RITA Form 11 is the Employer’s Municipal Tax Withholding Statement that Ohio employers use to report and remit local income tax withheld from employee wages to the Regional Income Tax Agency. You complete it by listing each RITA municipality where your employees worked (and, where applicable, lived) in Section B, carrying those totals into Section A, and submitting the return with payment by your assigned due date. The details that matter most are your filing frequency, the municipality-by-municipality breakdown, and the distinction between mandatory workplace withholding and optional residence withholding.
How Often You Have to File
RITA assigns one of three schedules based on how much tax you withheld for each municipality in the prior year. The thresholds are applied per city, not across your whole payroll.1Regional Income Tax Agency. Filing Due Dates
- Quarterly filing applies if you withheld $2,399 or less for the municipality in the prior calendar year and $200 or less in any month of the prior quarter. Returns are due April 30, July 31, October 31, and January 31.
- Monthly filing applies if you withheld more than $2,399 for the municipality in the prior year, or more than $200 in any single month of the prior quarter. Returns are due on the 15th of the following month.
- Semi-monthly filing may be required if you withheld $12,000 or more for the municipality in the prior year, or more than $1,000 in any month of the prior quarter. Payments are due on the third banking day after the 15th and the third banking day after the end of the month.
Because the thresholds run per municipality, a single employer can end up filing on different schedules for different cities. Check your frequency at the start of each year; interest and penalties begin accruing the moment a deadline passes.
Completing the Form
You can download Form 11 from ritaohio.com or complete it inside RITA’s online portal. The return has two working parts: Section A for summary totals and Section B for the breakdown by municipality.2Regional Income Tax Agency. RITA Form 11 Employer Withholding Statement
Header Information
At the top, enter your FEIN, your RITA account number, the period covered, and the due date. On preprinted packets, verify that the business name and address still match your records. Mismatched identifiers are one of the fastest ways to delay processing. If anything has changed, submit the Change Notice Form from your packet.
Section B First
Fill in Section B before Section A, because Section A pulls from it. For each municipality where employees worked (or lived, if you withhold residence tax), enter the municipality name, its RITA municipal code, the workplace wages subject to tax, the applicable tax rate, and the workplace tax withheld. A municipal breakdown must accompany every Form 11; a return with only summary totals will not be accepted.3Regional Income Tax Agency. Municipal rates across RITA cities generally run between 1% and 3%. Current rates and codes for every member municipality are posted on the agency’s website under the Tax Rates and Codes listing. Apply each city’s rate to the wages earned in that city to calculate the line.
Section A Totals
With Section B complete, carry the totals forward. Line 1 is total wages subject to workplace tax. Line 2 is total workplace tax withheld. Line 3 is total residence tax withheld, if any. Line 4 is the amount due and paid, equal to the sum of lines 2 and 3.2Regional Income Tax Agency. RITA Form 11 Employer Withholding Statement The Line 4 figure must match the check or electronic payment you submit. Discrepancies between Section A and the Section B detail lines trigger administrative inquiries, so verify the math before you file.
Workplace Tax Versus Residence Tax
Ohio law requires employers to withhold municipal income tax for the city where the employee physically works. You multiply the employee’s qualifying wages earned in that city by the city’s tax rate and withhold accordingly.4Ohio Legislative Service Commission. Ohio Revised Code 718.03 – Withholding Taxes From Qualifying Wages That workplace withholding is mandatory.
Residence tax, sometimes called courtesy withholding, is voluntary. If an employee lives in one RITA city but works in another, the employee can ask the employer to also withhold tax for the city of residence. It is collected in addition to the workplace tax, not instead of it.5Regional Income Tax Agency. Business FAQs – Employer Withholding – Workplace vs. Residence On Form 11, do not report the wages themselves in the wages column for residence tax; enter only the residence tax withheld amount.3Regional Income Tax Agency. What Counts as Qualifying Wages
Qualifying wages for Ohio municipal income tax generally track the amounts subject to Medicare tax on the federal W-2. That reaches well beyond base salary. It includes bonuses, commissions, tips, deferred compensation contributions to plans like 401(k)s and 403(b)s, Section 125 cafeteria plan amounts, and non-qualified deferred compensation. If an amount appears in the Medicare wage box on the W-2, it almost certainly counts for RITA purposes unless Ohio law specifically excludes it.
The form also accommodates adjustments from prior periods. If you overpaid in an earlier month, you can apply the credit against the current period’s liability. If you underpaid, add the shortfall plus any applicable interest. For larger corrections, RITA directs employers to Form 11A, the Amended Employer’s Municipal Tax Withholding Statement.
Submitting the Return and Paying
RITA’s online portal is the most efficient route. Log in, enter the withholding data, submit the return, and pay in the same session. The system issues immediate confirmation and keeps a filing history for later reference.
Payments can be made from a checking or savings account (ACH) at no cost, or by Visa, Mastercard, or Discover credit card with a 2.75% service charge added to the tax.6Regional Income Tax Agency. Payment Options On a $5,000 payment, that fee runs $137.50, which pushes most employers toward ACH.
If you file on paper, the mailing address depends on how many municipalities you report:
- Single municipality: RITA, PO Box 94983, Cleveland, OH 44101-4983
- Multiple municipalities: RITA, PO Box 94736, Cleveland, OH 44101-4736
- Overnight delivery: RITA, 4910 Tiedeman Rd, Brooklyn, OH 44144

Penalties for Late or Missed Payments
Under Ohio law, a municipality may impose a penalty of up to 50% of the withholding tax that was not paid on time.8Ohio Legislative Service Commission. Ohio Revised Code 718.27 – Penalty and Interest The ceiling is half the tax owed, which can rival the underlying liability for an employer that falls behind.
Interest also accrues on the unpaid balance. For 2026, RITA’s annual rate is 9%, calculated as the federal short-term rate rounded to the nearest whole percent plus five percentage points.9Regional Income Tax Agency. Penalty and Interest Rates If you discover an underpayment, correct it on your next Form 11 or file Form 11A rather than waiting for the agency to raise it.
Ohio law also provides that the officer or employee personally responsible for filing and paying the withholding tax can be held personally liable for failure to do so. The exposure is not confined to the business entity; it can reach the payroll manager or owner handling remittance.
Year-End Reconciliation: Form 17
Filing Form 11 each period is only part of the obligation. By the last day of February following the calendar year, every employer that withheld municipal tax must file Form 17, the Reconciliation of Income Tax Withheld, along with copies of employee W-2s.1Regional Income Tax Agency. Filing Due Dates
Employers that issued 10 or more W-2s during the year must submit the W-2 data electronically in the EFW2 format prescribed by the Social Security Administration. RITA provides a verification tool at ritaohio.com/W-2 to validate the file before submission. Employers with nine or fewer W-2s may submit paper Copy 1 instead.10Regional Income Tax Agency. Form 17 Instructions
Form 17 must cover every municipality for which tax was withheld or should have been withheld, and its totals should match the sum of your Form 11 filings for the year. If the numbers do not reconcile, expect follow-up from RITA. Employers that issued 1099-NEC forms for non-employee compensation must also furnish copies to RITA, but those go on a separate 1099 Transmittal Form rather than with Form 17.10Regional Income Tax Agency.
