How to Complete and Submit Your Virginia Tech Immunization History Form

Every incoming student must submit the Virginia Tech Immunization History Form, signed by a licensed healthcare provider, before starting classes. Undergraduates entering in the fall have until June 30; graduate students have until August 20. Spring entrants submit by January 5 (undergraduate) or January 15 (graduate).1Virginia Tech. What Vaccines You Need to Document

Virginia Tech’s requirements are:

  • MMR: two doses given at least one month apart, with the first dose on or after 12 months of age. Lab proof of immunity or physician-documented proof of disease also works, except for rubella, where proof of disease is not accepted.
  • Tetanus/diphtheria: a booster within the 10 years before you arrive. A plain Td booster counts; it doesn’t have to be Tdap.
  • Polio: a completed primary series with the last dose at age four or older.
  • Hepatitis B: a completed three-dose series, or a signed waiver.
  • Meningococcal vaccine: required for all entering students, or a signed waiver.

Students born before 1957 are exempt from the MMR requirement. If you want to show immunity to hepatitis B, meningococcal disease, or varicella through a blood test (titer) rather than a vaccination record, attach the lab results.2Virginia Tech. Virginia Tech Immunization History Form

How to Complete the Form

Start by pulling your vaccination records. Your pediatrician’s office, high school, previous college, or military service branch should have them. If your old doctor’s office has closed, check whether your state health department keeps an immunization registry.

Download and print the form from the Schiffert Health Center website. Put your full name and Virginia Tech ID at the top. For each vaccine, enter the date of each dose. You have two options for showing the dates: write them on the form itself, or attach an official immunization record signed or stamped by a qualified provider. Either way, every required vaccine has to appear somewhere.2Virginia Tech. Virginia Tech Immunization History Form

The form also includes a Tuberculosis Risk Assessment and, if you’ll be under 18 when you arrive, a Consent for Treatment of Minors section that a parent or legal guardian needs to sign.2Virginia Tech. Virginia Tech Immunization History Form

A licensed healthcare provider has to sign the immunizations section. If that’s not practical, attach a copy of your immunization record signed or stamped by a physician (or designee), registered nurse, or health department official.2Virginia Tech. The TB Screening Step

Every incoming student completes the TB Risk Assessment, a short questionnaire about travel history, exposure to active TB, and other risk factors. It appears on the paper form, but you also complete the TB screening online through the Healthy Hokies Portal.3Virginia Tech. Required Immunizations

Answer “yes” to anything on the risk assessment and you need to document a TB test, either a PPD skin test or a QuantiFERON Gold blood test. The test has to have been performed on or after March 1 for fall entry, or on or after July 1 for spring entry. A prior BCG vaccination does not exempt you from testing. A positive test triggers a required chest X-ray, plus documentation of whether treatment was recommended.3Virginia Tech. Required Immunizations

If you go with the PPD skin test, the form wants three dates: when it was placed, when it was read, and the reading in millimeters. Attach the result either way.2Virginia Tech. Virginia Tech Immunization History Form

What Happens if You Miss the Deadline

The $100 late charge posts first.2Virginia Tech. Incoming Students Under Virginia law, a student who hasn’t furnished an immunization history isn’t eligible to register for the second semester.5Virginia Code Commission. Virginia Code 23.1-800 – Health Histories and Immunizations Required; Exemptions

To clear a hold, call Schiffert Health Center’s Health Information Management office at 540-231-8104 and find out exactly what’s missing. Once you submit the rest and it’s processed, the hold comes off.6Virginia Tech. Medical Holds on Student Schedules

Medical and Religious Exemptions

Virginia law allows two kinds of exemption. A medical exemption applies if a licensed physician states that one or more vaccines would harm your health. A religious exemption applies if vaccination conflicts with your religious beliefs or practices.7Virginia Code Commission. Virginia Code Title 23.1 Chapter 8 Article 1 – Student Health Virginia Tech has a separate form for each; both are on the Schiffert Health Center website.3Virginia Tech. Required Immunizations

One trade-off to know about. If an outbreak or potential epidemic of a vaccine-preventable disease occurs at Virginia Tech, the state health commissioner can order an exempted student excluded from campus until the danger passes.3Virginia Tech. Required Immunizations

International Students

If certain vaccines were not available in your home country, submit the records you do have through the Healthy Hokies Portal and complete the rest after arriving in the United States. Virginia Tech waives the $100 late fee for international students in this situation, provided you finish the outstanding immunizations by the start of classes. The waiver applies only to international students who genuinely could not access certain vaccines abroad, not to domestic students traveling overseas.1Virginia Tech. Incoming Students

The TB screening matters here too. If you lived in or traveled through countries with higher TB rates, you still complete the risk assessment and, if indicated, provide a test result, even if you received a BCG vaccination as a child.3Virginia Tech. Required Immunizations
