Start With Certified Copies of Your Marriage Certificate

Every other step depends on this document. Certified copies come only from the county clerk’s office that issued your marriage license, not from any other Kentucky county.¹ After your officiant files the completed certificate, the clerk mails one certified copy back to the couple, so most newlyweds already have one in hand. Additional certified copies are available in person or by mail for a small fee that varies by county.

Order at least three. Social Security will hold one while it processes your application, the driver licensing office needs one, and a third covers your passport or an employer request. Photocopies and the decorative ceremony certificate are not accepted at any government office.

Update Your Social Security Record First

Social Security has to come first. The Kentucky Transportation Cabinet and the IRS both pull your identity from SSA’s records, so until your name matches there, later steps stall.

You’ll file Form SS-5, the Application for a Social Security Card.² Some applicants can now submit a name-change request online; if the online option isn’t available for your situation, schedule an appointment at your local SSA office or send the application by mail.³

You need to provide:

Your certified marriage certificate, which must show both your former and new names, as proof of the name change.
Proof of identity: a current U.S. driver’s license, state-issued ID, or U.S. passport.
Proof of citizenship or immigration status if it is not already on file with SSA.⁴

Every document must be an original or a copy certified by the issuing agency. Photocopies and notarized copies are not accepted.² If you mail the paperwork, SSA returns the originals after verification, but bringing them in person lets you leave with them the same day.

In-person applications usually produce a new card in 7 to 10 business days. Mailed applications currently run two to four weeks.⁵ Your Social Security number does not change; only the name attached to it does.

Update Your Kentucky Driver’s License Within 10 Days

Kentucky law requires you to apply for a corrected license or ID within 10 days of a name change.⁶ Handle it at any Kentucky Transportation Cabinet Driver Licensing Regional Office, which replaced the circuit court clerk licensing counters that used to do this work.

Take with you:

Your certified marriage certificate from the county clerk.
Your updated Social Security card. If the physical card hasn’t arrived, KYTC will accept a Social Security Number Application Process (SSNAP) confirmation once at least 48 hours have passed since SSA approved the change.⁷
Your current Kentucky driver’s license or state ID.

Update Your Passport

Which form to use, and whether you pay, depends on how recently your current passport was issued:

Passport issued within the last year and name changed within the last year: Form DS-5504, no fee. Expedited service adds $60.⁹
More than a year since issuance or since the name change: renew by mail with Form DS-82 if you’re eligible, or apply in person with Form DS-11. Standard renewal fees apply.

Routine processing runs four to six weeks; expedited runs two to three weeks. Add up to two weeks of mailing time on each end.⁹ If you have travel booked, start as soon as your driver’s license is updated and consider paying for expedited service.

Update Your Vehicle Title and Registration

If you own a vehicle in Kentucky, the title likely still shows your former name. Bring the current title and your photo ID to your county clerk’s office to update it; the marriage certificate is acceptable proof linking the two names.¹⁰ There’s no statutory deadline like the one for your driver’s license, but a mismatch between your ID and title can cause problems during a traffic stop or a future sale.

Tell Your Employer and Handle Tax Records

The IRS doesn’t have a name-change form. It pulls names directly from Social Security, so once SSA updates its records, the IRS side is handled. The catch is that the name on your tax return has to match SSA’s file. If you changed your name partway through the tax year and haven’t updated SSA yet, file under your former name so your return isn’t delayed and your refund isn’t held.¹¹

If a W-2 arrives in your old name after you’ve already updated with SSA, ask your employer for a corrected Form W-2c. You can also write the correct name on the copies you attach to your return.¹¹ Submit a new Form W-4 reflecting your new name and any change in filing status.

Your employer should also update your Form I-9. Federal guidance says the new name goes into Supplement B of the existing I-9 as soon as the employer learns of the change, and a copy of your marriage certificate should be kept with the form for any future inspection.¹² Updating SSA before or alongside notifying your employer prevents E-Verify mismatches later.

Update Voter Registration and Everything Else

Kentucky voters can update their name through the Secretary of State’s voter registration portal or by mailing in a paper voter registration form. Do it before the next election’s registration deadline so the poll book matches your ID.

Beyond government agencies, work through the rest of your accounts:

Banks and credit card companies. Most require an in-person visit or a scanned marriage certificate and updated ID. Update anywhere your name appears on checks, cards, or loan documents.
Insurance providers. Health, auto, homeowner’s, and life insurance policies all need your legal name for claims to process cleanly.
Utility companies. Electric, water, internet, and phone providers usually accept a phone call, though some ask for documentation.
Professional licensing boards. If you hold a Kentucky license in nursing, teaching, law, or real estate, contact the relevant board. Most don’t charge for a name change.

A checklist is worth keeping. Requirements vary by organization, and many want to see the certified marriage certificate or updated driver’s license before they’ll change anything.
