Texas Medical Board Form L is the Verification of Postgraduate Training and Professional Evaluation that a qualifying physician at each institution where you trained or practiced must complete and send directly to the Board. You do not fill it out, and you do not submit it. Anything Form L-related that comes from your own hands, email, or fax gets rejected.1Texas Medical Board. Form L Physician Licensure Evaluation
Who Is Allowed to Complete Form L
The evaluator must hold one of four positions at the institution: Chief of Staff, Department Chairman, Medical Director, or Training Director. A letter from a direct supervisor, a generic institutional verification form, or a standard letter of recommendation will not substitute.1Texas Medical Board. Form L Physician Licensure Evaluation
You need a separate Form L from every institution where you trained or practiced. This is where applications stall. Reach out early, especially to programs you left years ago, programs that have changed leadership, or programs that have closed. Some residency programs charge an administrative fee to process board verifications; $75 to $125 is common.
What Information the Form Collects
The top of the form captures your full legal name, your TMB ID number (you receive this when you start your application), and your date of birth. It does not ask for a Social Security Number.1Texas Medical Board. Form L Physician Licensure Evaluation
The evaluator then records your affiliation with the institution:
- Department of affiliation
- Position held (intern, resident, fellow, faculty, staff, or other)
- Start and end dates in mm/yy format
- Credit received: full, partial (with the number of months specified), or still in progress
Incomplete postgraduate years must be reported separately from completed ones. If you received only partial credit for a year, the evaluator has to say exactly how many months you earned.1Texas Medical Board. Form L Physician Licensure Evaluation
The Professional Evaluation
Form L is not just a training ledger. The evaluator rates you in four areas: professional ability, attention to duties, breadth of education, and interpersonal skills.1Texas Medical Board. Form L Physician Licensure Evaluation
They also have to disclose whether, to their knowledge, you have ever been involved in fraud or dishonesty, unprofessional conduct, disciplinary action by a licensing agency, arrest or criminal charges, malpractice claims, or loss of a controlled substance permit. A “yes” is not an automatic bar to licensure, but the evaluator will be asked to add details and the Board weighs the response in its review.
The “Unusual Circumstances” Questions
For training positions, Form L includes ten additional questions aimed at anything that could affect how the Board credits your time:1Texas Medical Board. Form L Physician Licensure Evaluation
- Leave of absence or break from training
- Resignation from training
- Limitations placed on you for professionalism or behavioral issues
- Written warning or documented counseling
- Probation
- Current investigation
- Reduction, suspension, or revocation of privileges or duties
- Delayed promotion or advancement
- Notice that your contract would not be renewed
- Suspension, termination, or dismissal
A “yes” to any of these triggers a request for more information. If your history includes one of these situations, tell the evaluator what you disclosed on your own application before they fill out the form. Inconsistencies between your self-report and the Form L response are what prompt Board follow-up and extend your timeline.
How the Institution Must Submit It
The completed form has to come straight from the institution to the Board. There are three accepted methods:1Texas Medical Board. Form L Physician Licensure Evaluation
- Mail. The evaluator places the form in an official hospital or institution envelope, seals it, and signs across the sealed flap. The signature across the flap is the authentication; no separate seal or stamp is required. Send to Texas Medical Board, MC-240, P.O. Box 2029, Austin, TX 78768-2029.
- Fax. The form must go with an official hospital or institution coversheet. Without the coversheet, or if the fax comes from you, it will not be accepted.
- Email. The institution emails the form directly to the Board. Emails from the applicant’s address are not accepted.
Confirm with the program administrator that they include the “MC-240” mail code in the address. That code routes the document to the right department, and leaving it off delays processing.2Texas Medical Board. Form L
Tracking Form L After It’s Sent
Once your licensure application is active, the TMB creates an account for you in the Licensure Inquiry System of Texas (LIST). LIST is the Board’s primary way of communicating with applicants: there is a message center for notes from Board staff and a separate tab listing items still needed.3Texas Medical Board. If a Form L arrives missing information, with an unsigned envelope flap, or with details that conflict with your application, the Board flags it in LIST. Check the portal often. The Board does not call applicants to chase deficiencies. When something is flagged, go back to the institution and have them submit a corrected form. Your application will not move forward until every Form L on file clears.
Where Form L Fits in the Application
Form L is one piece of a larger online application that walks you through identification, address, training and work history, and professional history. Several questions in the application point you to supplemental forms that third parties have to submit on your behalf; Form L is the most common of these.4Texas Medical Board. Forms
The documentation standards for physician licensure, including the Form L requirement, are set out in Texas Administrative Code Title 22, Part 9, Section 163.5.5eLaws. Make sure you and every evaluator are using the current version; outdated ones may be returned.
