Start With the Right Placard Type
Form PA-3 renews all three of Hawaii’s disability parking placards, but the process differs for each. On the form, check the “Renewing” box that matches your placard color and write in your current placard number, which appears on your blue identification card.1Hawaii Department of Health. - Blue (long-term) placard: valid for six years, renewed by mail, no fee.
- Red (temporary) placard: valid up to six months, renewed in person at a county office, $12 fee each time.
- Green DPPEP placard: allows free parking at meters for up to 2.5 hours, renewed at no cost.
Get the Medical Certification
Side 2 of Form PA-3 must be completed by a licensed physician, doctor of osteopathy (AMD), or advanced practice registered nurse. The provider certifies that you cannot walk 200 feet without stopping to rest and that the limitation results from a qualifying medical condition. They sign and date the form, print their name, and enter their Hawaii medical license number. Military physicians stationed in Hawaii may use their military license.1Hawaii Department of Health. Watch the signature window. For a blue placard, the certification is valid for 180 days from the date the provider signs it. For a red placard, it is valid for only 60 days. You fill out Side 1 yourself. Check the “Renewing” box, enter your current placard number, and provide your legal name, mailing address, date of birth, and contact information. Verify the placard number against your blue ID card, because a mismatched number can stall processing while the county tries to locate your record. Sign and date the form. If someone completes it on your behalf, that person signs as your representative and lists their relationship to you.

Red Placard in Person
Temporary red placard renewals cannot be mailed. Bring the completed Form PA-3, a valid photo ID, and the $12 fee to your county’s permit office. Timing Your Application
You can submit a renewal up to 60 days before your current placard expires.1Hawaii Department of Health. Person With a Disability Parking Permit Application Form PA-3 Plan the medical appointment so the provider’s signature stays inside its validity window when the form arrives at the county. For a blue placard, that gives you a comfortable 180 days between the signing and submission. For a red placard, work backward from the 60-day certification window and the day you plan to visit the county office.
While You Wait for the New Placard
Your current placard remains valid until the expiration date printed on it. Keep using it during processing and hold on to it until the new one arrives. Once the replacement comes, destroy the old placard or return it to the issuing agency so no one else can use it. If You Need a Replacement Instead of a Renewal
Form PA-3 also handles replacements for a lost, stolen, mutilated, or confiscated placard. Check the “Replacing” box rather than “Renewing,” and leave Side 2 blank; no new medical certification is required. Replacing a lost blue or red placard costs $12. A mutilated blue or red placard is replaced free if you return the remaining pieces. Green DPPEP replacements start at $30 and rise with each successive replacement.1Hawaii Department of Health.
