Georgia DFCS Form 809 is the employment verification form your caseworker uses to confirm your wages and job status when you apply for SNAP, Medicaid, or TANF.1Georgia Department of Human Services. What the Form Is For
Form 809 verifies income from an employer. Georgia’s SNAP policy lists it alongside pay stubs and electronic income tools like Truv as an accepted way to prove gross earned income. If you work for yourself, this is not your form. DFCS may mail you the form, hand it to you at an appointment, or ask you to download it. The Part You Fill In
Your section is short. At the top of the form, fill in the date, your DFCS case number, your caseworker’s name, your full name, your Social Security number, and the employer’s name. Copy the case number exactly from your appointment letter or notice.
Directly below the header is the authorization line: “I hereby authorize my employer to furnish complete information about my earnings to DFCS.”1Georgia Department of Human Services. Employment Verification Form Print your name, sign, and date. If you sign with an “X” rather than a written signature, a witness must sign and date next to your mark. That’s the entirety of what you complete. Hand the form to your employer as soon as you sign.
What Your Employer Will Fill In
Everything below the authorization is for payroll or HR to complete. They’ll answer twelve short questions about your job: start date, job title, first paycheck date and amount, rate of pay, weekly hours, pay frequency, any raise or reduction coming up, and, if you’ve left the job, the separation reason, last pay date, and final check amount. They’ll also fill in an earnings table listing recent pay periods, hours worked, gross pay, net pay, and tips. The form tells employers not to include advance Earned Income Tax Credit payments in the gross column.1Georgia Department of Human Services. With a payroll report in hand, the whole section takes about five minutes.
Getting the Form Back from Your Employer
The paperwork isn’t the delay. Waiting on your employer is. The form itself asks the employer to return it within five days.3Georgia Department of Human Services. Employment Verification Form Hand it to payroll or HR the same day you sign it, and follow up in two or three days if you haven’t heard back. Larger companies process these routinely. Smaller employers sometimes hesitate; it helps to mention that a state agency is requesting it and that most of the answers come straight off a payroll report.
If your employer refuses or stalls, tell your caseworker before the deadline runs out. DFCS can sometimes accept recent pay stubs or electronic income verification in place of Form 809.2Georgia Division of Family and Children Services. 3035 Verification
Before you send it back, check the form yourself. Every field the employer filled in should be legible. The signature, printed name, title, and date at the bottom should all be there. Caseworkers reject incomplete forms, which sends you back to payroll for a second try. Confirm the gross earnings column doesn’t include advance EITC payments; if it does, ask for a corrected copy.
Submitting the Completed Form to DFCS
Upload the finished form through your Georgia Gateway account for the fastest processing. Log in, open your case, and attach a scan or clear photo of every page. Keep a copy for yourself. If DFCS later has questions about what was reported, you won’t need to chase down another version from your employer.
Deadlines and Missing Them
Georgia’s SNAP policy gives you at least ten calendar days from the date of the verification request to return Form 809 and any other documents your caseworker asked for. If you need more time, ask before the deadline; the agency can extend it, but all verification has to be in within sixty days of your application date.5Georgia Division of Family and Children Services. 3105 Application Processing
Submitting after the initial ten-day window but before the sixtieth day can still get you approved, though benefits are prorated from the date DFCS receives your documents rather than backdated to when you first applied.5Georgia Division of Family and Children Services. 3105 Application Processing Missing the verification window entirely is the most common reason SNAP applications are denied in Georgia. A denial means starting over with a new application.
