Missouri Form 5862, the Certification of Homeless Status and Residency, lets you apply for a Missouri nondriver ID without the utility bills, leases, or bank statements the state normally requires as proof of address. You fill out the top half, an eligible homeless services provider fills out the bottom half and signs, and you bring the completed form to a license office with your other application documents.1Missouri Department of Revenue. What the Form Replaces
Form 5862 only substitutes for address verification. Everything else the Department of Revenue requires still applies: proof of identity, proof of lawful status in the United States, and your Social Security number. If your legal name differs from the name on your identity document, you also need paperwork tying the two together.2Missouri Department of Revenue. You cannot complete the form by yourself. The certification requires a signature from a qualifying agency, and a license office will reject the form without one.1Missouri Department of Revenue. Form 5862 – Certification of Homeless Status and Residency
Who Can Sign as the Agency
Missouri recognizes four types of homeless services providers for Form 5862 purposes, and each must have a Missouri address:1Missouri Department of Revenue. Form 5862 – Certification of Homeless Status and Residency
- A Missouri governmental agency or nonprofit organization that serves homeless individuals or unaccompanied youth.
- A Missouri-licensed attorney employed by a nonprofit who represents you in any legal matter.
- A local education agency liaison for homeless children and youth designated under 42 U.S.C. Section 11432(g)(1)(J)(ii), or a school social worker or counselor.
- A Missouri human services or public social services provider that receives state funding to deliver homeless services, health or mental health services, substance use disorder services, or public assistance and employment services.
A friend or relative letting you stay at their place does not qualify, no matter how long you have been there. If you are not already connected with an eligible provider, start by contacting a local shelter or a school homeless liaison if you have school-age children.
Filling Out Your Section
Form 5862 is one page. Download it from dor.mo.gov/forms/5862.pdf. In Section I you enter your first, middle, and last name, your date of birth, and a Missouri residence address where you can be reached. For most applicants that will be the agency’s or shelter’s address. Sign and date the section.1Missouri Department of Revenue. Form 5862 – Certification of Homeless Status and Residency
By signing, you are acknowledging two things that matter later. Your nondriver ID will be mailed to the agency, not to you directly. And you have 60 days from the agency’s receipt of the card to pick it up before the agency has to send it back to the Department of Revenue.1Missouri Department of Revenue. What the Agency Fills In
Section II is for the agency employee. They enter the organization’s name, full mailing address, and phone number, along with either the agency’s Federal Tax ID number or, for an attorney signing on behalf of a nonprofit, their attorney registration number. The employee prints their name, signs, and dates the form. Their signature certifies that you are experiencing homelessness at the time of certification and reside in Missouri.1Missouri Department of Revenue. Before you leave the agency, check that the mailing address in Section II is one you can actually get to. That is where your ID is going.
What Else to Bring to the License Office
Form 5862 handles only the address requirement. Bring original documents in the remaining categories; photocopies generally are not accepted.2Missouri Department of Revenue. Documents for Driver License, Nondriver ID, and Instruction Permit
- Proof of identity and lawful status, such as a certified U.S. birth certificate from a vital records agency with an embossed, stamped, or raised seal, a U.S. passport, or an immigration document. Hospital-issued birth certificates and birth registration cards do not count.
- Proof of your Social Security number, usually your Social Security card. The card must be signed if you are 18 or older and cannot be laminated. If no SSN has been assigned to you, bring a letter from the Social Security Administration about your SSN status.
- If your name has changed, a certified marriage license, court order, or other legal document linking your current name to the name on your identity document.
Gathering these can be the hardest part. The agency signing your Form 5862 may be able to help you request a replacement birth certificate or Social Security card before your license office visit, so ask them while you are there.
Fee
The form does not waive the application fee. The Class ND nondriver ID costs $24.3Missouri Department of Revenue. The Missouri Driver License and Nondriver ID
Submitting the Form and Picking Up Your ID
Take the completed Form 5862 and your supporting documents to any Missouri license office in person. The form cannot be mailed separately. Staff will keep it as part of your application file.1Missouri Department of Revenue. Form 5862 – Certification of Homeless Status and Residency
You will not walk out with the card. Because you used Form 5862, the ID is mailed to the agency address in Section II. Once it arrives, you have 60 days to collect it. If you miss that window, the agency is required to mark the envelope “Return to Sender” and send the card back to the Department of Revenue, and getting another means applying all over again.1Missouri Department of Revenue. Form 5862 – Certification of Homeless Status and Residency Stay in touch with the agency after your license office visit and pick up the card as soon as they let you know it has arrived.
One Limitation on the Card You Will Receive
A REAL ID-compliant nondriver ID (the one with a star in the upper right corner) requires two address-verification documents from two different sources.4Missouri Department of Revenue. Missouri REAL ID Information Form 5862 counts as one. If it is the only address document you can produce, you will receive a non-compliant card marked “NOT FOR REAL ID PURPOSES.” That card still works as everyday identification, but starting May 7, 2025, a REAL ID-compliant document is required to board domestic flights and enter certain federal buildings. If you need REAL ID, ask the agency whether you can also produce a second qualifying address document from a different source.
