To challenge the CNA exam in California, you apply to the California Department of Public Health (CDPH) under its equivalency pathway, prove you already have qualifying nursing education or medical training, clear a criminal background check, and then take the same two-part competency evaluation that training program graduates take. The 160 hours of classroom and clinical instruction get waived; the test does not.
Who Qualifies for the Equivalency Route
CDPH limits this pathway to people who already have relevant medical education or experience. You may qualify if you fit one of these categories:1California Department of Public Health. CNA FAQ – Equivalency
- You are currently enrolled in or have completed a Registered Nurse, Licensed Vocational Nurse, or Licensed Psychiatric Technician program and have finished at least the Fundamentals of Nursing course.
- You received medical training during military service and can document it with a DD-214 or official transcript.
- You hold an RN, LVN, or psychiatric technician license from another country.
- You hold one of those licenses from another U.S. state.
Most equivalency applicants also have to show they have worked in a paid nursing-related role within the last two years under the supervision of a licensed health professional, documented with a pay stub or W-2. If You Already Hold a CNA Certificate From Another State
You are not the person challenging the exam. California offers a separate reciprocity pathway that transfers an active, in-good-standing out-of-state CNA certification without any competency evaluation. After CDPH verifies your credentials and you clear the background check, the department issues your California certificate directly.2California Department of Public Health. Building Your Equivalency Application Package

- The Certified Nurse Assistant Equivalency/Reciprocity Application (CDPH 283E), with Section A completed for equivalency.
- Education documentation. An official sealed transcript from your nursing program is the standard proof. Students who have not graduated yet can substitute a sealed letter on official school letterhead listing equivalent training completed, confirming completion of Fundamentals of Nursing, and showing dates and hours or units. Military applicants can submit a DD-214 in place of a transcript. Foreign-educated applicants may submit a copy of their foreign transcript.
- Proof of paid nursing-related work in a facility within the last two years, unless you are a current student or graduated within the past two years.
- Fingerprint clearance. In California, complete a Live Scan and include a copy of the Request for Live Scan Services form (BCIA 8016). Outside California, submit two FD-258 fingerprint cards with a $32 check or money order payable to “The Department of Justice.”3California Department of Public Health. Live Scan providers charge a rolling fee on top of the DOJ processing fee, and that rolling fee varies by location. Allow up to 30 business days for CDPH to review the package. What the Competency Evaluation Covers
The competency evaluation has two parts, and you have to pass both.
The Knowledge Test
The knowledge test is multiple choice. If you have difficulty reading English, you can request an oral version where questions and answer options are read aloud; the oral version adds reading comprehension questions. California uses approved testing vendors, so exact question counts and time limits may vary depending on which vendor is assigned to you.
The Skills Demonstration
You perform patient care tasks in front of a nurse evaluator. Tasks are assigned randomly from a pool, hand hygiene is embedded in every exam, and each skill is scored against a checklist with designated critical element steps. Getting only the critical steps right is not enough on its own; you also need to demonstrate enough of the remaining steps to meet the overall standard for each task.5Credentia. The pool includes tasks like applying elastic stockings, assisting with ambulation using a transfer belt, counting and recording pulse and respirations, cleaning dentures, and assisting with bedpan use.6Credentia. Skills You will not know in advance which skills you will draw, so practicing all of them is the point.
Fees vary by vendor and change over time. Expect to pay separately for the knowledge test and skills evaluation, and expect the oral version to cost more than the standard written test. Payment methods depend on the vendor and testing site listed in your CDPH approval letter.
If You Fail One or Both Parts
Failing one part does not force you to retake the other. You retest only the portion you failed, with a new registration and a separate fee each time. California allows a maximum of three attempts to pass both parts.7Credentia. If you use all three attempts without passing both parts, or you exceed the allowed window, you lose the equivalency route entirely. At that point you would have to complete a state-approved 160-hour Nurse Assistant Training Program before testing again.7Credentia. After You Pass
CDPH issues your CNA certificate once it has three things on file: your application, your DOJ background clearance, and your successful exam results from the testing vendor.9California Department of Public Health.
