To complete California Form DE 2501, you fill out Part A with your personal, employment, and wage-continuation details, and your treating physician or practitioner completes Part B certifying that your condition prevents you from working. What to Have on Hand Before You Start
The form moves faster when you gather these first:
- Your Social Security number.
- The name, address, and phone number of your last or current employer, plus the same for any second employer.2Employment Development Department. Application for Disability Insurance Benefits
- The exact last date you worked for each employer before the disability began.
- Your treating physician or practitioner’s full name, address, phone number, and license number.
- Any sick leave, vacation, PTO, or other pay your employer has continued or will continue during your disability. This affects your benefit calculation, so it needs to be accurate.
You do not need to calculate your own base period or benefit amount. The EDD pulls your wage history automatically from roughly 5 to 18 months before your claim start date.3Employment Development Department. Filling Out Part A: Your Claimant Statement
Part A is the section you complete yourself. On SDI Online, you answer the same questions in a guided sequence; on paper, you write each entry in by hand.2Employment Development Department. Application for Disability Insurance Benefits
After the standard personal information, the form asks about your employer. Enter the business name, address, and phone number, then the exact last day you actually worked. If you had a second employer, use the separate block for that job. State government employees should give the address of their personnel office and list the agency name rather than a smaller department subdivision.
You then describe your regular occupation and select the physical demands of your job from a scale that runs from mostly sitting up through constantly lifting over 100 pounds. Next you choose the reason you stopped working: illness, injury, pregnancy, layoff, voluntary quit, or other.
The wage-continuation questions matter more than they look. If your employer is paying you sick leave, vacation, PTO, or any other wages during your disability, you have to report it here, because those payments can reduce your weekly benefit. Underreporting can trigger an overpayment later, and if the EDD decides you left it out on purpose, the overpayment is treated as fraud, carries a 30 percent penalty on top of what you owe, and the penalty must be repaid separately from any benefit offsets.5Employment Development Department. One field asks whether the EDD may share your benefit payment information with your employer. Answering “no” keeps the dollar figures private. It does not, however, hide the fact that you filed a claim: the EDD is required by law to notify your employer that a claim was filed regardless of how you answer.4California Legislative Information. Getting Part B Certified by Your Physician
Your claim is not complete until your treating physician or practitioner fills out Part B. In that section, the medical professional certifies that you cannot perform your regular work, provides a diagnosis, estimates when you can return to work, and enters their license number and contact information.2Employment Development Department. The handoff works differently depending on how you filed Part A. If you submitted Part A online, the system gives you a receipt number. Pass that number to your physician. They then log into SDI Online and pull up your claim using the receipt number or the last four digits of your Social Security number, and submit their certification electronically.6Employment Development Department. For a fully paper claim, Part A and Part B are typically mailed together after both are complete. Either way, Part B must reach the EDD within 49 days of the date your disability began.7Employment Development Department. When and How to Submit
The filing window is narrow. You cannot file the DE 2501 earlier than nine days after your disability begins, and you must file no later than 49 days after it begins.1Employment Development Department. Filing after day 49 can cost you benefits entirely, though the EDD will accept a late claim if you attach a written explanation for the delay.
Filing Through SDI Online
Create a myEDD account at edd.ca.gov and complete identity verification through ID.me. That verification is required before you can reach SDI Online.8Employment Development Department. Filing by Mail

After the DE 2501 Reaches the EDD
Expect information by mail in about two weeks, though times vary.2Employment Development Department. Application for Disability Insurance Benefits During that window, the EDD verifies your medical certification and your earnings history, and sends the required claim-filed notice to your most recent employer.9California Legislative Information. Every new claim carries a seven-day unpaid waiting period. Benefits start on the eighth day of your disability.3Employment Development Department. Disability Insurance – Benefits and Payments FAQs Your weekly amount is roughly 70 to 90 percent of the wages from your highest-paid base-period quarter, depending on income. For claims starting in 2026, the weekly minimum is $50 and the maximum is $1,765, and benefits can run up to 52 weeks.11Employment Development Department. If your disability lasts longer than your initial certification, you have to keep certifying. Claimants who are not on automatic payment receive a Claim for Continued Disability Benefits (DE 2500A) every two weeks. Sign, date, and return it through SDI Online or by mail within 20 days, but not before the ending date printed on the form, since you cannot certify for days that have not yet happened. Claimants on automatic payment instead receive a Disability Claim Continued Eligibility Questionnaire (DE 2593) after about 10 weeks and have 20 days to return it. If your recovery takes longer than your physician originally estimated, your physician has to submit a Physician/Practitioner’s Supplementary Certificate (DE 2525XX) through SDI Online or on paper to extend the claim. Benefits stop at whatever date the physician last certified, so an extension needs to be in before that date passes.13Employment Development Department. Common Mistakes That Delay or Denied Claims
Three problems account for most of the trouble people run into with the DE 2501. The first is timing: filing before day nine, or letting Part B slip past day 49. The second is wage-continuation reporting on Part A, where leaving out sick leave, vacation, or PTO creates overpayments that can be classified as fraud if the EDD concludes the omission was intentional, adding a 30 percent penalty on top of the amount owed.14Employment Development Department. Benefit Overpayments FAQs The third is the physician handoff: when the receipt number never reaches the physician, or the physician does not use SDI Online and does not know a paper certification is an option.
One boundary worth naming. The DE 2501 covers non-work-related conditions only. If your illness or injury arose out of your job, workers’ compensation is the correct system, not SDI.1Employment Development Department. Am I Eligible for Disability Insurance Benefits? And most self-employed workers are not covered by SDI at all through payroll, so a DE 2501 will not pay out unless you previously enrolled in Disability Insurance Elective Coverage through Form DE 2565.15Employment Development Department.
