How to Complete and Submit the Fort Bend ISD UIL Physical Form

Before your student can practice, scrimmage, perform, or compete in a Fort Bend ISD athletics program, marching band, or cheer squad, you need to submit a completed UIL Pre-Participation Physical Evaluation form through the district’s Rank One portal. For the 2026–2027 school year, the Fort Bend ISD UIL physical form must be dated on or after April 1, 2026, and the examining provider’s stamp must appear on it for the district to accept it.¹

What’s Actually in the Packet

The “physical” is really three forms, and each has its own signing and renewal rules. Miss one and your student’s clearance stalls.

The medical history form. Required every year. Both the student and a parent or guardian must sign it before any participation.²
The physical examination form. UIL requires a fresh exam when a student enters the first and third years of high school, and for any seventh- or eighth-grader in marching band. The UIL form itself notes that local district policy may require an annual exam beyond that minimum.³⁴
The sudden cardiac arrest awareness form. Required every year, signed by both student and parent or guardian.⁵

One physical covers both athletics and marching band; you don’t need separate exams for each activity.³ And even in a year when UIL doesn’t require a new physical, certain “yes” answers on the medical history form trigger one anyway. Prior hospitalization, surgery, heart-related symptoms, concussion, or seizures all require written provider clearance before your student can participate.⁴

Texas Education Code Section 33.096 requires the district to tell you that your student can request an electrocardiogram alongside the standard physical. An ECG is optional; the school cannot require one.⁶

Where to Download the Current Form

Fort Bend ISD posts the correct year’s form on its Athletics page under “Physical Form – UIL.”¹ If the district hasn’t updated its copy yet, the standard UIL version is on the UIL athletics forms page.⁵ Print both the medical history section and the physical examination section. Use only the current year’s version; an outdated form gets rejected.

Filling Out the Medical History at Home

Do this part before the doctor’s appointment. The physician relies on it to focus the exam, and rushing it wastes the visit. The form asks about hospitalizations in the past year, prior surgeries, and allergies to medication, food, pollen, or insect stings.⁷ List every current medication, prescription and over-the-counter, including inhalers.

Take the cardiac section seriously. It asks whether any family member died from heart problems or had an unexpected death before age 50, and whether any relative has been diagnosed with enlarged heart, hypertrophic cardiomyopathy, or long QT syndrome. Your student’s own questions cover chest pain during exercise, fainting or near-fainting during activity, and unexplained shortness of breath.⁷ Sudden cardiac arrest in young athletes is often linked to undiagnosed inherited conditions, so answer honestly.

The form also covers concussions, seizures, asthma, and other chronic conditions. Both student and parent or guardian sign at the bottom, certifying the answers are complete and correct; the signature line warns that false responses can result in UIL penalties.⁷

Who Can Perform the Physical

UIL limits the exam to four types of providers:

Physicians (MD or DO)
Physician assistants licensed by a State Board of Physician Assistant Examiners
Advanced practice registered nurses recognized by the Board of Nurse Examiners
Doctors of chiropractic

A form signed by anyone else, including a school nurse, paramedic, or athletic trainer, will not be accepted.⁴ Confirm the provider’s credentials before you book.

What the Provider Fills Out

The examiner completes the physical exam side: height, weight, blood pressure, pulse, vision, and a systems check of the heart, lungs, abdomen, joints, and neurological function. At the bottom, the provider marks one of three clearance outcomes:

Cleared to participate without restrictions.
Cleared after completing evaluation or rehabilitation for a specific issue.
Not cleared, with the reason stated on the form.⁴

Fort Bend ISD requires the provider’s stamp on the form along with a printed name, signature, and exam date.¹ If the office doesn’t use a stamp, ask for the practice name, address, and phone number written legibly. A stamp is the safest way to keep the form from being kicked back.

What the Physical Costs

Walk-in and urgent care clinics in the Houston area typically charge around $40 for a basic UIL physical, more if an ECG or additional testing is added. Your student’s pediatrician or family doctor may bill insurance for a well-child visit that includes the sports physical, but a standalone sports physical is not guaranteed to be covered as preventive care.⁸ Some community organizations hold low-cost physical events before the school year starts; your campus athletic coordinator will know about local options. If you pay out of pocket, the cost counts as a deductible medical expense on your federal taxes.⁹

Uploading to Rank One

Once the provider has cleared and stamped the form, upload it through the Fort Bend ISD Rank One portal at fortbendisd.rankonesport.com. Scan or photograph both sides of the completed document and confirm every signature, date, and stamp is legible before you submit through your student’s profile.¹⁰

The campus athletic trainer then reviews the upload for correct signatures, the provider stamp, an exam date on or after April 1, 2026, and a clearance status that allows participation. Check your student’s status in Rank One and look for a green clearance indicator. If anything is missing, the trainer flags the account and you’ll need to correct and re-upload. No student may participate in any practice, scrimmage, performance, or game until clearance is finalized.⁴

Submit early. Waiting until the week before the first practice creates a bottleneck; trainers are reviewing hundreds of forms at once, and any issue with your submission means your student sits out while you scramble to fix it. Get the physical done in April or May and upload it the same week.

One More Piece of Eligibility

A cleared physical only handles the medical side. Texas Education Code Section 33.081 also requires students to maintain a grade of 70 or above in every academic class on a 100-point scale to stay eligible for extracurricular competition, checked at each six-week reporting period. A student who falls below 70 in any class is suspended from competition for at least three weeks, though practices can continue during the suspension. Advanced Placement, International Baccalaureate, honors, and dual-credit courses in core subjects are exempt.¹¹
