How to Complete and Submit the Kentucky FS-2 SNAP Review Form

The Kentucky FS-2 SNAP review form, formally called the Mid-Review Notice, is the short check-in that households on 12- or 24-month SNAP certifications must complete partway through their certification period to keep benefits running. The Cabinet for Health and Family Services mails a reminder when yours is due, and you can file it through the kynect benefits portal, by mail, by fax, or in person at your local Department for Community Based Services (DCBS) office. Miss the deadline and your case closes, so the date on the notice matters more than any other detail on the form.

When the FS-2 Is Due

Timing depends on how long your certification period runs. Kentucky assigns SNAP households to 4-, 12-, or 24-month certifications. If you’re certified for 12 months, the FS-2 is due during your sixth month. If you’re certified for 24 months, it’s due during your twelfth month.1Kentucky Legislative Research Commission. 921 KAR 3:035 – Certification Process Households on a four-month certification skip the mid-review entirely and go straight to a full recertification.

Your specific due date is printed on the notice the Cabinet sends you and shows on your kynect dashboard. If you plan to file online, note that the system cuts off at 5:30 PM Eastern on the due date. After that, the SNAP Mid-Review button disappears and electronic submission is no longer available.2Cabinet for Health and Family Services. What the FS-2 Asks

The Mid-Review is not a full reapplication. It’s a set of yes-or-no questions about whether anything in your household has changed since you were last certified. The form covers five areas:

  • Contact information: whether your phone number, email, or mailing address has changed.
  • Income: whether anyone in the household started or stopped a job, changed employers, had a change in wage rate or full-time/part-time status, or had a change in unearned income of more than $50.
  • Resources: whether cash on hand, checking accounts, savings accounts, or other liquid assets have changed.
  • Expenses: whether rent, mortgage, utilities, or child support obligations have changed.
  • Household members: whether anyone has moved in or out.

Answer “yes” to any item and you’ll identify which household member the change applies to and enter the new details. The caseworker uses what you report to recalculate your benefit for the rest of the certification period.2Cabinet for Health and Family Services. Quick Reference Guide SNAP Mid-Review

If your household includes someone 60 or older or someone with a disability, out-of-pocket medical costs above $35 per month that aren’t covered by insurance can be claimed as a deduction.3Food and Nutrition Service. Filing the FS-2 Online Through kynect

The quickest route is kynect.ky.gov. When your review is due, a notification banner and a “SNAP Mid-Review” button appear at the top of your Resident Dashboard. Click the button to start.

The system walks you through each category one screen at a time. For each one, choose “Yes” or “No.” A “Yes” opens a follow-up where you pick the household member affected and enter the new information. If you clicked “Yes” by mistake and actually have nothing to update, select “No Changes” on the follow-up screen to move on.2Cabinet for Health and Family Services. At the end you reach the signature page. Read the SNAP Mid-Review Rights and Responsibilities, click “I Agree,” type your first name, middle initial, last name, suffix if applicable, and the date, then click “Submit SNAP Mid-Review.” A confirmation screen tells you the submission went through.2Cabinet for Health and Family Services. Filing a Paper FS-2

If you’d rather use paper or don’t have reliable internet, three options are open to you. Get proof of delivery whenever you can.

  • Mail: send the completed form to DCBS, P.O. Box 2104, Frankfort, KY 40602. Certified mail gives you a tracking number as evidence of timely filing.
  • Fax: send it to (502) 573-2007.
  • In person: bring it to your local DCBS office during business hours and ask for a date-stamped receipt.

These are the standard SNAP document channels listed on Kentucky’s application materials.4AWS. Kentucky Application for SNAP The head of household or an authorized representative must sign the paper form; an unsigned form is treated as incomplete.

If a caseworker later asks for supporting documents, you can upload them through the kynect Document Center. The portal accepts phone photos, so a scanner isn’t necessary.5kynect. kynect benefits Document Upload Quick Reference Guide

What Happens After You Submit

Once the Cabinet has your form, a caseworker checks the information against existing records. With no reported changes and nothing flagged, your benefits continue at the same amount through the end of the certification period. If you did report changes, the caseworker recalculates your allotment and mails a notice telling you whether your monthly amount goes up, goes down, or stays the same.

When something needs proof, the agency sends a Request for Information asking for specific documents such as pay stubs, a lease, or utility bills. Respond quickly. The federal rule gives households 10 days from the date the state mails a reminder notice to provide what’s needed.6eCFR. 7 CFR 273.12 – Reporting Requirements Uploading through the kynect Document Center or dropping documents at your local DCBS office is the fastest way to close out the request.

If You Miss the Deadline

Missing the due date sets off a defined sequence. The state mails a reminder notice giving you 10 extra days to turn in a completed report. If you file within that window, the agency processes the review and issues benefits no later than 10 days after the normal issuance date. If you still don’t respond, your SNAP case is terminated.6eCFR. 7 CFR 273.12 – Reporting Requirements

Termination for a missed mid-review isn’t permanent, but getting back on takes either reapplying or completing the overdue report and any verification the agency wants. The coverage gap can run for weeks. The simplest fix is to handle the FS-2 as soon as it shows up on your dashboard rather than waiting on the deadline.

If Your Benefits Are Reduced or Ended

When the Cabinet lowers or ends your benefits based on what you reported, you can request a fair hearing. Kentucky follows the federal timeframe for filing and allows an additional 30 days past that deadline if you can show good cause for the delay.7Kentucky Legislative Research Commission. 921 KAR 3:070 – Fair Hearings

File the request before the effective date of the reduction shown on your notice and your benefits continue at the prior level while the hearing is pending. Continuation is automatic unless you waive it.7Kentucky Legislative Research Commission. 921 KAR 3:070 – Fair Hearings One caution: if the hearing decision goes against you, the state can recover the extra benefits you received during that period. A hearing request is worth filing when you believe the agency miscalculated your income or misread your report, but using it to buy time can leave you with an overpayment to repay.
