The DE 4365DI is a Request for Eligibility Information that California’s Employment Development Department mails to Disability Insurance claimants when it needs to verify identity, address, or other claim details before releasing benefits. If you received one, your DI claim is on hold until EDD gets the documents listed on the form. The form tells you which documents are required and prints a specific response date. Send exactly what it asks for, by that date, and your claim moves again.

Why the Form Arrived
EDD sends the DE 4365DI when it can’t automatically confirm something on your claim. What to Send
Read the form first. You only need to provide what it specifically requests, not everything below. The categories EDD may ask about are identity, address, employment and wages, Social Security number, and date of birth.
Identity

Acceptable photo IDs include:
- State ID card or driver license
- U.S. passport, foreign passport, or passport card
- U.S. military card or military dependent’s ID card
- Permanent Resident Card (Green Card)
- Certificate of Citizenship or Certificate of Naturalization
- Employment Authorization Document
- Consular ID card
Address

- Utility bill (electric, gas, garbage, or sewer)
- Cable TV, phone, or medical bill
- Insurance statement or bank account statement
- Mortgage statement
- Property tax bill from the current tax year
- Current lease or rental agreement showing your name and address
- Government-issued document (federal, state, county, or city) with a mailing address
Employment and Wages
Send last year’s W-2, or a pay stub, payment statement, or tax form dated within the 12 months before your claim. The document must show your name, at least the last four digits of your Social Security number or employee ID, and your employer’s name. Social Security Number and Date of Birth
Social Security number verification takes three items together: a copy of your Social Security Statement (SSA-7005), proof of wages earned, and a photo ID. Date of birth verification requires a certified birth certificate or other official birth certification issued by a government entity.2Employment Development Department. How to Send It
You have two options: upload through SDI Online or mail paper copies. Online is faster and gives you immediate confirmation of receipt.
Online
Log in to your myEDD account at edd.ca.gov, open SDI Online, and use the document upload option. Make sure your scans or photos are clear and legible. Even if you verified your identity through ID.me when you filed, the DE 4365DI means something still needs confirmation, so send whatever the form specifically requests.2Employment Development Department. By Mail
Mail copies to the address printed on your DE 4365DI. Include the form itself so EDD can match your documents to your claim, and write your Social Security number on every page. Use the return envelope if one came with the form. Certified mail gives you a dated receipt if timing is ever disputed.
Send copies only. EDD does not return original documents.
The Response Deadline
Your DE 4365DI prints a specific response date. EDD’s website describes this window as roughly 10 calendar days from the mailing date,2Employment Development Department. If You Miss the Deadline
Missing the response date has real consequences. You may be disqualified from receiving disability benefits, required to repay any benefits already paid, and subject to additional penalties as required by law.2Employment Development Department. Acceptable Documents for Verification
If the form arrived late or you’re having trouble gathering documents, call EDD before the deadline passes. Explaining the situation may give you a chance to submit before your claim is acted on. Silence is what causes disqualifications.
Mistakes That Cause Resubmission
The most common problem is sending the wrong type of document. If EDD asks for identity verification and you send a utility bill, the response doesn’t satisfy the request and EDD will ask again while your benefits stay on hold.
Other errors that trigger a second round:
- Expired IDs. Both documents must be current. A driver license that lapsed last month won’t count.
- Missing the back of a card. EDD requires front and back copies.
- Illegible copies. Dark photocopies or blurry phone photos get rejected.
- Old address documents. Utility bills and bank statements must be dated within the last two months.
- Sending originals. Always send copies; EDD doesn’t return anything.
Check your documents against EDD’s acceptable documents page before submitting. A few extra minutes on the front end saves weeks on the back end.
