Which Name Changes the Marriage License Allows

  • Replace your birth surname with your spouse’s last name.
  • Keep your spouse’s last name as your new surname and move your birth surname into the middle name slot.
  • Hyphenate the two surnames.

Bring to your appointment:

  • Your updated Social Security card.
