First, Check That You Actually Want to Cancel

Medi-Cal is administered at the county level, so if you’d rather speak with someone, contact your county human services office by phone, in person, or by mail. You can find yours through BenefitsCal or through your county’s department of social services website.
Have your Medi-Cal Beneficiary Identification Card (BIC) number, Social Security number, and date of birth ready before you call. Write down the representative’s name, the date, and any confirmation number they give you. If you visit in person, bring copies of your documents rather than originals so you can leave them there. Put the Request in Writing
Some counties accept verbal reports over the phone, but a signed letter creates a paper trail that protects you. Keep it brief. For clear-cut situations like moving out of state or the death of a beneficiary, California allows the county to terminate Medi-Cal without a full redetermination. The eligibility worker records the event, certifies that no redetermination could keep the person eligible, and sends a Notice of Action explaining the basis for termination.
Documents to Have Ready
What you need depends on why you’re leaving:
- New employer insurance: a copy of the new insurance card or a letter from your employer confirming coverage and its start date.
- Income increase: recent pay stubs, an offer letter, or your most recent tax return.
- Moving out of California: a lease agreement, utility bill, or other proof of your new out-of-state address.
- Marriage or household change: a marriage certificate or documentation of the change.
You’ll also need personal identification such as a driver’s license or state ID. If someone is submitting the cancellation on your behalf, the county will need written authorization from you. Complete paperwork up front prevents the back-and-forth that stretches these requests out for weeks.
Confirm the Coverage Actually Ended
The county sends a written Notice of Action specifying the date your Medi-Cal coverage terminates. That notice is your official proof, so don’t rely on a phone call to verify termination. If you haven’t received one within about 30 days, call and ask for a status update, referencing the date you submitted and any confirmation number. If you were enrolled in a managed care plan, call the plan after the Notice of Action arrives to make sure they’ve been notified. Plans sometimes continue to show you as an active member after the county has processed termination, and that mismatch can create billing problems if you see a provider. Employer Coverage
If you have access to health insurance through a job, you generally have 60 days after losing Medi-Cal to request special enrollment in that employer’s plan. What Cancellation Doesn’t Erase
Estate Recovery for Ages 55 and Up
If you or a family member received Medi-Cal at age 55 or older, canceling doesn’t wipe out what the state may eventually recover from the estate after death. The Department of Health Care Services can seek repayment for nursing facility services and home and community-based services provided on or after the beneficiary’s 55th birthday. DHCS will not pursue recovery if the beneficiary is survived by a spouse, a registered domestic partner, a child under 21, or a blind or disabled child of any age. Your 1095-B for Tax Season
Medi-Cal coverage is reported to the IRS through Form 1095-B, and the government agency sponsoring the coverage files it rather than a private insurer.
