How to Complete and File the Nevada SR-1 Traffic Crash Report

If you were in a crash in Nevada that police did not investigate at the scene, you have 10 days to mail a completed Nevada SR-1 form, along with three attachments, to the DMV in Carson City. Miss the deadline or leave out an attachment and the report is treated as void, which can cost you your driving privileges for a year.

Do You Actually Have to File

Two conditions have to be true at the same time. The crash happened on a Nevada highway or somewhere open to the public, and it caused bodily injury, death, or at least $750 in total damage to any one person’s vehicle or property.¹ The $750 is measured per person, not across everyone involved.

There’s one exception that resolves most crashes. You don’t need to file an SR-1 if a police officer investigated at the scene under NRS 484E.110 and the officer’s report includes each driver’s insurance company name, policy number, and coverage dates.¹ If officers came out but only directed traffic or never recorded the insurance details, the exception doesn’t apply and you still have to file.

If you’re too hurt to complete the form, the duty shifts to the vehicle’s owner, who then has 10 days from learning about the crash.¹

What to Gather Before You Start

Pulling this together in one sitting is faster than restarting the form three times because a field is blank.

Your full legal name, address, driver’s license number, and date of birth.
The other driver’s name, address, and license number. If you didn’t exchange this at the scene, check any police incident card or ask your insurer, which may have it.
For every vehicle involved: VIN, license plate, year, make, and model.
The exact insurance company name and the policy number that was active on the date of the crash for each driver. This is the field the DMV cross-checks later, so accuracy matters.
Names and addresses of every passenger in every vehicle.
The date, time, location, direction of travel for each vehicle, and the point of impact.

The Three Attachments

The form itself says it will be treated as void without the required attachments.² Most rejected filings are complete forms mailed without paperwork.

A copy of your insurance card, front and back, showing the policy that was active at the time of the crash. Not a policy you bought afterward.
A repair estimate if any one person’s damage reached $750 or more. The estimate has to come from an established repair shop, a licensed insurance adjuster, or a licensed appraiser. If your insurer has already declared the vehicle a total loss, attach that written determination instead.¹
A doctor’s statement of injury for each person in your vehicle who was injured or killed.²

No injuries and damage under $750 per person? You only need the insurance card copy.

Filling Out the Form

Download the SR-1 from dmv.nv.gov. It runs two pages: page one is the facts of the crash, page two is where you sign.

Enter your details and the other driver’s details in their columns. Verify every policy number character by character. A transposed digit is the fastest way to trigger a coverage-verification problem on your own record. If the other driver was uninsured or refused to hand over information, note that in the field rather than leaving it blank.

When you describe the crash, stick to observable facts: direction of travel, approximate speed, road conditions, where each vehicle was struck. Skip opinions about fault. A small diagram helps when the narrative alone leaves the positions unclear.

Sign and date page two. An unsigned form is incomplete and comes back to you.

There is no online submission for the SR-1. Mail the form and every attachment to:

Use certified mail or delivery confirmation so you have proof it arrived within 10 days, and keep a photocopy of the entire packet. If someone later challenges whether you filed on time, that paper trail is what protects you.

What the DMV Does With It

The DMV cross-references the insurance information against the carriers you named to confirm you carried at least Nevada’s minimum liability at the moment of the crash: $25,000 for bodily injury to one person, $50,000 for bodily injury to two or more, and $20,000 for property damage.³ If coverage checks out, nothing further is required from you and the report goes into the DMV’s confidential crash records.

Penalties

Willfully failing to file when required can result in a one-year suspension of your driving privileges. The suspension stays in place until the DMV either receives the report or gets evidence you didn’t intentionally skip filing. Filing an SR-1 with information you know to be false is a gross misdemeanor.¹

If verification turns up a coverage lapse, fees run on a tiered scale from $250 for a first lapse of 30 days or less up to $1,750 for a third offense with a lapse over 180 days, and a third offense also carries a 30-day license suspension. An SR-22 is required starting at a 91-to-180-day first lapse.⁴ Once required, an SR-22 must be maintained for three continuous years from the date your license is actually reinstated, not from the date of the crash. Let it lapse and the DMV suspends your license and registration again. There is no statute of limitations on the requirement, so waiting out a suspension doesn’t erase it.⁵
