SCDMV Form RG-007B is the one-page application a South Carolina business or organization uses to get disability parking placards, special disability license plates, or both for vehicles titled in the organization’s name. You complete three short sections, attach payment, and submit it in person at any SCDMV branch or by mail to the Blythewood headquarters. No doctor’s certification is required.1South Carolina Legislature. South Carolina Code 56-3-1960 – Temporary and Permanent Placards
Who the Form Is For
RG-007B is restricted to organizations that transport people with disabilities as part of their operations, such as nursing homes, assisted-living facilities, hospitals, transit services, and nonprofits that drive people to medical appointments. The vehicles must be titled and registered in the organization’s name. You cannot use this form for a personal vehicle, even one used to volunteer-drive disabled individuals.1South Carolina Legislature. South Carolina Code 56-3-1960 – Temporary and Permanent Placards
Through one submission you can request organizational placards (one per vehicle registered in the organization’s name), special disability license plates for vehicles of special design and equipment built to transport certified disabled individuals, or both.2South Carolina Department of Motor Vehicles. People with Disabilities3South Carolina Legislature. South Carolina Code 56-3-1910 – License Plates for Handicapped Persons Individuals seeking a personal placard or plate should use Form RG-007A instead, which does require a physician’s certification.
What to Gather First
Before you start writing on the form, pull together:
- The organization’s full legal name, street address, and mailing address if different.
- The name of your liability insurance company. Use the carrier’s name, not your agent or broker.4South Carolina Department of Motor Vehicles. Organization Placard and/or License Plate for People Who Have a Disability
- For each vehicle that needs a plate: the VIN, make, year, and current license plate number.
- Payment. A check or money order made out to the SCDMV works by mail; cash is accepted in person at branches.
Organizations do not need a physician’s certification, and photographs are not required on organizational placards.1South Carolina Legislature. South Carolina Code 56-3-1960 – Temporary and Permanent Placards2South Carolina Department of Motor Vehicles. People with Disabilities

Filling Out the Form Section by Section
Section I: Type of Transaction
Check one box: Original, Replacement, or Renewal. For a first-time application, check Original. For a replacement, write in the prior placard number. The current fees are printed at the top of the form for reference.4South Carolina Department of Motor Vehicles. Organization Placard and/or License Plate for People Who Have a Disability
Section II: Business and Organization Information
Every applicant fills out this section, whether you want a placard, a plate, or both. Enter the organization’s legal name, street address, and separate mailing address if applicable. Phone and email are optional but useful if the SCDMV needs to reach you about the application.
Two certifications sit in this section and both carry weight. The first states that the listed vehicles are titled and registered in the organization’s name and used to transport people with disabilities. The second is an insurance certification signed under penalty of perjury that the vehicles are insured with the named company and will remain covered throughout the registration period.4South Carolina Department of Motor Vehicles. Organization Placard and/or License Plate for People Who Have a Disability Write the insurance company’s name on the line provided.
The owner or an authorized agent signs and dates Section II and prints their name and title. That signature covers both certifications, so the signer needs actual authority to bind the organization.
Section III: Vehicle Information
Section III is only required if you are requesting license plates. For each vehicle, enter the VIN, make, year, and current plate number. Leave the “DMV Use Only” column blank. If you are requesting placards only, skip this section.4South Carolina Department of Motor Vehicles. Organization Placard and/or License Plate for People Who Have a Disability
