Primary documents include an original or certified birth certificate, a U.S. passport book or card, a Certificate of Naturalization, a Permanent Resident Card (I-551), or a military ID (active duty, dependent, or DD-214). Applicants under 18 who use a passport also need a birth certificate or proof of custody.
Secondary documents include a photo ID from another state’s motor vehicle department, a Social Security card, or a Louisiana college or high school photo ID. If you’ve lost an out-of-state license, bring an official driving record from the state that issued it.
