant applicants: within 20 days.
- Everyone else: within 45 calendar days.
Once approved, coverage is retroactive to the first day of the month you applied. File on March 15 and your coverage starts March 1. That matters if you received care between filing and approval.10AHCCCS. Changes to Retroactive Coverage Frequently Asked Questions
Two groups get more. Children under 19 can receive prior-quarter coverage going back before the month of application. Pregnant women, including up to 60 days postpartum, can receive coverage for up to three months before the application month, as long as they would have qualified during those months.10AHCCCS. If Your Application Is Denied
You have the right to appeal if your application is denied, your benefits are stopped, or the agency doesn’t act within the required timeframe.1Arizona Department of Economic Security. How to Apply for Medical Assistance The denial notice spells out how to file and the deadline for doing so.
An appeal is a formal hearing where you can submit additional documentation or correct errors. Common reasons for denial are incomplete paperwork, unreported income, and failing to respond to a request for information. If the fix is a missing document, gathering it and attaching it to your appeal is often enough. Keep copies of everything you submit, including your original application date, so you have a clear record if the process drags on.
Keeping Coverage Once You’re Approved
Report income changes to AHCCCS as soon as you know about them, or within 10 calendar days for unexpected changes. Not reporting can cost you coverage or create a debt for benefits you weren’t entitled to.11AHCCCS. Income Changes
AHCCCS renews eligibility periodically. Sometimes it can confirm your continued eligibility automatically and will simply send a letter. Otherwise you’ll get a pre-filled renewal form; you have 30 days to review it, correct anything that’s changed, attach any requested proof, and return it.12AHCCCS. Renewal Processes
If coverage ends because you missed a renewal, submitting the completed renewal form within 90 days of the discontinuance date can restore your eligibility without a brand-new application.12AHCCCS. Renewal Processes That 90-day window is far easier than starting over.
