Iowa Form 411007 is the Application for Certificate of Title and/or Registration, the single form you file with any county treasurer to title and register a vehicle in Iowa. Get the paperwork together first, fill out the form once, and file within 30 days of your purchase date.
What to Gather Before You Fill Anything Out
The county treasurer processes Form 411007 alongside a stack of supporting documents. Missing one item stops the transaction.

- The 17-character VIN, copied exactly. A single transposed digit will cause rejection.

- A Damage Disclosure Statement (Form 411108) if the vehicle is eight model years old or newer, completed by the seller.1Iowa Department of Transportation. Title a Vehicle
- Proof of identity. The treasurer may ask for a driver’s license, state ID, or Social Security card.2Iowa Department of Transportation. Register a Vehicle
- Lienholder name and address if you financed the purchase.
Buying new from a dealer? The dealer usually handles most of this for you.
Filling Out the Form
The top of Form 411007 captures the vehicle’s identifying details: year, make, model, body style, and VIN. Copy the VIN character by character from the title or the door jamb sticker rather than from memory. The form also asks whether you’re applying for a regular title, a salvage title, or registration only. Check the box that fits your transaction.
The owner section takes the full legal names and residential addresses of every person who will appear on the title. If two people will co-own the vehicle, the conjunction between the names carries legal weight. Names joined with “and” mean both owners must sign to sell or transfer the vehicle later. Names joined with “or” mean either owner can authorize a transfer alone. Pick the one that matches how you want future transactions to work. If a bank or credit union holds a lien, enter the lienholder’s information in the lien section. The finished title will be mailed to the lienholder, not to you, until the loan is paid off and the lien is released.
Odometer and Damage Disclosures
Federal law requires the seller to provide a written odometer disclosure during most vehicle transfers, certifying whether the reading is accurate, has exceeded the odometer’s mechanical limits, or does not reflect actual mileage.5eCFR. Two categories are exempt. Vehicles with a gross vehicle weight rating above 16,000 pounds do not need a mileage disclosure.6eCFR. 49 CFR 580.17 – Exemptions Vehicles 20 or more model years old are also exempt. For the 2026 calendar year, that means model year 2006 and older.
The Damage Disclosure Statement is a separate Iowa requirement. Form 411108 must accompany Form 411007 whenever the vehicle is eight model years old or newer, and the seller reports whether the vehicle sustained damage above the statutory threshold.1Iowa Department of Transportation. Where to File
File the completed application with any county treasurer’s office in Iowa. Despite the common assumption that you must go to your county of residence, Iowa Code 321.20 allows initial registration and title applications to be submitted to any county treasurer.7Iowa Legislature. Most counties handle title transactions in person during business hours. Some, including Dallas County, require an appointment; others take walk-ins.3Dallas County, IA. What You’ll Pay
Two charges apply to every transaction:

- The fee for new registration, a one-time 5% charge on the purchase price of each newly registered vehicle under Iowa Code 321.105A. This is separate from Iowa’s 6% sales and use tax.8Iowa Department of Transportation. Changes to the Vehicle Title Certificate
You’ll also owe the annual registration fee, which varies by vehicle weight and age. Most offices accept cash, checks, and credit cards. Some add a surcharge for card payments, so ask when you schedule.
The 30-Day Deadline
Iowa law gives a new owner 30 calendar days from the date of purchase to apply for a new title and registration.9Iowa Legislature. Miss the window and penalties are assessed automatically.10Polk County Iowa. Title Transfers If the annual registration fee becomes delinquent while you delay, you’ll also owe the delinquent registration amount prorated from the date it first came due to the month you finally apply.9Iowa Legislature. Iowa Code 321.46 – New Title and Registration Upon Transfer of Ownership The penalties alone are modest, but stacked with back registration on an older vehicle, the total climbs. Filing on time costs the least by a wide margin.
Titling a Vehicle from Another State
If you bought the vehicle out of state or are moving to Iowa, the checklist stretches a little. Along with Form 411007 and a bill of sale, surrender the out-of-state title with the assignment section properly completed by the seller.1Iowa Department of Transportation. Title a Vehicle Form 411108 is still required if the vehicle is eight model years old or newer, and you’ll need proof of Iowa insurance meeting the state’s financial liability coverage requirements.
Update your auto policy to reflect your Iowa address before you visit the treasurer’s office; your insurer needs to issue a card showing the Iowa address. The 30-day clock starts when you establish residency, so don’t wait for your old state’s registration to expire.
After You File
When you file in person, the treasurer typically hands you the registration receipt and license plate or renewal sticker at the counter. Mailed applications, or plates that need to be ordered, arrive later. Keep the temporary registration documents in the vehicle until the permanent items show up.

If a lienholder is listed, the certificate is mailed to the lienholder rather than to you.
If the treasurer’s office finds a problem, whether a missing signature, a VIN mismatch, or an incomplete damage disclosure, they will contact you with the specific correction needed. You usually only need to fix the flagged item and resubmit that portion, not start over.
