How to Complete and Submit Oklahoma’s Vaccine Exemption Form (Certificate of Exemption)

To opt a child out of required childhood vaccines in Oklahoma, a parent or legal guardian files the Oklahoma vaccine exemption form, officially ODH Form 216-A (Certificate of Exemption), with the Oklahoma State Department of Health Immunization Service. The form covers children in public and private K–12 schools, licensed childcare facilities, and Head Start programs, and it recognizes three grounds for exemption: medical, religious, and personal belief.1Justia. Oklahoma Code 70-1210.192 – Exemptions Each ground has its own signature and documentation rules, and picking the wrong combination is the main reason forms come back unapproved.

Pick the Right Type of Exemption First

The exemption type you choose determines who has to sign and what you have to write.

A medical exemption applies when a licensed physician determines that a specific vaccine would endanger the child’s life or health. The physician signs the form and identifies which immunizations the child cannot safely receive.1Justia. Oklahoma Code 70-1210.192 – Exemptions A parent’s signature alone will not do it; without the physician’s signature the form is returned.

A religious exemption is available when immunization conflicts with the teachings of the child’s religion. The parent or guardian certifies on the form that immunization is contrary to those teachings, and either a religious leader or the parent signs the religious-objection section.2Oklahoma State Department of Health. Certificate of Exemption No supporting letter from a congregation or denomination is required.

A personal belief exemption covers convictions not tied to a specific religion. The parent signs and writes a brief summary of the objection in the space provided on the form.3Oklahoma Digital Prairie. A sentence or two explaining your objection is enough.

Where To Get the Form

ODH Form 216-A is available for download at oklahoma.gov/health/immunizations. That is the only official source. County health departments and private doctors do not issue the exemption certificate, and you cannot pick one up in person at OSDH.4Oklahoma State Department of Health. Immunization Service Print the form and complete it by hand in legible writing. Entries that cannot be read will cause the form to be returned.

Completing ODH Form 216-A

At the top, fill in the child’s full legal name (last, first, middle initial), date of birth, and the name of the school, childcare facility, or Head Start program the child attends.5Oklahoma State Department of Health. Certificate of Exemption Match the name exactly as it appears on enrollment records.

Next, check which immunizations the exemption covers. The options on the form are:

  • DTaP/Td/Tdap (diphtheria, tetanus, pertussis)
  • Hepatitis A
  • MMR (measles, mumps, rubella)
  • Hib (Haemophilus influenzae type B)
  • Varicella (chickenpox)
  • Polio
  • Hepatitis B
  • Pneumococcal
  • All

Check “All” only if you are declining every required vaccine. Otherwise, check only the specific ones you are opting out of; your child will still need to show proof of immunization for anything left unchecked.5Oklahoma State Department of Health. Certificate of Exemption

Then complete the signature section that matches your chosen exemption type. For medical, the physician signs and states the medical reason. For religious, a religious leader or the parent signs the religious-objection certification. For personal belief, the parent signs and writes the brief statement of objection. A form missing the correct signature, or a personal-belief form without the written statement, will be returned unapproved.4Oklahoma State Department of Health. Immunization Service

Where To Send the Completed Form

The completed form has to reach the OSDH Immunization Service for approval. You can mail it yourself or hand it to your child’s school, which will forward it. The mailing address is:

In-person submission at OSDH is not available. Incomplete, illegible, or unsigned forms are returned to whoever submitted them.4Oklahoma State Department of Health. Immunization Service Once the certificate comes back approved, the parent gives it to the child’s school, childcare facility, or Head Start program, which keeps a copy on file. Keep your own copy too. The form itself warns that lost records are not grounds for an exemption.2Oklahoma State Department of Health. Certificate of Exemption

When You Have To File a New Form

An approved exemption is not permanent. Any exemption submitted before a student enters seventh grade expires at the end of sixth grade, and the parent must submit a new exemption request to OSDH before the child starts seventh grade.4Oklahoma State Department of Health. Immunization Service That is the one scheduled refiling point.

Outside that cutoff, you do not need to resubmit yearly. The school keeps the approved certificate on file for as long as it covers. If your child transfers to a new childcare facility, the existing approved certificate can be reused even if it lists the previous program’s name, as long as the child’s immunization status has not changed.4Oklahoma State Department of Health. What an Approved Exemption Does Not Cover

Approval lets your child attend school or childcare without the vaccines listed on the form, but it does not guarantee uninterrupted attendance during a disease outbreak. If an outbreak occurs at the school, OSDH or the local health department will review immunization records and recommend to the Commissioner of Health whether exempt students should be temporarily excluded. The Commissioner can order that exclusion for the duration of the outbreak.4Oklahoma State Department of Health.
