Pennsylvania Form UC-1609 is the one-page notice a Pennsylvania employer gives to a departing worker at the time of separation, containing the employer identifying information the worker will need to file an unemployment compensation claim. What’s On the Form

- Employer legal name, as it appears on the PA-100 Enterprise Registration.
- PA UC account number or FEIN.
- Employer address, or the address of a third-party administrator if one handles your unemployment claims. This is where the department will mail its notices, including the Employer’s Notice of Application (UC-45), fact-finding forms, and determinations.
- Contact person, title, phone, and email for follow-up on separation information.
- Employee name.
- Last four digits of the employee’s Social Security number (not the full number).
- Whether the employer expects the employee to return to work, and if so, an estimated return date in MM/DD/YY format.
- Signature and date of the employer representative completing the form.

How Employers Complete and Distribute It

The PA UC account number is the single most important entry. When it goes into the worker’s application, the department can immediately route correspondence to the correct employer address and pull the right wage records. Take the recall question seriously. If you check “Yes” and give an estimated return date, the worker can be exempted from job-search requirements until that date. When the Notice Is Required
Section 206.1 of the Pennsylvania Unemployment Compensation Law requires every employer, whether or not they pay UC contributions, to notify separating employees about the availability of UC benefits at the time of separation.4Pennsylvania Department of Labor and Industry. The statute specifies what the notification must cover:
- That UC benefits are available to workers who are unemployed and meet the law’s requirements.
- That the employee can file a claim in the first week employment stops or hours are reduced.
- That assistance is available through the department’s website or a toll-free number, which the employer must provide.

Form UC-1609 covers all of that, which is why the department recommends it. Give it to the worker at the moment of separation. Waiting until the employee asks often means they’ve already filed without your account number, slowing things down for everyone.
How Workers Use the Form to File
If your employer just handed you a UC-1609, it’s a reference sheet for your unemployment application. You don’t mail the form itself. You enter the employer information from it into your claim.
File online at benefits.uc.pa.gov, which is open around the clock, or call 1-888-313-7284 Monday through Friday from 8 a.m. to 4 p.m.5Commonwealth of Pennsylvania. You’ll need your employer’s name, address, and phone number, along with your dates of employment, plus your own Social Security number and any work-authorization documents that apply.5Commonwealth of Pennsylvania. Never got a UC-1609? You can still file. The form isn’t a prerequisite. Give whatever employer details you have, starting with the company’s legal name and address, and the department will work to match your claim to the right account.
What Happens After Filing
Once the claim is in, the department cross-references your employer entries against its records of employer accounts and wage data. If everything matches, you’ll receive a Notice of Financial Determination (Form UC-44F) by mail, listing each employer and the wages they reported in each quarter of your base year.6Department of Labor and Industry. If you’re financially eligible, that same notice sets your weekly benefit rate, partial benefit credit, maximum benefit entitlement, and any dependent allowance.6Department of Labor and Industry. If the employer data you entered doesn’t match departmental records, the claim goes to manual review, which can add weeks. A correctly copied UC account number is the easiest way to avoid that.
Why Accurate Information Protects the Employer
Handing over a properly completed UC-1609 is a legal obligation, and it also protects your account. When the worker files using your correct account number and address, the department sends claim notices to you or your TPA directly, so you can respond promptly to requests for separation information. When the employee files without the account number, notices can go to the wrong address or match the claim to the wrong employer entity. By the time you realize it, the response window may already have closed. A few minutes with the form at separation can prevent months of undoing incorrect charges.
Keeping a Copy
Save a copy of every UC-1609 you distribute. Federal employment-tax recordkeeping rules require employers to retain employment tax records for at least four years.7Internal Revenue Service. If a charge dispute or audit comes up, a signed copy shows the department you gave the worker the required notification and the correct information to file with.
