NYC Form W-532 is a wage verification request the New York City Human Resources Administration (HRA) sends to employers when it needs to confirm what a current or former employee earns. If one arrived on your desk, HRA is reviewing that person’s eligibility for public assistance, and New York State law requires you to complete the form and return it.
Why the Form Is Mandatory
Section 143 of the New York State Social Services Law requires every employer doing business in New York to furnish wage, salary, earnings, and benefits information whenever an authorized social services official asks for it. The form prints an abstract of the statute near the top so there’s no ambiguity: responding is a legal obligation, not a request for a favor.1New York State Senate. HRA uses your answers to decide whether the employee qualifies for cash assistance, Medicaid, or another program.
What to Fill In on the Front
HRA pre-fills the employee’s name, Social Security number, and date of birth. Everything else is on you. If it’s easier, you can attach a pay ledger or a computer printout of pay records in place of writing out the payroll grid by hand, as long as all the requested information is clearly shown.2NYC Human Resources Administration. Employment and Pay
- Start date, position, and (if applicable) end date and reason for leaving.
- Rate of pay, pay frequency, hours per week, and number of tax exemptions claimed.
- For each pay period HRA lists: check release date, pay period dates, gross pay excluding earned income credit, hours scheduled versus hours actually worked, and any overtime, vacation, sick, or comp pay.

Deductions
Itemize every deduction. The form’s categories include federal income tax withheld, NYC tax withheld, FICA, disability insurance, earned income credit, life insurance, bonds, credit union, IRA, pension, 401(k), union dues, health insurance premiums, and any other payroll deductions.2NYC Human Resources Administration. What to Fill In on the Back
The reverse side covers benefits. For health insurance, indicate whether the employee has coverage and whether it comes through the company or a union. If they do, supply the carrier name, policy or ID number, group number, coverage dates, the names of everyone covered, and which categories the plan includes (major medical, inpatient hospital, outpatient, dental, optical, drug and pharmacy, home care, nursing home, and senior care).

If the Employee No Longer Works for You
You still have to respond. Fill in the employment end date and reason for leaving on the front, then complete only the reverse side using the employee’s last week’s earnings. Two follow-up questions apply to former employees: whether health insurance coverage is still available, and whether the policy can be converted to an individual plan. If conversion is possible, include the cost the employee would pay. How and Where to Return It
Sign and date the employer section at the bottom. Print your name, title, company name, address, employer ID number, and phone number. Return the form to the address or fax number printed on the copy HRA sent you; different local HRA offices route these requests differently, so use the contact on your form rather than a general HRA address. The Bureau of Eligibility Verification usually handles the responses.
Send it back quickly. Section 143 does not set a deadline in days, but the statute makes the response mandatory once a social services official asks. Form W-532 vs. Form W-532T
If your form is titled “Eligibility Verification Review Questionnaire” and carries the code W-532T, it isn’t the employer version. HRA sends W-532T to the assistance applicant or recipient, not to the employer, and the person named on the case fills it out with their own income and household information.3New York City Human Resources Administration.
