To have North Carolina child support payments deposited straight into your bank account, complete the NC Child Support Direct Deposit Form (DSS-4718) and mail it to NCCSE–EFT, PO Box 19807, Raleigh, NC 27619. The PDF is available from the North Carolina Department of Health and Human Services, and payments route electronically through the State Child Support Collection and Disbursement Unit once the agency verifies your account.1North Carolina General Assembly. What to Gather Before You Start
Have all of this in front of you before you open the form:
- Your MPI number. This is your Master Participant Index number assigned by NC Child Support Services, up to 10 digits long. You can find it on any correspondence from the program or by logging into the eChildSupport portal.3North Carolina Department of Health and Human Services. Login Help – NC Child Support Services
- Your full Social Security Number.
- Your bank’s name, phone number, and full address.
- The nine-digit routing number and your account number.
- Supporting documentation that depends on the account type. For a checking account, attach a voided preprinted check; starter checks are not accepted. For a savings account, your bank must complete the verification section on the form itself.4North Carolina Department of Health and Human Services. NC Child Support Direct Deposit Authorization
If you have a checking account but prefer not to send a voided check, you can have the bank complete the verification section instead, the same way a savings account is handled.
How to Fill Out the Form
The form is a single page with three numbered sections. At the top, print your last name, first name, middle initial, Social Security Number, and MPI number.4North Carolina Department of Health and Human Services. NC Child Support Direct Deposit Authorization
Section 1 asks you to check the type of request. If you’re setting up direct deposit for the first time or switching bank accounts, check “Start/Change Direct Deposit.” Check either “Checking Acct” or “Savings Acct” to show where the money should land. Then fill in the bank name, phone number, address, routing number (nine individual boxes), and account number. Write every digit legibly. One misread number can send your payment to the wrong account or get the request rejected.
Section 2 is for your bank to complete. A bank representative fills in the routing number, account number, and the bank’s name and address, then signs and stamps it. This section is mandatory for savings accounts. It’s optional for checking accounts when you’ve attached a voided check.
Section 3 is your authorization signature. Signing and dating here authorizes NC Child Support Enforcement to deposit payments into the account you listed. Everyone completes this section regardless of account type.4North Carolina Department of Health and Human Services. NC Child Support Direct Deposit Authorization

Don’t confuse this with the NC Child Support Centralized Collections address (PO Box 900006), which is for mailing payments, not for the direct deposit form. The eChildSupport portal also lets custodial and noncustodial parents upload documents to their accounts, which may be an option for submitting this form electronically.5North Carolina Department of Health and Human Services. How Long Until Deposits Start
After Child Support Services receives the form, the agency contacts your bank to verify the routing and account numbers before activating electronic deposits. This verification is sometimes called a pre-note: a zero-dollar test transaction sent through the Automated Clearing House network to confirm the account exists and can receive funds. No money moves during the pre-note; it only checks the electronic path.
NCDHHS says to allow three to four weeks for direct deposit to take effect from the date the agency receives your form.2North Carolina Department of Health and Human Services. Electronic Funds Transfer/Electronic Data Interchange During that window, payments keep arriving by whatever method you were using before, usually a paper check. Once verification clears, all future distributions switch to electronic deposit automatically. Watch your bank account for the first deposit to confirm the setup is active.
Changing or Stopping Direct Deposit
If you switch banks or need to update your account number, complete a new DSS-4718 and check the “Start/Change Direct Deposit” box. Attach the right documentation for the new account (voided check for checking, bank verification for savings) and mail it to the same NCCSE–EFT address. A new verification period applies each time you change accounts, so expect the same three-to-four-week window before deposits reach the new account.2North Carolina Department of Health and Human Services. To stop direct deposit entirely, check the “Stop Direct Deposit” box on the form, sign it, and send it to NCCSE–EFT.4North Carolina Department of Health and Human Services. Keep your mailing address on file current so paper checks reach you without interruption.
