Who Has to File the DR 2058
Colorado requires an interlock-restricted license before you can get any other license reissued if your license was revoked for a BAC of 0.15 or higher, your record designates you as a persistent drunk driver, you were revoked as a habitual offender with a DUI or DWAI in the mix, or you were revoked for circumventing an interlock device.3Justia Law. What to Have Ready Before You Sign
The DR 2058 itself is short, but the DMV processes it as part of a larger reinstatement file. Pull the rest together first, or the affidavit will sit unused.

Filling Out and Notarizing the Form
Download the DR 2058 from the Colorado DMV’s forms page, or pick one up from your interlock provider.6Department of Revenue – Motor Vehicle. Your signature certifies under oath that you have obtained a signed lease agreement for an approved ignition interlock device for each vehicle where your name appears as owner or co-owner, and for any other vehicle you may drive during the restriction period.1Colorado Department of Revenue. The affidavit must be notarized. A notary public witnesses your signature and completes the block at the bottom of the page with their seal, signature, and commission expiration date.1Colorado Department of Revenue. You will need a registered myDMV account and a clear scan or photo of the signed, notarized form. If Your Vehicle Situation Changes
If you filed a DR 2056 non-ownership affidavit and later buy a vehicle or gain regular access to one, you have to install an interlock device, sign a lease agreement, and file the DR 2058 with the DMV for the remaining duration of your restriction.3Justia Law. After You File: Staying in Compliance
Getting the restricted license issued is not the end of the process. The interlock device must be serviced every 60 days. Miss a service appointment and the DMV can suspend your license until you are back in compliance.7Department of Revenue – Motor Vehicle. For the entire restriction period, you may drive only vehicles equipped with a properly functioning interlock device. Compliance also matters for how long the restriction lasts. If your monthly monitoring reports show the device blocked your vehicle for excessive breath alcohol in three out of any twelve consecutive reporting periods, the DMV adds another twelve months to your restriction.3Justia Law. Penalties for Driving Without the Device or Tampering
Driving a vehicle without an approved interlock device while your license is restricted, or circumventing the device in any way, is a class 1 traffic misdemeanor. A conviction triggers mandatory revocation of your interlock-restricted license for one year or the remaining period of your original license restraint, whichever is longer.3Justia Law. Tampering with the device, or helping someone else tamper with one, is a class 2 misdemeanor. That covers intercepting, bypassing, or interfering with the device’s operation. Even without a criminal conviction, the DMV can revoke your license based on administrative records alone if they show you operated a vehicle without the device or tried to circumvent it.3Justia Law.
