Arizona Department of Transportation Motor Vehicle Division (ADOT MVD) forms are free PDF downloads from the MVD Forms Library at azdot.gov, and most can be filed online through AZ MVD Now, mailed to the Phoenix processing center, or handed across the counter at a field office.1Arizona Department of Transportation. MVD Forms Library The library lists well over a hundred forms, but a small group covers almost every situation an Arizona driver or vehicle owner runs into.
The Forms Most People Actually Need
- Title and Registration Application (96-0236) when you buy a vehicle, transfer ownership, or register a vehicle in Arizona for the first time.
- Bill of Sale (48-2004) to document a private sale between individuals.
- License/Identification Application (40-5122) for a driver license, commercial license, motorcycle endorsement, learner’s permit, state ID, or an Arizona Travel ID upgrade.2Arizona Department of Transportation. License/Identification Application (40-5122)
- Power of Attorney (48-1001) to let someone else sign title paperwork for a specific vehicle when you cannot appear in person.
Specialty forms for aircraft registration, dealer licensing, fleet registration, and disability placards live in the same library and can be searched by form number or keyword.
Title and Registration Application (96-0236)
Arizona law gives you fifteen days from the purchase date to apply for a certificate of title. Licensed dealers get thirty.3Arizona Legislature. Form 96-0236 asks for the vehicle description, VIN, odometer reading, and new owner information. Individuals fill in a driver license number; a business uses its Federal Employer Identification Number. The ownership designation at the top matters more than it looks. “OR” between co-owners means either person can transfer the vehicle alone. “AND” means both signatures are required, and if one owner dies, that share moves through probate before the vehicle can be sold.5Arizona Department of Transportation. For a private-party purchase, attach the signed title from the previous owner and a completed Bill of Sale. The application also carries a certification that you accept the odometer reading the seller provided and that MVD is not responsible for verifying it.5Arizona Department of Transportation. Odometer disclosure is required for vehicles of model year 2011 and newer, for twenty years from the model year. Older vehicles, trailers, vehicles with a gross weight rating of 16,000 pounds or more, and new vehicles bought solely for resale are exempt.6Arizona Department of Transportation. Bill of Sale (48-2004)
The Bill of Sale records who sold what to whom and for how much. Buyer and seller each provide name, driver license number, date of birth, and mailing address. The vehicle section takes the VIN, year, make, and body style. The seller’s signature must be notarized or witnessed by an MVD agent; without that step, MVD will not accept the form.7Arizona Department of Transportation. Write the sale price clearly in blue or black ink. Ambiguous digits invite processing delays and tax disputes because Arizona uses the sale price to calculate applicable taxes. If the vehicle is a gift, write $0 or “gift” in the amount field. Keep a copy. The original goes to MVD with the title application.
License/Identification Application (40-5122)
One form covers most credential transactions: first-time license, renewal, replacement, upgrade to a Travel ID, commercial driver license, motorcycle endorsement, learner’s permit, and state ID card. Documents to Bring
Arizona requires three categories of proof at the counter:
- Proof of identity and authorized presence: a certified birth certificate, valid U.S. passport, permanent resident card, certificate of naturalization, or unexpired USCIS employment authorization document.
- Social Security number: your Social Security card or another document showing the full number, such as a W-2.
- Two proofs of Arizona residency, each showing your name and physical residential address. Utility bills, bank or credit card statements, rental agreements, and mail from MVD itself all qualify.9Arizona Department of Transportation. Arizona DL/ID Requirements
Originals or certified copies only. Photocopies and phone screenshots are not accepted.
Arizona Travel ID
Federal REAL ID enforcement began May 7, 2025, and a standard Arizona license is no longer accepted for boarding domestic flights or entering certain federal facilities.10Transportation Security Administration. REAL ID The Arizona Travel ID is the state’s REAL ID–compliant credential. You apply on the same Form 40-5122, but you must bring the full identity, Social Security, and residency document set. A valid U.S. passport works at airport checkpoints if you prefer not to upgrade.
Power of Attorney (48-1001)
Form 48-1001 appoints an attorney-in-fact to sign title paperwork and handle MVD transactions for one specific vehicle. It lists the vehicle description and the names of both the owner and the person receiving authority. The owner’s signature grants authority to “sign all papers and documents required to secure the title, and further grant the authority to endorse and transfer title thereto.”11Arizona Department of Transportation. This is the form military members stationed out of state use, along with families managing a deceased relative’s vehicle and buyers or sellers who cannot meet in person. The authority is tied to the vehicle listed. It does not extend to your other MVD business.
Where to Get the Forms
Every current MVD form is a free PDF from the official Forms Library. You can search by form number if you know it or browse by keyword.1Arizona Department of Transportation. MVD Forms Library Paper copies are available at MVD field offices and at Authorized Third Party providers, which are private businesses licensed by the state to process MVD transactions.12Arizona Department of Transportation. Authorized Third Party Providers Skip unofficial sites that charge for forms ADOT gives away, since some post outdated versions MVD will reject.
How to Submit
Online Through AZ MVD Now
The AZ MVD Now portal at azmvdnow.gov handles a growing list of transactions with no office visit: registration renewal, plate replacement, license or ID application, title replacement, eTitle transfer, and appointment scheduling.13AZ MVD Now. AZ MVD Now Express services like registration renewal, restricted-use permits, and voter registration do not require an account. Everything else does.
In Person
Field offices are open Monday through Friday, 7:30 a.m. to 5:00 p.m., with limited services after 4:45 p.m. On the second Wednesday of every month, offices open at noon.14Arizona Department of Transportation. The advantage of the counter is that an agent can spot a problem on the spot instead of mailing your paperwork back weeks later.

Include copies of every supporting document. ADOT warns applicants to allow up to six weeks for title processing by mail.15Arizona Department of Transportation. Vehicle Title Keep copies of everything until your final documents arrive. If something is lost in transit, your copies are the only proof of what you sent and when.
What Gets Forms Rejected
Every rejection resets the clock, and the same handful of errors cause most of them.
- Choosing “OR” when you meant “AND” (or the reverse) on Form 96-0236. This changes who can transfer the vehicle later and is hard to fix after the title is issued.
- Sending in a Bill of Sale without notarization or MVD-agent witnessing on the seller’s signature.
- Filing the title application past the fifteen-day deadline, which triggers the escalating late penalty.4Arizona Legislature. Arizona Code 28-2162 – Delinquent Registration; Penalty; Lien; Failure to Apply
- Illegible handwriting on the VIN, odometer reading, or sale price. Block letters, blue or black ink.
- Skipping odometer disclosure on a 2011-or-newer vehicle that is less than twenty years old, or writing “exempt” on a vehicle that does not qualify.
Check every field against the existing title before you seal the envelope or hand it across the counter. Fixing one wrong digit later costs far more time than getting it right the first time.
